I think doing a survey like this is a really good idea. We have an account at our disposal (as the PSF) at http://www.surveymonkey.com/ which we use for PyCon feedback. Maybe we can leverage that, if we can come up with good questions?

Following the example given in the original article, I was considering a single freeform question: "why did you stop contributing after your last patch to CPython?" (of course, that text should be decorated with a greeting and an introduction and the wording can be improved).
