Physical Form: Students taking four or more credit hours are asked to submit a physical form dated within one year of attendance, signed by a physician or advanced practice professional. This is a recommendation for non-athletes — student athletes are required to submit a physical and should contact their athletic trainer for specific requirements. Submit your physical form by mail using the green Health Services envelope from your admission package, or fax to 919-635-3778.
