I believe San Gwann means St John in Maltese, thus the flag is canting as it shows the paschal lamb, a usual attribute of St John the Baptist.

Similarly, the coat-of-arms of Puerto Rico - originally named Island of San Juan Bautista de Puerto Rico - has the Paschal Lamb sitting over a red Book of Revelations (with seven seals instead of three). The question is, why is the Book of Revelations, [written by St John the Evangelist] associated with St John the Baptist?
