The base of the combustion chamber of a Simon-Carves boiler had become choked with displaced bricks and ashes. Deceased went down into the ashpit to clear the obstruction through an inspection door chamber. The boiler was being fed with pulverised coal, and suddenly the pit was filled with flames, having been blown back. He scrambled out of the pit, and jumped into a tank of water.
