Background: Beta-thalassemia trait (βTT) is a common hemoglobinopathy and imposes a significant b... more Background: Beta-thalassemia trait (βTT) is a common hemoglobinopathy and imposes a significant burden on the global healthcare. Screening of this disorder is immensely important as it can significantly reduce the future incidence of thalassemia major. This study was conducted in order to evaluate the role of discrimination indices to differentiate cases of βTT from other causes of microcytic anemia. Materials and Methods: A single-center study was carried out on 1600 subjects reported during November 2015 to November 2017.Tests for serum iron and ferritin were also conducted in individuals showing low MCV. All the selected samples were subjected to complete blood counts and blood morphology. Cellulose acetate alkaline electrophoresis and iron profile were carried out to differentiate IDA and beta thalassemia trait. Comparison of various parameters and discrimination indices were calculated for all the samples. Results: Shine and Lal index, Mentzer index and red cell distribution width index had revealed better discriminative function compared to England and Fraser index, Srivastava index, and Green and King Index. Conclusion: Though HbA2 estimation is the gold standard for diagnosing βTT, in developing countries, discrimination indices are rapid, reliable and easy tools to screen cases of βTT and send the subject for further electrophoresis and high performance liquid chromatography evaluation.

The study was undertaken to evaluate the role of ultrasound guided fine needle aspiration cytolog... more The study was undertaken to evaluate the role of ultrasound guided fine needle aspiration cytology (FNAC) of gastrointestinal masses in providing higher diagnostic yield and assess its reliability in distinguishing between benign and malignant gastrointestinal lesions. 86 patients with gastrointestinal masses after thorough clinical examination underwent ultrasonographic examination. Those found to have a mass were subjected to FNAC. Smears prepared were fixed in 95% ethyl alcohol and stained with Haematoxylin and Eosin and Papanicolaou stains. Forty (46.5%) were males and 46 (53.5%) females with majority of patients in the third decade in both sexes. Malignant lesions constituted the maximum number of 42 (48.8%) cases, followed by 36 (41.8%) cases of benign and inflammatory lesions. Adenocarcinoma was the most common malignancy accounting for 36 (85.7%) cases; out of which 16 (44.4%) cases were adenocarcinoma stomach and 20 (55.6%) cases were adenocarcinoma intestine. Among the inflammatory and benign lesions, 30 (83.3%) cases were tuberculous followed by 4 (11.1%) cases appendicitis, The mean age of malignancy was 39.2 years and the average age of tuberculous patients in our study was 25.5 years. On cytohistological correlation, 30 were found to be true positive for malignancy, 5 true negative and 1 false negative. There were no false positive cases. We obtained a sensitivity of 93.8%, specificity of 100% and diagnostic accuracy of 94.4% in our study. Ultrasound guided FNAC is an effective and useful method which eliminates the need for surgical biopsy and provides sufficient information for the initiation of therapy.

Sebaceous carcinoma is a rare locally aggressive neoplasm of the sebaceous gland, which occurs fr... more Sebaceous carcinoma is a rare locally aggressive neoplasm of the sebaceous gland, which occurs frequently in elderly males. It constitutes 0.2 to 4.6% of all the malignant cutaneous tumours and occurs commonly in the periocular area. Scalp is a rare site for this tumor. Here we report a case of sebaceous carcinoma in a young 28 years old male patient who presented with firm to hard exophytic growth over right posterior occipital region with enlarged suboccipital and post auricular lymph nodes. Wide local excision was done and a diagnosis of sebaceous carcinoma of the scalp with lymph node metastasis was rendered after histopathological and Immunohistochemical examination.

Chromohysteroscopy is thought to improve the diagnostic accuracy of hysteroscopy and reach the final diagnosis in AUB and is the subject matter of this study.Methods: A prospective cross-sectional study was performed on 35 women with AUB, who presented to the Dept. of Obstetrics and Gynecology, JNMCH, Aligarh, over a period of 2 years (2014-2016). After transvaginal sonography, all cases underwent hysteroscopy followed by chromohysteroscopy using 5% methylene blue dye. Differentially stained pattern of the uterine cavity was observed and hysteroscopic guided biopsy was taken from stained and unstained areas, followed by endometrial aspiration biopsy. Scrotalcalcinosis is a rare benign local process. Calcinosis cutis has four major types according to etiology. Scrotal calcinosis is mostly multiple, commonly occurs between third and fourth decades of life. It is extremely rare in infants. This is a case report of single calcified nodule that occurred in the scortum of an 8 month old boy. The lesion was identified by cytology and confirmed with histology. Various sites of few infantile calcinosis have been described till now. Here we report a very rare and first case in an infant, who presented with idiopathic calcinosis cutis in scrotum.

Adult granulosa cell tumor (GCT) of the ovary is oftentimes a hormonally active, stromal cell neo... more Adult granulosa cell tumor (GCT) of the ovary is oftentimes a hormonally active, stromal cell neoplasm that is distinguished by its ability to secrete sex steroids such as estrogen and is mostly diagnosed in postmenopausal women. Patients may present with vaginal bleeding as a result of prolonged exposure to tumor-derived estrogen. In addition, GCT is a vascular tumor that may occasionally rupture and result in abdominal pain, hemoperitoneum, and hypotension, mimicking an ectopic pregnancy in younger patients. We report a rare case of a metastatic adult granulosa cell tumor of the ovary in a 25-year-old woman who presented with abdominal swelling and pain for the last 6 months with an ill-defined, firm left adnexal mass on pelvic examination with ascites. Cytological impression of the ascitic tap was suggestive of Granulosa cell Tumor. Histopathology of the mass confirmed the cytopathological diagnosis.

However, in India and most of the other developing countries the lower limit is often accepted as 10 g/dl [1,2,3]. According to the Nutrition Impact Model Study's 2011 estimates, the worldwide prevalence of anemia in pregnant women was 38% [4]. Developing nations have a very high prevalence of anemia in the world with almost 58 percent of pregnant women in India being anemic [5]. Antenatal Iron deficiency anemia accounts for 7595% of cases [6]. Anemia results in an increased number of preterm, low birth weight, impaired cognitive development of children, postpartum hemorrhage, postpartum depression and reduced adult work productivity [7].

To investigate the role of serum ferritin and oxidative stress in the development of GDM and to a... more To investigate the role of serum ferritin and oxidative stress in the development of GDM and to assess their relationship with the ensuing hyperglycemia. A case–control study was carried on 90 non-anemic pregnant women of 20–40 years with a gestation of 24–28 weeks. Study group (n = 65) was identified according to the Diabetes in Pregnancy Study Group India (DIPSI) criteria (2-h plasma glucose ≥ 140 mg/dl) and controls (n = 25) having 2-h plasma glucose < 120 mg/dl. DIPSI 2-h plasma glucose, HbA1c and serum ferritin were measured and oxidative stress index (OSI) was calculated. Statistical tests were performed using SPSS version 25.0. Pre-pregnancy BMI showed a significant difference between control and study group. DIPSI 2 h blood glucose, HbA1c, serum ferritin and OSI were significantly higher in study group compared to control group. Both 2 h blood glucose and HbA1c were positively correlated with serum ferritin and OSI, serum ferritin and OSI were also positively correlated with each other. Higher pre-pregnancy BMI elevates serum ferritin, which in turn increases the OSI. Both ferritin and oxidative stress raises 2 h blood glucose and HbA1c in GDM patients possibly by causing in-vivo pancreatic β –cell injury and death (ferroptosis). Serum ferritin and OSI could become newer personalized theranostic and monitoring targets in overweight/obese pregnant females especially GDM patients.

Giant cell angiofibroma (GCA) is a rare, soft-tissue neoplasm that has a tendency to arise from t... more Giant cell angiofibroma (GCA) is a rare, soft-tissue neoplasm that has a tendency to arise from the mesenchymal tissues of the head and neck, especially involving the orbital region. It is a benign neoplasm characterized by the presence of multinucleated stromal giant cells and angiectoid spaces. GCA belongs to the group of solitary fibrous tumor with histomorphological features intermediate between those of solitary fibrous tumor and giant cell fibroblastoma. CD34 immunoreactivity of tumor cells carries potential diagnostic value. Recurrence after the complete surgical excision is rare. We report the case of a 62-year-old male who presented with complaints of pain and heaviness in the lower abdomen. On computed tomography, a well-defined solid cystic mass was observed in the pelvis. Surgical resection was done, and the histopathological and immunohistochemical examination findings rendered the diagnosis of GCA. The patient had an uneventful postoperative period.

She also complained of difficulty in breastfeeding for 1 week. On examination, the right breast was normal but there was a 4 cm × 4 cm retroareolar soft lump in her left breast. Examination of the bilateral axilla was normal. A provisional diagnosis of galactocele was made and the patient was subjected to ultrasonic mammography and fine needle aspiration cytology (FNAC). Ultrasonic mammography of the left breast showed a heterogeneous, hypoechoic lesion with thick septations and internal echoes suggestive of a complex cystic lesion, leading to a differential diagnosis of either hydatid cyst or breast abscess. On FNAC, the aspirate was not milky and cytological examination was performed, which revealed hydatid scolices, hooklets, foamy macrophages and granular debris, leading to the final...

Serum IgG, IgA and IgM were estimated in 42 cases of carcinoma and 12 cases of fibroadenoma of br... more Serum IgG, IgA and IgM were estimated in 42 cases of carcinoma and 12 cases of fibroadenoma of breast. The results were compared with 20 healthy female controls. Results showed increased levels of serum IgA in carcinoma breast cases. This increase was statistically significant (p = 0.001) when compared with healthy controls while IgG and IgM values were found to be insignificant. Values of IgG, IgA and IgM in cases of fibroadenoma breast when compared with the controls were found to be insignificant. Statistically significant increased value of IgA was also observed in medullary carcinoma and non-metastasizing tumours when viewed separately, suggestive of good prognostic index of serum IgA level estimation in the case of carcinoma breast.

Serum IgG, IgA and IgM were estimated in 42 cases of carcinoma and 12 cases of fibroadenoma of br... more Serum IgG, IgA and IgM were estimated in 42 cases of carcinoma and 12 cases of fibroadenoma of breast. The results were compared with 20 healthy female controls. Results showed increased levels of serum IgA in carcinoma breast cases. This increase was statistically significant (p = 0.001) when compared with healthy controls while IgG and IgM values were found to be insignificant. Values of IgG, IgA and IgM in cases of fibroadenoma breast when compared with the controls were found to be insignificant. Statistically significant increased value of IgA was also observed in medullary carcinoma and non-metastasizing tumours when viewed separately, suggestive of good prognostic index of serum IgA level estimation in the case of carcinoma breast.
