Introduction: Trauma is the new scourge of humankind as without utmost and urgent care, it can re... more Introduction: Trauma is the new scourge of humankind as without utmost and urgent care, it can result in the loss of limb and life. Civilian trauma is mostly caused by road traffic accidents with resultant skeletal, vascular, and neurological injuries. Vascular injuries most commonly involve the extremities and occur in young males. Materials and Methods: Ours is prospective observational study, in which 50 patients of the lower limb vascular injury were included in the study. Results: All of them were male, and mostly, they were in the second and third decades of life. Popliteal artery was the most commonly injured vessel, and road traffic accident was the most common mode of injury. Fifty-six percent (n = 28) had favorable outcomes with 8% mortality (n = 4). Twenty-six (52%) patients had bony involvement, the most commonly injured bone was tibia (n = 16, 32%). The mean warm ischemia time was 15.56 ± 14.03 (range: 3–48) h. Mangled Extremity Severity Score (MESS) scores were significantly different in salvaged and nonsalvaged limbs (P = 0.004). The sensitivity and negative predictive value of MESS were very high (100%), but the specificity and positive predictive value for unfavorable outcomes were low (57.14% and 64.71%). Skeletal injury, neurological involvement, and a blunt mechanism of injury were found to have a significant association with an unfavorable outcome in the patients. Conclusion: We concluded that MESS cannot be completely relied on, and the surgeon's clinical assessment and experience are necessary to decide the plan of management. Warm ischemia time is not the most important factor affecting limb survival, and other factors such as neurological and bony involvement need to be considered. MESS helps to frame the data for the clinician and can help in counseling patients, and their families that poor outcomes are more likely when MESS score is high. Further, large-scale, multicenter randomized trials are necessary for the identification of other risk factors and their incorporation into scoring systems.

Background: The TTK Chitra mechanical heart valve prosthesis (CHVP) is a mono-leaflet, tilting di... more Background: The TTK Chitra mechanical heart valve prosthesis (CHVP) is a mono-leaflet, tilting disc design heart valve, which was developed in India and relatively cheaper than western Mechanical Prosthetic valve. Very limited studies are available for the comparison of CHVP versus western mechanical prosthetic heart valve. This study was designed to compare the Indian valve vs. western valve. Methods: It was a single centre, retrospective-prospective follow-up study. The study was approved by the Institutional Ethics Committee(Ethical clearance was not required) and written informed consent was obtained from all participants. Among records of the hospital management system 531 medical records screened, 378 subjects met the inclusion-exclusion criteria and were included for analysis. SPSS software was used for analysis. Results: The results of this study indicate that the long term performance of the St. Jude Mechanical heart valve and TTK Chitra mechanical valve prostheses is comparable in terms of clinical benefits, adverse events and mortality. Conclusions: The CHVP offers equivalent results at almost half the cost of an imported St Jude Mechanical heart valve making the prospect of cardiac surgery available to a large number of deserving patients in resource-limited settings.

Tetralogy of Fallot (TOF) is the most common form of cyanotic congenital heart disease and repres... more Tetralogy of Fallot (TOF) is the most common form of cyanotic congenital heart disease and represents 5-7% of all congenital cardiac lesions. 1 The etiology of TOF is the cephalad and anterior deviation of infundibular septum leading to the cardinal features of ventricular septal defect (VSD), right ventricular hypertrophy (RVH), pulmonary stenosis (PS) and aortic override. 2 Alfred Blalock in 1944, performed the first systemic-topulmonary shunt, followed by the first complete repair by C. Walton Lillehei a decade later, the surgical strategy has evolved considerably over the years. Current evidence supports early repair of TOF to minimize the ABSTRACT Background: Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease with complete surgical repair being its most appropriate treatment currently. Although in a developing country, pediatric cardiac surgery is not being practiced as frequently as the adult cardiac surgery either due to limited resources centres or surgeon reluctance. The aim of this study is to evaluate and compare the operative and postoperative outcome of the initial 50 patients with the subsequent 50 patients undergoing corrective surgery of TOF in a new cardiac centre. Methods: This is a comparative descriptive study performed at a single centre comprising of two groups of initial 50 and subsequent 50 patients who underwent complete surgical correction of TOF and compare their operative and postoperative outcomes. Results: The mean age of patients in group I and II were 82.4 and 74.3 months respectively. There was no significant difference in the preoperative characteristics of the patients of both groups. Among operative parameters there was significant improvement in the cardiopulmonary bypass time and aortic cross clamp time in group 2 with p value of 0.0017 and 0.0324 respectively. The requirement for transannular patch also came down in group 2 (p=0.016). Mortality in group 1 and 2 were 6 and 2 respectively (p=0.14), other postoperative characteristics were similar in both groups. Conclusions: With growing experience, proper planning and perseverance pediatric cardiac surgery too can be performed with acceptable results in a new centre.

Primary pulmonary synovial sarcoma (PPSS) is a rare and aggressive malignancy with a presentation... more Primary pulmonary synovial sarcoma (PPSS) is a rare and aggressive malignancy with a presentation similar to that of lung carcinoma. We present an 18-year-old male who presented with complaints of chest pain, cough, and hemoptysis. Physical examination and blood parameters were normal. Imaging studies revealed a large, well-defined, round homogeneous mass in the left lower lobe. His fine-needle aspiration cytology was suggestive of a round cell tumor. The patient was operated under general anesthesia through left posterolateral thoracotomy. The tumor was removed en masse and sent for histopathological examination. His histopathology and immunohistochemistry suggested PPSS of biphasic variety.

This report presents the rationale and design of a multi-center clinical trial that examines the ... more This report presents the rationale and design of a multi-center clinical trial that examines the efficacy and safety of antipsychotic combination treatment in acutely ill schizophrenia patients compared to antipsychotic monotherapy. Antipsychotic combination treatment is common in clinical practice worldwide, despite clinical guidelines generally not recommending such practice due to lacking evidence for its efficacy and safety. Olanzapine has a related chemical structure and comparable receptor-binding profile as clozapine, which demonstrated superior efficacy in combination studies, but has a more unfavorable side-effect profile compared to olanzapine. Amisulpride and olanzapine have shown promising therapeutic efficacy in meta-analyses in monotherapy for people with schizophrenia. Combining amisulpride and olanzapine, complementary receptor-binding properties may enhance efficacy and possibly reduce (or at least not augment) side effects due to the different receptor profiles and metabolization pathways. Accordingly, we hypothesize that patients treated with amisulpride plus olanzapine show greater improvement on the Positive and Negative Syndrome Scale total score after 8 weeks versus either monotherapy. A randomized, double-blind controlled trial is performed at 16 German centers comparing flexibly dosed monotherapy of oral amisulpride (400-800 mg/day), and olanzapine (10-20 mg/day) and amisulpride-olanzapine co-treatment. Sample size was calculated to be n = 101 per treatment arm, assuming an effect size of 0.500 and a two-sided alpha = 0.025 and beta = 0.90. Recruitment for this trial started in June 2012. Until December 2018, 328 patients have been randomized. Trial conduct has been extended to reach the projected sample size. Publication of the study results is expected in 2019 informing an evidence-based recommendation regarding specific antipsychotic combination treatment.

Parapneumonic pleural effusion is a common health problem in developing countries, seen in 36%-57... more Parapneumonic pleural effusion is a common health problem in developing countries, seen in 36%-57% of patients with pneumonia, and around 15%-20% of these patients develop empyema. [1] The management of uniloculated pleural effusion is simple, however, once effusion becomes infected and multiloculated, the management becomes difficult. This complicated parapneumonic effusion (CPE)/empyema is a potentially dangerous condition with serious consequences such as persistent sepsis, bronchopleural or bronchocutaneous fistula or progress to restrictive lung disease, and sometimes even death. [2,3] Various management protocols have been tried for management of multiloculated empyema, but still, there are no standard guidelines that can be followed in all the cases. [4] In dealing with CPE, intrapleural fibrinolytics may be a safe, easy, cost-effective management option. [5] Streptokinase (STK) was the first fibrinolytic agent used for intrapleural adhesiolysis by Tillet et al. [6] Other fibrinolytic agents such as urokinase or tissue plasminogen have also shown successful results. [7] The safety and efficacy of fibrinolytic agents in adults are well established by numerous randomized trials, [3] however, only a few trials have been done in pediatric population. [7] Another important context to efficacy of fibrinolytics is that the success of fibrinolytic therapy should be judged as patients not needing surgical interventions rather than volume of pleural fluid output and radiological improvement. [8] The dispute over the optimal therapeutic approach has been accentuated since the introduction of early thoracotomy and decortications via video-assisted thoracic surgery (VATS). [9]

Introduction: The management of empyema thoracis has evolved over the years. After all lung surge... more Introduction: The management of empyema thoracis has evolved over the years. After all lung surgeries chest drain is required, however they suffer from inter observer variability and impair mobility of the patient. However, the newer digital thoracic drain system are portable and have alarms for various situations, furthermore they eliminate inter-observer variability. One such device is Thopaz™ (Medela inc, Switzerland). We wanted to compare efficacy of Thopaz™ with our conventional intercostal chest tube drain in patients undergoing decortications for empyema thoracis. Materials and Methods: One hundred patients were enrolled in study and were randomized into conventional and thopaz group with 50 patients in each group with help of opaque envelopes. Results: Both the groups were comparable in demographic parameters. Majority of the patients in our study were children and young adults. Majority of empyema thoracis involved right side with nontubercular empyema thoracis being the most common cause of decorticartion. Patients managed with Thopaz had a significantly shorter air leak duration, shorter duration of postdecortication chest tube placement and shorter postoperative hospital stay. All postoperative complications were less in Thopaz group. Conclusions: Patients with empyema thoracis undergoing open decortications when managed with digital chest drainage system (Thopaz) experienced faster reduction in air leak, a shorter duration of chest tube placement and in hospital stay. Thopaz usage is also associated with reduction in rate of postoperative complications. We recommend that this digital chest tube drainage system is a very useful tool in armamentarium of thoracic surgeon after lung surgeries.

Patients and Methods: This is a comparative descriptive study which included initial 50 patients in the STK group and subsequent 50 patients in the placebo group. Intrapleural STK was given twice daily at a dose of 5000 IU/kg for 7 days. In the placebo group, 50-ml normal saline was instilled intrapleurally through the chest tube. This study evaluated the efficacy of oral propranolol comparing to oral steroids in management of pediatric hemangiomas. In North India from January 2012 to January 2015, sixty children <6 years old with superficial hemangiomas were divided into 2 groups; oral propranolol vs. oral prednisolone. All participants were assessed for electrocardiogram, heart rate, blood pressure and sugar and initial therapy was started using 1 mg/kg and in absence of adverse effects, 2 mg/kg was administered after 2 weeks. The hemangioma Activity Score (HAS) was used for scoring and patients were followed up for 6 months. The propranolol group mostly showed early response to the drug and needed the drug for less time compared to corticosteroid group. Diaphragmatic agenesis is a rare diagnosis, typically made early in infancy and is generally associated with other genetic anomalies, especially anueploidy syndromes. It is associated with a high mortality, if not treated in infancy. However, a few patients have survived till adulthood. In this report, we describe the case of an adult female who presented with progressive shortness of breath during third trimester of pregnancy with complete agenesis of the right side of the diaphragm. A new diaphragm was created using polypropylene mesh after which she improved symptomatically and the lung showed good expansion.

Methods: We retrospectively identified 50 patients in which BBFs were constructed between January 2015 and January 2018. Results: The mean age of patients in group I and II were 82.4 and 74.3 months respectively. There was no significant difference in the preoperative characteristics of the patients of both groups. Among operative parameters there was significant improvement in the cardiopulmonary bypass time and aortic cross clamp time in group 2 with p value of 0.0017 and 0.0324 respectively. The requirement for transannular patch also came down in group 2 (p=0.016). Mortality in group 1 and 2 were 6 and 2 respectively (p=0.14), other postoperative characteristics were similar in both groups. Conclusions: With growing experience, proper planning and perseverance pediatric cardiac surgery too can be performed with acceptable results in a new centre.

We present an 18-year-old male who presented with complaints of chest pain, cough, and hemoptysis. Physical examination and blood parameters were normal. Imaging studies revealed a large, well-defined, round homogeneous mass in the left lower lobe. His fine-needle aspiration cytology was suggestive of a round cell tumor. The patient was operated under general anesthesia through left posterolateral thoracotomy. The tumor was removed en masse and sent for histopathological examination. His histopathology and immunohistochemistry suggested PPSS of biphasic variety.

This report presents the rationale and design of a multi-center clinical trial that examines the ... more This report presents the rationale and design of a multi-center clinical trial that examines the efficacy and safety of antipsychotic combination treatment in acutely ill schizophrenia patients compared to antipsychotic monotherapy. Antipsychotic combination treatment is common in clinical practice worldwide, despite clinical guidelines generally not recommending such practice due to lacking evidence for its efficacy and safety. Olanzapine has a related chemical structure and comparable receptor-binding profile as clozapine, which demonstrated superior efficacy in combination studies, but has a more unfavorable side-effect profile compared to olanzapine. Amisulpride and olanzapine have shown promising therapeutic efficacy in meta-analyses in monotherapy for people with schizophrenia. Combining amisulpride and olanzapine, complementary receptor-binding properties may enhance efficacy and possibly reduce (or at least not augment) side effects due to the different receptor profiles and metabolization pathways. Accordingly, we hypothesize that patients treated with amisulpride plus olanzapine show greater improvement on the Positive and Negative Syndrome Scale total score after 8 weeks versus either monotherapy. A randomized, double-blind controlled trial is performed at 16 German centers comparing flexibly dosed monotherapy of oral amisulpride (400-800 mg/day), and olanzapine (10-20 mg/day) and amisulpride-olanzapine co-treatment. Sample size was calculated to be n = 101 per treatment arm, assuming an effect size of 0.500 and a two-sided alpha = 0.025 and beta = 0.90. Recruitment for this trial started in June 2012. Until December 2018, 328 patients have been randomized. Trial conduct has been extended to reach the projected sample size. Publication of the study results is expected in 2019 informing an evidence-based recommendation regarding specific antipsychotic combination treatment.

Parapneumonic pleural effusion is a common health problem in developing countries, seen in 36%-57... more Parapneumonic pleural effusion is a common health problem in developing countries, seen in 36%-57% of patients with pneumonia, and around 15%-20% of these patients develop empyema. [1] The management of uniloculated pleural effusion is simple, however, once effusion becomes infected and multiloculated, the management becomes difficult. This complicated parapneumonic effusion (CPE)/empyema is a potentially dangerous condition with serious consequences such as persistent sepsis, bronchopleural or bronchocutaneous fistula or progress to restrictive lung disease, and sometimes even death. [2,3] Various management protocols have been tried for management of multiloculated empyema, but still, there are no standard guidelines that can be followed in all the cases. [4] In dealing with CPE, intrapleural fibrinolytics may be a safe, easy, cost-effective management option. [5] Streptokinase (STK) was the first fibrinolytic agent used for intrapleural adhesiolysis by Tillet et al. [6] Other fibrinolytic agents such as urokinase or tissue plasminogen have also shown successful results. [7] The safety and efficacy of fibrinolytic agents in adults are well established by numerous randomized trials, [3] however, only a few trials have been done in pediatric population. [7] Another important context to efficacy of fibrinolytics is that the success of fibrinolytic therapy should be judged as patients not needing surgical interventions rather than volume of pleural fluid output and radiological improvement. [8] The dispute over the optimal therapeutic approach has been accentuated since the introduction of early thoracotomy and decortications via video-assisted thoracic surgery (VATS). [9]

After all lung surgeries chest drain is required, however they suffer from inter observer variability and impair mobility of the patient. However, the newer digital thoracic drain system are portable and have alarms for various situations, furthermore they eliminate inter-observer variability. One such device is Thopaz™ (Medela inc, Switzerland). We wanted to compare efficacy of Thopaz™ with our conventional intercostal chest tube drain in patients undergoing decortications for empyema thoracis. Materials and Methods: One hundred patients were enrolled in study and were randomized into conventional and thopaz group with 50 patients in each group with help of opaque envelopes. Results: Both the groups were comparable in demographic parameters. Majority of the patients in our study were children and young adults. Majority of empyema thoracis involved right side with nontubercular empyema thoracis being the most common cause of decorticartion. Patients managed with Thopaz had a significantly shorter air leak duration, shorter duration of postdecortication chest tube placement and shorter postoperative hospital stay. All postoperative complications were less in Thopaz group. Conclusions: Patients with empyema thoracis undergoing open decortications when managed with digital chest drainage system (Thopaz) experienced faster reduction in air leak, a shorter duration of chest tube placement and in hospital stay. Thopaz usage is also associated with reduction in rate of postoperative complications. We recommend that this digital chest tube drainage system is a very useful tool in armamentarium of thoracic surgeon after lung surgeries.

In North India from January 2012 to January 2015, sixty children <6 years old with superficial hemangiomas were divided into 2 groups; oral propranolol vs. oral prednisolone. All participants were assessed for electrocardiogram, heart rate, blood pressure and sugar and initial therapy was started using 1 mg/kg and in absence of adverse effects, 2 mg/kg was administered after 2 weeks. The hemangioma Activity Score (HAS) was used for scoring and patients were followed up for 6 months. The propranolol group mostly showed early response to the drug and needed the drug for less time compared to corticosteroid group. In this report, we describe the case of an adult female who presented with progressive shortness of breath during third trimester of pregnancy with complete agenesis of the right side of the diaphragm. A new diaphragm was created using polypropylene mesh after which she improved symptomatically and the lung showed good expansion.
