Rare co-existance of disease or pathology Background: Whether creatine-kinase (CK) is elevated or... more Rare co-existance of disease or pathology Background: Whether creatine-kinase (CK) is elevated or not in chronic inflammatory demyelinating polyneuropathy (CIDP) and its variants is not comprehensively investigated. Case Report: We report the case of a 47-year-old male who developed weakness of the left lower leg and the right index finger at age 42 years. At age 44 years, paresthesias and dysesthesias of both lower legs and mild right lower leg weakness additionally developed. CK was recurrently elevated since age 42 years but paraprotein and anti-myelin-associated glycoprotein (MAG)-antibodies were negative. Nerve conduction studies at age 43 years showed an axonal and demyelinating lesion with conduction blocks. Cerebrospinal fluid (CSF) investigations revealed mild pleocytosis and elevated protein, which is why CIDP variant was diagnosed. Immunoglobulins were administered with success. Because of recurrent relapses, immunoglobulins were increased at age 45 years, resulting in stabilization. Currently, the patient is infusing immunoglobulins subcutaneously himself. Conclusions: CIDP variants may go along with CK elevation, an axonal lesion, pleocytosis, and asymmetry of the lesion. A vanishing effect of immunoglobulins over time may be characteristic of CIDP variants.

Referring physicians are valuable sources of information for radiologist to improve quality in radiology. In dif ferent healthcare systems neuroradiology has developed differently. Healthcare in Azerbaijan is developing and investments take place both in infrastructure and knowledge. Tools to compare healthcare in Azerbaijan and western countries can help to guide these improvements by providing detailed information about a specific field resulting in a more focused approach. Methods. A survey covering the field of neuroradiology was developed, with a variety of items including the availability of equipment, the contentment with the radiological report, the experience of the neurologist, and factors omitting neurological imaging. Data was collected both in Azerbaijan and the Netherlands. Results. In both countries most neurologist do not receive formal neuroradiology training. The data demonstrate a higher avail ability of advanced computed tomography (CT) and magnetic resonance imaging (MRI) machines in the Netherlands compared with Azerbaijan, and a higher experience with requesting imaging and case reading for the Dutch respondents. The financial situation of the patient can be a reason for Azerbaijan neurologists to omit imaging (due to the establishment of CT and MRI in most cases in specialized and private centers). Conclusion. The presented survey allows comparison of neuroradiological healthcare between countries in general, and iden tification of required preconditions to improve collaboration between neurologists and radiologists. A future lower financial threshold for imaging and a future increase in availability of PACS (picture archiving and communication system) will facili tate the improvement of neuroradiological skills of Azerbaijan neurologists. Both Dutch and Azerbaijan neurologists consider weekly clinical neuroradiological conferences together with radiologists useful to improve their skills.

Compared to healthy controls, in multivariate logistic regression models, IFN-β was strongly asso... more Compared to healthy controls, in multivariate logistic regression models, IFN-β was strongly associated with headache (OR 7.105, pb0.001), while IFN-β-naïve controls presented a less strong association (OR 2.263, pb0.05). Moreover, MIDAS score was higher in the IFN-β group (16.9±20.7 vs. 2.1±5.8, pb0.001), while no statistical difference was detected in the IFN-β-naïve group (2.2 ±4.4 vs. 2.1±5.8, p 0.974). Likewise, multivariate linear regressions showed a positive association of IFN-β with the MIDAS score (β=7.043, pb0.001). Conclusion: In RRMS patients, IFN-β is associated with higher odds and severity of headache compared to healthy controls, while IFN-βnaïve patients present higher odds for headache but no difference in headache severity.

Guba-Khachmaz economic region is situated in northeast of Azerbaijan. This region consists of Siyazan, Shabran, Guba, Gusar, and Khachmaz administrative regions. Overall area of the region is 7.66 thousand sq. km, accounting for 8.8% of country's area. This economic region is characterized with its unique landscape features. There are four sharply differing altitude zones within the region ranging from 26 meters to 4466 meters, which are plains, foothill, mid-mountaineous, and high mountaineous zones. Climate also changes depending on the altitude, as warm climatic conditions prevail in plains, whereas high mountaineous areas are characterized with cold-humid and cold climate [9]. Landscape peculiarities and climatic conditions resemble smaller model of the country. Based on the results of National Census of Azerbaijan Republic for 2009, total number of population in the region is 488,741 thousand, accounting for 5.48% of country's population. 33.99% of population (166,105 thousand persons) reside in urban areas, whereas remaining 66.01% (322,636 thousand persons) are settled in rural areas. A slow but steady increase in neurodegenerative disorders has been noted in recent decades. Degen... more A slow but steady increase in neurodegenerative disorders has been noted in recent decades. Degenerations in the nervous system are found in Alzheimer’s disease (AD), Parkinson’s disease (PD) and motor neuron disease (MND). In MND spinal neurons degenerate causing muscle atrophy invariably leading to death from respiratory failure. What is causing this degeneration? No consistent explanation has been presented despite intense scientific efforts. Some indications point toward metallotoxic etiologies. MND clusters have been observed in regions with elevated metal concentrations in water and soil. Several studies show increased MND frequency in certain occupations. MND-like conditions are found in animals and animal metal exposure experiments show specific spinal cord accumulations of metals. To address possible metal contributions in MND causation we measured concentrations of 22 metals in cerebrospinal fluid (CSF) from 17 MND patients and 10 controls. Ethical approval was received. Statistically significantly increased concentrations were found for manganese (Mn), aluminum, cadmium, cobalt, copper, zinc, lead, vanadium and uranium (Table 1). Most of the detected metals are well known neurotoxicants. Manganese showed the most prominent differences between cases (median 5.67 mg/L) and controls (median 2.08 mg/L) (Fig. 1). Simultaneous sampling from plasma however did not show elevated concentrations, indicating metal accumulations in MND CSF. The cases were presented with scores of mild to moderate anxiety and depression. ASEX scores of RLS patients were higher in the study group. Limitations of this study, include relatively small number of participants and all patients had dopamine agonist treatment. Further studies with more patients who are not treated are needed to clarify the relationship between RLS and psychiatric, sexual disorders.

Patients and methods: We compared national data per number of inhabitants and per 100 annual incident first-ever ischaemic strokes (AIIS) in 46 countries. Population estimates and ischaemic stroke incidence were based on United Nations data and the Global Burden of Disease Report 2019, respectively. Results: The estimated mean number of acute SUs in 2019 was 3.68 (95% CI: 2.90–4.45) per one million inhabitants (MIH) with 7/44 countries having less than one SU per one MIH. This region is located in the north-east of Azerbaijan. The population is 488,741, which is 5.48% of the total country population. 33.99% of population (166,105 person) live in sities, 66.01% (322,636 person) in rural regions. The most of urban population (65.6%) lives in Siyazan region, rural population (75.11%) in Guba region. 49.84% of the population is men and 50.16% is women. All cases of incidence and mortality from stroke in the region have been retrospectively studied from the data of Central Region Hospitals. According to the statistical work, in relation to the statistics indicating qualitative signs, level of mortality falling to 1000 people has been counted.

Mean age of stroke cases during investigated years was 65.2±0.9. Males had stroke at a younger age – 62.2±1.3 than females – 68.2±1.2 (p<0.001). Patients residing in rural areas had stroke at older age than urban population, which was at a statistically significant level. Most of the stroke patients fell into the age groups of 50-59 and 70-79. In first age group men, in second women. Mean age for ischaemic and hemorrhagic stroke cases was 65.8±0.9 and 57.6±3.4, respectively.

Objective: To survey the accessible MD clinical training in these regions. Methods: We designed a survey on clinical training in MD in different medical fields, at postgraduate and specialized levels. We assessed the characteristics of the participants and the facilities for MD care in their respective countries. Systemic side effects of topical betamethasone may completely resolve after discontinuation of the causative agent. A 59‐year‐old male patient with scalp actinic keratosis developed fatigue and the need to sleep shortly after starting topical betamethasone/diclofenac ointment. Additionally, he experienced vertigo, nausea, sleep disturbance, change in character and recognition, tiredness, visual impairment, and elevated blood pressure. Since all symptoms resolved completely within 48 hours after discontinuation of the ointment, they were attributed to glucocorticoids.

Rhabdomyolysis was not attributed to this injected drug but to compound heterozygous variants in LPIN1. The study has a number of shortcomings. Triggers of rhabdomyolysis should be unequivocally identified, a more extensive family history should be taken, and previous CKVs should be provided. Functional and biochemical tests should be carried out to confirm or exclude pathogenicity of the LPIN1 variants.

This region is located in the north-east of Azerbaijan. The population is 488,741, which is 5.48% of the total country population. 33.99% of population (166,105 person) live in sities, 66.01% (322,636 person) in rural regions. The most of urban population (65.6%) lives in Siyazan region, rural population (75.11%) in Guba region. 49.84% of the population is men and 50.16% is women. All cases of incidence and mortality from stroke in the region have been retrospectively studied from the data of Central Region Hospitals. According to the statistical work, in relation to the statistics indicating qualitative signs, level of mortality falling to 1000 people has been counted.

The ame of this study was exploring age as risk factor for stroke among 173 stroke cases which r... more The ame of this study was exploring age as risk factor for stroke among 173 stroke cases which registered during 3 years (01.01.2007-31.12.2009) in Siyazan region of Azerbaijan. Mean age of stroke cases during investigated years was 65.2±0.9. Males had stroke at a younger age – 62.2±1.3 than females – 68.2±1.2 (p<0.001). Patients residing in rural areas had stroke at older age than urban population, which was at a statistically significant level. Most of the stroke patients fell into the age groups of 50-59 and 70-79. In first age group men, in second women. Mean age for ischaemic and hemorrhagic stroke cases was 65.8±0.9 and 57.6±3.4, respectively.

We assessed the characteristics of the participants and the facilities for MD care in their respective countries. Systemic side effects of topical betamethasone may completely resolve after discontinuation of the causative agent. A 59‐year‐old male patient with scalp actinic keratosis developed fatigue and the need to sleep shortly after starting topical betamethasone/diclofenac ointment. Additionally, he experienced vertigo, nausea, sleep disturbance, change in character and recognition, tiredness, visual impairment, and elevated blood pressure. Since all symptoms resolved completely within 48 hours after discontinuation of the ointment, they were attributed to glucocorticoids.

In a recent article by Yim et al., a 15-month-old male is described who experienced severe rhabdo... more In a recent article by Yim et al., a 15-month-old male is described who experienced severe rhabdomyolysis with a creatine-kinase value (CKV) of 127494 U/l one day after intramuscular injection of an unidentified drug by the general practitioner. Rhabdomyolysis was not attributed to this injected drug but to compound heterozygous variants in LPIN1. The study has a number of shortcomings. Triggers of rhabdomyolysis should be unequivocally identified, a more extensive family history should be taken, and previous CKVs should be provided. Functional and biochemical tests should be carried out to confirm or exclude pathogenicity of the LPIN1 variants.
