Serum triglycerides, total cholesterol, high-density lipoprotein (HDL), low-density lipoprotein (... more Serum triglycerides, total cholesterol, high-density lipoprotein (HDL), low-density lipoprotein (LDL), and very-low density lipoprotein (VLDL) cholesterol levels were quantified in Stage I and Stage IV breast cancer patients in order to evaluate the changes in serum lipids and lipoproteins in the early and advanced stages of the disease. When compared with data from age-matched healthy females, fasting serum triglycerides and VLDL cholesterol levels were found to be significantly increased and HDL cholesterol levels significantly decreased in patients with breast cancer. Furthermore, a significant increase in triglycerides and VLDL cholesterol and decreases in total, HDL and LDL cholesterol levels were demonstrated in patients with Stage IV disease when compared to those with Stage I breast cancer. No significant difference was found in total and LDL cholesterol between Stage I breast cancer patients and healthy controls.

We also documented the refusal of second endoscopy offers and if available the final diagnose. All patients were performed gastroscopy by same endoscopist. Results: In this period 83 patients with gastric ulcer underwent repeat gastroscopy. Average numbers of endoscopy was 2.15. In nine patients, 3 gastroscopy procedures were needed where as only two patients were in need of fourth gastroscopy and no one needed fifth. Seventy-seven of 83 patients were ulcer free at repeat endoscopies. But in 6 (7.2%) patients were shown to have malignant ulcer. Four of them were diagnosed after second endoscopy but two patients' malignancies could be diagnosed at third procedure. Five of the patients had gastric adenocancer and the other single case had gastric lymphoma. Two of 5 (40%) adenocancer were early gastric cancer. Conclusion: Our study re-enforced the recommendation that all patient with gastric ulcers should undergo repeat endoscopy to exclude malignancy. The malignancy rate was 7.2% in our cases. This high rate must encourage clinicians and endoscopist to insist on repeating endoscopy to assess ulcer healing.

The mechanism of impaired glucose metabolism that develops in most patients with pancreatic cance... more The mechanism of impaired glucose metabolism that develops in most patients with pancreatic cancer (PC) is obscure. The association between PC and diabetes is controversial. Impaired glucose tolerance (IGT) or diabetes mellitus may develop as a clinical manifestation of PC; however diabetes may be a predisposing risk factor for PC. We aimed to investigate the relationship between diabetes and PC, and also the impact of tumor removal on glucose metabolism. Materials & methods: Eighteen pancreatic cancer patients with resectable tumors and without previous diabetes history were enrolled. All patients were performed oral glucose tolerance test with insulin levels before and after whipple procedure. Results: Eight of 18 (44.4%) patients were diabetic before surgery where as four (22.2%) had IGT. Only 6 (33.3%) of patients had normal glucose metabolism at first clinical admission. After pancreatectomy only 4 (22.2%) patients were diabetic and 1 (5%) patient had IGT. Thirteen patients (72%) had normal glucose metabolism after tumor removal. In 8 patients, impaired glucose metabolism improved after surgery. Only one patient out of 6 (16%) with normal glucose metabolism initially developed IGT after surgery. All patients with diabetes and IGT had hyperinsulinemia before and after surgery. Even insulin levels were lower after surgery, glucose metabolism was improved.

The association between PC and diabetes is controversial. Impaired glucose tolerance (IGT) or diabetes mellitus may develop as a clinical manifestation of PC; however diabetes may be a predisposing risk factor for PC. We aimed to investigate the relationship between diabetes and PC, and also the impact of tumor removal on glucose metabolism. Materials & methods: Eighteen pancreatic cancer patients with resectable tumors and without previous diabetes history were enrolled. All patients were performed oral glucose tolerance test with insulin levels before and after whipple procedure. Results: Eight of 18 (44.4%) patients were diabetic before surgery where as four (22.2%) had IGT. Only 6 (33.3%) of patients had normal glucose metabolism at first clinical admission. After pancreatectomy only 4 (22.2%) patients were diabetic and 1 (5%) patient had IGT. Thirteen patients (72%) had normal glucose metabolism after tumor removal. In 8 patients, impaired glucose metabolism improved after surgery. Only one patient out of 6 (16%) with normal glucose metabolism initially developed IGT after surgery. All patients with diabetes and IGT had hyperinsulinemia before and after surgery. Even insulin levels were lower after surgery, glucose metabolism was improved.
