It was used by Frodo in Lórien of Galadriel, the joint ruler of that land with her consort Celeborn. As such the usage seems to be informal, and simply an abbreviated way of saying 'Lady of the Elves of Lórien'. Though Galadriel was certainly one of the most powerful and important Elves in Middle-earth, Frodo's use of the general title 'Lady of the Elves' does not imply that she held authority over all of Middle-earth's Elves.
