Objectives-To investigate differing patterns and associations of osteoarthritis of the knee in pa... more Objectives-To investigate differing patterns and associations of osteoarthritis of the knee in patients referred to hospital. Methods-Two hundred and fifty two consecutive patients (161 women, 91 men; mean age 70 years, range 34-91 years) referred to hospital with osteoarthritis of the knee underwent clinical, radiographic, and synovial fluid screening. Results-Radiographic changes of osteoarthritis of the knee (definite narrowing with or without osteoarthritic features) were bilateral in 85% of patients. Of 470 knees affected, 277 (59%) were affected in two compartments and 28 (6%) in three compartments. Unilateral and isolated medial tibiofemoral osteoarthritis were more common in men. Calcium pyrophosphate crystal deposition was common (synovial fluid identification in 132 (28%) knees; knee chondrocalcinosis in 76 (30%) patients) and associated with disability, bilateral, multicompartmental and severe radiographic osteoarthritis, marked osteophytosis, attrition, and cysts. Multiple clinical nodes (58 (23%) patients) and radiographic polyarticular interphalangeal osteoarthritis (66 (26%) patients) were associated with a higher frequency of inactivity pain, disability, multicompartmental and severe radiographic change. Forestier's disease predominated in men but showed no other associations. Conclusions-In a group of patients referred to hospital osteoarthritis of the knee is usually bilateral and affects more than one compartment. Severe and multicompartmental radiographic changes are associated with calcium pyrophosphate crystal deposition, nodal change, and polyarticular interphalangeal osteoarthritis.

This paper presents a systematic review of the existing literature and reports on a large case series comparing implant fractures between the Exeter V40 series; 125 mm and conventional length stem systems. Methods A systematic literature search was performed adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) criteria. In parallel, we performed a retrospective single centre study of Exeter V40 femoral stem prosthetic fractures between April 2003 and June 2020. Results There are 25 reported cases of such prosthetic fractures confined to small case series and case reports within the literature. We report an additional 19 cases to the literature (mean age 66.3 years (SD 11.7); 12 (63%) females; BMI 32.9 kg/m2 (SD 5.9)). The mean time from index procedure to fracture was 7.8 years (SD 3.6; 2.5 to 16.3). Exeter V40 stem fracture incidence was 0....

The war to end all wars finished its deadly drama at 11am on the 11 November 1918. The total numb... more The war to end all wars finished its deadly drama at 11am on the 11 November 1918. The total number of military casualties in World War I was about 40 million; estimates range from 15 to 19 million deaths with around 23 million wounded military personnel, ranking it among the deadliest conflicts in human history. To those of us reading these figures a hundred years later, I am sure all of us will have relatives who were affected by this human sacrifice. For myself it's 61988 Private Thomas A. Jones of the Suffolk Regiment, a grandfather I never met, his death hastened by being gassed during trench warfare. The battalion was the standard operational unit of all British Infantry during The Great War. In 1914, an infantry battalion was manned by 30 officers and 977 other ranks, making a total of 1,007. These numbers were halved by the Battle of Passchendale. Every battalion had a medical officer assisted by at least 16 stretcher-bearers. The medical officer was tasked with establishing a Regimental Aid Post near the front line. From here, the wounded were evacuated and cared for by men from a Field Ambulance in an Advanced Dressing Station. Once treated, casualties would be moved rearward several miles to the Casualty Clearing Station. This might be on foot or by a horse drawn wagon, motor ambulance, lorry or in some cases by light railway.

Surgical practice in the UK changed in 1993, when Suzanne Holmes and her cardiac surgical colleag... more Surgical practice in the UK changed in 1993, when Suzanne Holmes and her cardiac surgical colleagues introduced the surgical care practitioner role (SCP). Within a consultant led extended surgical team, SCPs work alongside a variety of healthcare practitioners to provide safe patient care, meet service demands and educate the future surgical workforce. This article reviews the history of this development over the last fifteen years in the context of a busy orthopaedic department and discusses some unforeseen consequences.

The aim of this paper is to report on findings identifying some of the difficulties encountered b... more The aim of this paper is to report on findings identifying some of the difficulties encountered by the multidisciplinary team in the development and implementation of a care pathway for patients diagnosed with schizophrenia. Policy direction has shifted towards greater team working and blurring of professional boundaries. Moreover, there is greater need to deliver care according to set standards and for patients to reach particular outcomes of care. Care pathways are relatively new for psychiatry and will potentially uncover tensions within the team. Data were collected by participant observation and semistructured interviews over a period of 12 months on an acute psychiatric unit. The care team developed the care pathway and the process of development and implementation was observed through action research. Cross-sectional indexing was used to analyse the data, and themes were developed using interview and observational data collection methods. Clinicians argued strongly for clear role boundaries but also defended their perceived control over health care from other professions. The findings indicate that designing a care pathway for people with schizophrenia may produce conflicting perceptions from the team. Conflict may arise through professions being unwilling to accept plurality over roles, which may hinder progress in meeting the needs of patients. The findings also counter the impression that care pathways can be implemented with little impact on the team.
