Background & Importance: Hydatid disease of the spine is rare but constitutes approximately 50% o... more Background & Importance: Hydatid disease of the spine is rare but constitutes approximately 50% of bone involvement cases in human hydatidosis. It is a severe condition associated with a high rate of morbidity, disability and mortality. Case Presentation: In the present paper, we report an intriguing case of cervical spinal hydatid disease mimicking traumatic burst fracture. Conclusion: A high degree of suspicion combined with good-quality neuroimaging is important for early and correct diagnosis. Because of the location and the potential for progression, spinal hydatid disease surgical treatment is the mainstay of treatment.

Background & Aim: Spinal cord injury (SCI) is one of the worst kinds of traumatic injuries with r... more Background & Aim: Spinal cord injury (SCI) is one of the worst kinds of traumatic injuries with remarkable social and economic effects on communities. Methods & Materials/Patients: In this prospective randomized clinical trial, 122 patients with traumatic spinal cord injury were admitted to Poursina hospital within 48 hours of injury to compare granulocyte colony stimulating factor (G-CSF) and high-dose methylprednisolone as neuroprotective therapy. Results: In this research, 122 patients were studied out of whom 62 patients were included in the granulociote colony-stimulating factor (G-CSF) group with a mean age of 40.4, and 60 patients in the methylprednisolone group with an average age of 40.10 years. 55 out of 122 patients (45%) were completely paralyzed (Grade AIS:A) and 67 (55%) were with incomplete spinal injury (Grade AIS:B,C,D). The average American spinal injury association (ASIA) sensory scores in the two groups were similarly compared in the same four time intervals, and p values were 0.7, 0.3, 0.2, and 0.1. They were not statistically significant. Conclusion: According to the results, the G-CSF was shown to be beneficial in average ASIA motor and sensory scores in the two groups of male patients at six months of onset of G-CSF treatment (p value=0.04), and average ASIA motor scores improvement in the falls subgroup patients also differed significantly within six months of onset of G-CSF treatment (p value=0.03). A multicentre prospective randomized clinical trial to compare the placebo effect with G-CSF protocol's treatment, and also assessment of the cost benefits of the common medical treatment versus G-CSF are needed.

Pneumorrhachis (PR), which involves the entrapment of air or gas within the spinal canal, is a ra... more Pneumorrhachis (PR), which involves the entrapment of air or gas within the spinal canal, is a rare clinical entity. The pathogenesis and etiology of this uncommon entity vary and may present a diagnostic challenge. Air in the spinal canal can be divided into primary and secondary PR, extra-or intradural PR and etiologically classified as iatrogenic, traumatic and non-traumatic. PR is typically asymptomatic but can be symptomatic-by itself or by its underlying pathology. The latter, although often severe, might be concealed and must be carefully examined to facilitate adequate patient treatment. Here we report a rare case of traumatic PR in a 28-year old man following a motor vehicle accident in northern Iran, who sustained paraplegia and sphincter dysfunction without any laceration and wound in the spinal area.

Introduction: Cutaneous metaplastic synovial cyst (CMSC) is a rare cutaneous lesion characterized... more Introduction: Cutaneous metaplastic synovial cyst (CMSC) is a rare cutaneous lesion characterized by a tender subcutaneous nodule, which usually occurs at the site of previous surgical or local trauma. Histologically, the lesion includes a cystic structure with villous-like projections and a lining mimicking hyperplastic synovium. Case Presentation: We reported the first case of CMSC which developed at the surgical incision site of treatment of a maxillofacial fracture. In addition, we reviewed English literature to evaluate all previously reported CMSC cases and discussed its clinical and histopathological features and etiology. From 1987 to now, reviewing the English literature about CMSC includes 17 studies that described 28 cases, and our presented case was the 29th. There was no sex predilection and age of patients ranged from 7 to 82 years, but most of them were over 40 years. We can see this lesion in any site of the body and hand/arm is the most prevalent involved region (28% cases). Most of the patients had a history of previous local trauma or operation in the involved area. Conclusions: Although the actual etiology of CMSC remains unclear, trauma, as the most probable etiologic factor, plays a role in development of CMSC. Surgical excision of the lesion is the preferable treatment choice and rate of recurrence is low.

Lund and Milroy 3 have shown that mucocele remnant retains respiratory epithelium with normal mucociliary clearence after marsupialization, which allows physiologic sinonasal function. However, the need for a complete mucosal cavity removal is still controversial. In summary, the endoscopic approach has several benefits, which are mainly better cosmetic outcomes and and lower complication rates. However, in a review of 250 cases of osteoplastic flap technique for the treatment of mucoceles by Hardy and Montgomery, 18 19% of patients had complications including 13 patients with abdominal infection and 7 patients with cerebrospinal fluid leaks. CONCLUSIONS Endoscopic procedures for mucoceles have a number of advantages including the lower rate of morbidity. This technique can be performed easily in an outpatient clinic under local anesthesia; thus, the patients with an underlying medical problem or with a high risk for general anesthesia are good candidates for the endoscopic procedure. Morever, this technique allows the better visualization of the surgical area both preoperatively and postoperatively and also provides a better preservation of the anatomic structures and nasal mucosa. If performed by experienced surgeons, an endoscopic surgery should be considered as an only procedure, specifically for the treatment of complicated mucocele cases.

Papillary carcinoma of the thyroid is the most common differentiated malignancy of thyroid, which... more Papillary carcinoma of the thyroid is the most common differentiated malignancy of thyroid, which is characterized by slow progression and a good prognosis. Being extremely rare, brain metastases from papillary thyroid carcinoma (PTC) have a poor prognosis; they are usually asymptomatic, but a few could be very symptomatic and life threatening. Case Presentation: We report a rare case of brain metastasis from PTC with sudden loss of consciousness and normal follow-up screening tests, 17 years after primary diagnosis of PTC. The patient underwent radical surgery urgently and the histopathologic diagnosis of metastatic brain tumor of thyroid papillary carcinoma was made. Postoperative course was uneventful, and the patient's neurologic deficits improved. Discussion: An early diagnosis and treatment of brain metastasis of PTC can result in longer survival time for patients and magnetic resonance imaging can be used as the most sensitive method for detecting brain metastases. The treatment modalities must be individualized and surgery could be a more suitable treatment option in case of hemorrhagic metastasis.

In addition, resection of the mandibular periosteum may have resulted in poor blood supply to the reimplanted bone, resulting in bone absorption. In conclusion, it seems that decalcification and lyophilization of resected tumorous bone have the possibility of long-term preservation with osteoinductive potential as well the devitalization of tumor cells. However, further investigation is needed to clarify the mechanisms of the bone-loss pattern over the long term.

The influence of heat treatment on microstructure and mechanical properties of 3.5Ni steel has be... more The influence of heat treatment on microstructure and mechanical properties of 3.5Ni steel has been studied in this paper. The experiment results showed that the microstructure and grain of the normalized and tempered steel exhibited a low uniformity, which deteriorated toughness. The grain could be refined after quenching and tempering, and the carbides were recognized dispersing at grain boundaries, the low temperature toughness was improved. After secondary quenched and tempered, the size of grains became smaller, and the low temperature toughness was better.
