A female software employee aged 45 years admitted in hospital with the chief complaints of back pain, sleeplessness nights due to rhythmic and pulling sensations at her lower limbs. She was an Indian origin but settled in America and came to India. Her history was found to be frequent consumption of coffee 10-12 times per day and eating coffee chocolate atleast 3 per day. Her family history was known with parkinson’s disease in her grandfather. She has past medication history. She was treated with tramadol 100mg, pantoprazole 40mg and diazepam 2mg for one week but no recovery was observed in rhythmic and pulling sensations but well responded to back pain. The present study investigates the antipyretic activity of the extracts of Ocimum sanctum plant on brewer’s yeast induced fever in experimental rats. 30 albino rats weighing 120g150g were used. They were divided in to five groups of six rats each. Group one serve as control (n=6) and was given 1ml of normal saline, group two serves as inducer group (n=6) was treated with brewer yeast alone, group three serves as standard group (n=6) was given 150mg/kg of paracetamol, while groups four and five serves as test groups were treated with 100mg/kg and 300mg/kg (n=6) of Ocimum sanctum respectively. A 20% suspension of 10ml/kg of brewer’s yeast was injected subcutaneously to induce fever in all the experimental animals. After 18hrs, the rectal temperature was taken and the animals, were administered Ocimum sanctum (100mg/kg, 300mg/kg) and paracetamol (standard group, 150mg/kg) orally. An unusual case of Huntingston’s disease (HD) was reported due to cerebral atrophy at Hyperostosi... more An unusual case of Huntingston’s disease (HD) was reported due to cerebral atrophy at Hyperostosis Frontal Interna region of brain in a 57 years female patient. Patient came to hospital with complaints of mild slurred speech, tremors in right hand, difficulty in walking, generalized myalgia, on and off fever with chills, abnormal movements of lower jaw and urinary incontinence. Her family history was known with HD in grandfather. Her past medical history was known to be with Post menopausal state. Her physical examination reported with high temperature-101 F. Her abnormal blood reports were mild depletions of Hb-10.3 g/dl (), PCV-31 g%, & RBC Count-3.6 ml/m and very low RBS levels105mg/dl. Brain CT scan report reveals diffuse cerebral atrophy at hyperostosis frontalis interna. Patient scored low in her emotion recognition tests and cognitive ability tests and finally diagnosed as HD associated with hypogycemia and hyperthermia.

A male North African patient aged 22 y* admitted in hospital in Hyderabad with chief complaints of hematuria with dysuria, fever with night sweats, and shortness of breath during exercise. His skin appears as pale. His history known that he came to India for his academics. He has a habit of swimming in lake at his country. He has no known past medical history and medication history. His blood reports were found to be low hemoglobin levels and low Red blood cells (RBCs) count. His urine analysis detected with reddish colored erythrocytes and crystals with an absence of proteins and casts. Microscopic examination of urine detects S. haematobium eggs by using repeated concentration techniques.

An unsual case was reported in 50 years male patient associated with dual conditions. He came to ... more An unsual case was reported in 50 years male patient associated with dual conditions. He came to hospital with chief complaints of mild chest pain, excessive thirst, stress and also severe headache. He is over stressed due to his office work. His abnormal reports were with elevated blood pressure and sugar levels. His urine examination appears pale yellow colour, cloudy and with the presence of sugar, pus cells and epithelial cells. He was treated with amlodipine 5 mg OD, Tab rosuvastatin 20mg HS, Inj IV pantoprazole 40mg OD for his diagnosed conditions. After 4 days under observation patient again reported elevated levels of indirect bilirubin with no hepatic symptoms. So it was understood that type-2 diabetes and hypertension became the factors to cause Gilberts syndrome. There were previous studies that Gilbert’s syndrome has a more prevalence with diabetes and hypertension individually. This case was reported with both denovo conditions of diabetes and hypertension.

Background: Different bio-analytic methods have been developed for determining drug concentration... more Background: Different bio-analytic methods have been developed for determining drug concentration in plasma, but methods for sitagliptin determination are still very rare. In this study, RP-HPLC based method has been developed for assessing sitagliptin concentration in plasma. Aim: To develop and validate RP-HPLC based analytical method for estimating sitagliptin in human plasma for pharmacokinetic applications. Methods: In the present study, the mobile phase composed of acetonitrile: 0.5% triethanolamine (20:80) with pH 6.5 has been utilized. Samples of plasma containing sitagliptin and internal standard (IS)-rosiglitazone were extracted with dichloromethane:diethyl ether (4:6; v/v) at pH 7.4. The rate of flow was 1 ml/min. The retention time was about 5,232 and 6,903 minutes respectively for sitagliptin and rosiglitazone. Respiratory distress is a rare adverse effect of alendronate that is typically associated with se... more Respiratory distress is a rare adverse effect of alendronate that is typically associated with severe dyspnoea and wheezing and typically requires hospitalization. The patient with a history of dyspnoea and wheezing during the strenuous workload was treated promptly with alendronate for newly diagnosed osteoporosis. After 2 days, respiratory distress complications were restarted and we accurately reported the patient with basophilia, elevated immunoglobulin E (with a blood test), and allergic bronchopulmonary aspergillosis (with computed tomography scan image). The prospective patient was adequately understood as alendronate-induced respiratory distress with an unfortunate rechallenge method. Although there is no direct causal relationship from this adverse case report, the possible mechanism has discussed typically based on peer-reviewed literature.

A case of leiomyoma at gastroesophageal (GE) junction was identified in a male patient with uncon... more A case of leiomyoma at gastroesophageal (GE) junction was identified in a male patient with uncontrolled hypertension and arrhythmia. Patient came with chief protestations of body pains, burping, stomach pain and uncontrolled hypertension. He was with oral medications like amlodipine 5 mg, pantoprazole 40mg, clopidogrel 75 mg, vitamin supplements and syrup antacid. His blood pressure was 150/100 mmHg. He has elevations of direct bilirubin-0.03mg/ dl and basic phosphate-192U/L. Esophagus endoscopy determines 23 × 22 mm size mixed echoic lesion with deformed cells at GE junction. His ECG demonstrates cardiac arrhythmias. Endoscopic ultrasonography and cardiac MRI identifies a mass from the submucosal layer at GE junction compressing the esophageal veins. Fibroids at GE junction were removed by thoracoscopy. Histopathological examination confirms leiomyoma with an impression of round mass lesion composed of bundle of spindle cells arranged like braid with minimal abnormal and enlarged nuclei. To his previous prescription ceftriaxone 500 mg and ibuprofen 400 mg were included.

An unordinary case report of henoch-schonlein purpura (HSP) in a relationship with hepatitis C an... more An unordinary case report of henoch-schonlein purpura (HSP) in a relationship with hepatitis C and compensated cirrhosis was identified in 14 y aged male patient. He was admitted in the pediatric department with stomach pain, yellow skin, rashes with tingling and erythematous injuries over the legs with agony and swelling since multi-day. He feels pain during walking and appears to be with swelling of lower leg muscles. His unusual liver function test was distinguished with elevated levels of bilirubin-3 mg/dl, basic phosphatase-314 U/l, aspartate aminotransferase-55 U/l and alanine aminotransferase-60 U/l. His skin biopsy shows up leukocytoclastic vasculitis and IgA depositions. Liver biopsy revealed nuclei enlargement with extensive cell change and scattered cell plates. His blood test was with the presence of the hepatitis C virus (HCV) antibodies. He was finally diagnosed as HSP associated with HCV and compensated cirrhosis.

Objectives 1. Retrospective survey of potential Drug-Drug interactions. 2. To analyse the risk wh... more Objectives 1. Retrospective survey of potential Drug-Drug interactions. 2. To analyse the risk which it was associated with drug interaction in hypertensive patients. 3. To identify the drug that most commonly implanted in Drug-Drug interactions and to recognise the antihypertensive drug mostly interacting with other drug. Methodology The study was done at general medicine ward of secondary care hospital in Secunderabad for a period of 9 months for the detection of drug interactions in prescriptions containing anti hypertensive drugs which was prescribed. The study involves following steps  Collection of prescription (schedule method).  Recording of prescription details.  Analyzing prescriptions and finding the individual drug interaction. Findings A total of 151 prescriptions were collected on the basis of inclusion (age, sex, other diseases like DM, etc) and exclusion (pregnant) criteria and analyzed for drug-drug interactions. It is shown that 20% (31) of prescriptions having drug-drug interactions in hypertensive patients. From these amlodipine is interacting with 8 drugs, furosemide with 5 drugs, atenolol with 6 drugs, enalapril with 5 drugs, ramipril with 3, digoxin with 2, nifedipine with 2, nebivolol with 2. From this study we concluded that amlodipine and digoxin, were commonly interacting with more no of drugs and nifedipine and nebivolol are least. Those 31 drug interactions case sheets were individually again studied and found that there were 26 different drug combinations causing interactions. Some drugs were interacting with more than one drug in the list. The severity of these 26 different interactions was founded that there were 4 major interaction and 15 moderate interactions. From this individualized study, amlodipine was found to be most interacting drug and combination which repeated mostly was amlodipine and atenolol, causing reduction in heart rate, cardiac contractility and hypotension. Conclusion It is concluded that the drug interaction cases constitutes 20% among the 151 hypertensive prescriptions. In future these drug interactions effectively overcome by  Dose adjustment.  Monitor blood pressure.  Use combination with caution.  Clinically monitoring the patient.

His skin biopsy shows up leukocytoclastic vasculitis and IgA depositions. Liver biopsy revealed nuclei enlargement with extensive cell change and scattered cell plates. His blood test was with the presence of the hepatitis C virus (HCV) antibodies. He was finally diagnosed as HSP associated with HCV and compensated cirrhosis.

To analyse the risk which it was associated with drug interaction in hypertensive patients. 3. To identify the drug that most commonly implanted in Drug-Drug interactions and to recognise the antihypertensive drug mostly interacting with other drug. Methodology The study was done at general medicine ward of secondary care hospital in Secunderabad for a period of 9 months for the detection of drug interactions in prescriptions containing anti hypertensive drugs which was prescribed. The study involves following steps  Collection of prescription (schedule method).  Recording of prescription details.  Analyzing prescriptions and finding the individual drug interaction. Findings A total of 151 prescriptions were collected on the basis of inclusion (age, sex, other diseases like DM, etc) and exclusion (pregnant) criteria and analyzed for drug-drug interactions. It is shown that 20% (31) of prescriptions having drug-drug interactions in hypertensive patients. From these amlodipine is interacting with 8 drugs, furosemide with 5 drugs, atenolol with 6 drugs, enalapril with 5 drugs, ramipril with 3, digoxin with 2, nifedipine with 2, nebivolol with 2. From this study we concluded that amlodipine and digoxin, were commonly interacting with more no of drugs and nifedipine and nebivolol are least. Those 31 drug interactions case sheets were individually again studied and found that there were 26 different drug combinations causing interactions. Some drugs were interacting with more than one drug in the list. The severity of these 26 different interactions was founded that there were 4 major interaction and 15 moderate interactions. From this individualized study, amlodipine was found to be most interacting drug and combination which repeated mostly was amlodipine and atenolol, causing reduction in heart rate, cardiac contractility and hypotension. Conclusion It is concluded that the drug interaction cases constitutes 20% among the 151 hypertensive prescriptions. In future these drug interactions effectively overcome by  Dose adjustment.  Monitor blood pressure.  Use combination with caution.  Clinically monitoring the patient.
