The first thing that comes to mind is that your session expired and you need to log-in again. After logging in myself I see the form in all of it's glory.

I went there last night, and I was able to get a form. I kept that tab open over night, and tried to submit this morning. When I did that, I got permission denied errors. It seems that something weird has happened to my account, or bug tracker itself changed in my sleep. Anyone have any idea what's going on here? Daniel
