In Brief:
Overall, for common high-volume surgical procedures, operative times and patient complexity have significantly increased over time. There does not appear to be empirical evidence of efficiency gains that would justify an efficiency adjustment as proposed in the Calender Year 2026 Medicare Physician Fee Schedule.

In Brief:
This national study of patients with early-stage pancreatic ductal adenocarcinoma affirms that operative resection, in the absence of multimodal therapy, is associated with improved outcomes compared with no resection. However, the association with improved survival is clinically annulled in the absence of a negative-margin resection.

In Brief:
A comprehensive departmental opioid reduction toolkit reduced discharge opioid prescriptions by 67.1% by introducing several low cost, scalable interventions aligning patient prescriptions with need. We demonstrated a 3.4-fold increase in opioid-free discharges, reduced average prescription sizes, and reduced odds of postdischarge prescriptions.
