Description of the flag
On line, in a local official document, it says:
diseño de la bandera de la Región San Martín
meaning «design of the flag of the San Martín region», which suggests that there is a regional flag in San Martín.

Though the flag was adopted informally in 2015, it was not until August 2, 2017 that the flag was adopted as official by Regional Ordinance N� 013-2017-GRSM/CR. For whatever reason, the official adoption of this flag was quite delayed.

Earlier flag
San Martin used the triangular red-white flag, although I don't have photo evidence. Before that it had a vertical green, white, green flag (like Nigeria).
