The Baby-Friendly Hospital Initiative was launched in 1991 to encourage maternity services to support and educate mothers to breastfeed by implementing Ten Steps to Successful Breastfeeding. Research Aim: To explore midwives’ experiences of implementing the Baby-Friendly Hospital Initiative in a Baby-Friendly accredited public hospital in Australia. Methods: In this prospective, cross-sectional qualitative study we used focus groups to explore midwives’ experiences. Background Breastfeeding has positive impacts on the health, environment, and economic wealth of ... more Background Breastfeeding has positive impacts on the health, environment, and economic wealth of families and countries. Nevertheless, barriers to accessing high-quality breastfeeding support are evident in the low global exclusive breastfeeding rate of 41%. The World Health Organization (WHO) launched the Baby Friendly Hospital Initiative (BFHI) in 1991 as a global program to incentivize maternity services to implement the Ten Steps to Successful Breastfeeding (Ten Steps). These were developed to ensure that maternity services remove barriers for mothers and families to successfully initiate breastfeeding and to continue breastfeeding through referral to community support after hospital discharge. However, in 2020 only 26% of Australian hospitals were BFHI-accredited. This study aimed to examine the social return on investment (SROI) of implementing the BFHI in one public maternity unit in Australia. Method The study was non-experimental and conducted in the maternity unit of an Australian BFHI-accredited public hospital with around 1000 births annually. This facility illustrated costs for BFHI implementation in a relatively a uent urban population, and more than three in four births in Australia take place in public hospitals. Stakeholders considered within scope of the study were the mother-baby dyad and the maternity facility. We interviewed the hospital's Director of Maternity Services and the Clinical Midwifery Educator, guided by a structured questionnaire, which examined the cost (nancial, time and other resources) and bene ts of each of the Ten Steps. Analysis was informed by the Social Return on Investment (SROI) framework, which uses monetary values to measure social, environmental and economic outcomes of change. This information was supplemented with micro costing studies from the literature that measure the bene ts of the BFHI.

This study aimed to evaluate the ten steps to successful breastfeeding policy implementation as t... more This study aimed to evaluate the ten steps to successful breastfeeding policy implementation as the basis to enhance the achievement of exclusive breastfeeding practices in the Kediri District. This study was an evaluative study using the CIPP (Context, Input, Process, Product) model. There were seven inpatient primary healthcare centres involved in this study. Respondent in this study were the head or staff of primary healthcare centres (7 people) and all midwives (94 people). The results show that based on the context evaluationthe objective of the policy implementation reached 77.6%. The input evaluation showed that the infrastructure and facilities, as well as the health personnel' support, was not adequate with the value of 56.4%. In terms of the process evaluation, the primary healthcare centres have not formulated the policy and have not formed supporting groups for breastfeeding, thusthe score only reached 73.9%. The product evaluation, the exclusive breastfeeding decreased in 2018 and stay behind the trend in the past four years. It has not met the target of a strategic plan formulated by the regional government. In conclusion, the policy of ten steps to successful breastfeeding in the inpatient primary healthcare centres has not been implemented well. It is recommended that the local government should formulate regulation about exclusive breastfeeding. The District Health Office should socialize the ten steps to successful breastfeeding and include it in the assessed elements in the accreditation of primary healthcare centres. In addition, it is necessary to form a KP-ASI group in the community.

While there is evidence of the effectiveness of those Ten Steps in increasing breastfeeding rates, there has been no published analysis of the key differences between the two versions. We aim to summarise the key changes in each of the Ten Steps and explore the benefits and cost implications. We first review the background to recent changes and then compare the evolution of each of the Ten Steps since 1989. Thirdly, we explore the implications of new implementation guidelines in terms of the cost and benefits from different perspectives. Revisions are subtle, yet meaningful for implementation. A major change made by WHO is subdividing the Ten Steps into 1) critical management procedures, and 2) key clinical practices. Based on the initial survey at the Nangggalo Health Center already using the e-Puskesmas application in the recording and reporting section only the officers still use manual reporting because of the lack of facilities in the implementation of recording and reporting and work placement not in accordance with the profession. The purpose of this research is to analyze the implementation integrated puskesmas recording and reporting system at the Nanggalo Padang Health Center in 2018. The study was conducted at the Nanggalo Health Center on 02 to 14 July 2018, the type of research conducted was qualitative with a phenomenological approach. The informants in this study were 4 people, all informants were scrutinized by using in-depth interview guidelines. Data analysis in this study uses the Colaizzi model approach. The sampling technique is purposive sampling. The results obtained are for input components such as human resources are quite good, but for SP2TP officers there is no information technology department, there is no special facility for SP2TP, and no special funds for SP2TP. For the process component, the manual recording and reporting of the SP2TP implementation is not yet available, and there is no monitoring and evaluation at the Nanggalo health center. Based on the above conclusions, the researcher suggested to the Puskesmas that there should be additional human resources for D3 graduates medical records and health information in accordance with Permenpan No. 30 of 2013, providing facilities and funds to support the implementation of SP2TP, in the process of recording and reporting should use the e-Puskesmas application and need the existence of monitoring and evaluation in the implementation of SP2TP.

Penelitian ini bertujuan untuk menganalisa kepuasan pelanggan sebelum dan sesudah RSUD Kab Sidoarjo menjadi badan layanan umum daerah (BLUD). Penelitian ini menggunakan pendekatan observasi. Responden adalah 150 orang pasien rawat jalan dan rawat inap yang menggunakan jasa rumah sakit pada saat penelitian dilaksanakan, sebelum (sebelum 1998) dan sesudah rumah sakit menjadi BLUD. Penelitian ini dilaksanakan tgl 28 agustus 2013 sampai 2 september 2013 dengan metode observasi dan wawancara dan menggunakan questionnaire. Hasil-nya mayoritas pelanggan merasa puas , selebihnya tidak memuaskan.

The rate of exclusive breastfeeding in Indonesia did not meet the national target which was set a... more The rate of exclusive breastfeeding in Indonesia did not meet the national target which was set at 80% in 2014 but reduced to 50% in 2019. Based on Basic Health Research (2013), only 38% of babies aged 0-5 months were exclusively breastfed. The Indonesian Ministry of Health (Indonesian Health Profile, 2016) reported that only 55.7% of babies were exclusively breastfed. Though it seems that the national target rate was reached, in fact some provinces had low exclusive breastfeeding rates. Indonesia has published regulations for breastfeeding such as 'Government Regulation Number 33 Year 2012 on Exclusive Breastfeeding'. The objective of this research was to analyse breastfeeding policy and its implementation in Indonesia. The research methodology was a literature review on Indonesian regulation. The review demonstrates that policy implementation still needs to be monitored and evaluated. Recommendations are made for the medical professional association and the government.

In 2018, Indonesia had a lower exclusive breastfeeding rate of 37% at 6 months postpartum; ranging from 20% to 56%, showing unequal breastfeeding support throughout the country. The World Health Organization (WHO) launched the Ten Steps to Successful Breastfeeding (Ten Steps) in 1989, later embedded in UNICEF’s Baby-Friendly Hospital Initiative (BFHI) program in 1991. The BFHI aims to encourage maternity facilities worldwide to ensure adequate education and support for breastfeeding mothers by adhering to the Ten Steps and complying with the International Code of Marketing of Breastmilk Substitutes. Based on the initial survey at the Nangggalo Health Center already using the e-Puskesmas application in the recording and reporting section only the officers still use manual reporting because of the lack of facilities in the implementation of recording and reporting and work placement not in accordance with the profession. The purpose of this research is to analyze the implementation integrated puskesmas recording and reporting system at the Nanggalo Padang Health Center in 2018. The study was conducted at the Nanggalo Health Center on 02 to 14 July 2018, the type of research conducted was qualitative with a phenomenological approach. The informants in this study were 4 people, all informants were scrutinized by using in-depth interview guidelines. Data analysis in this study uses the Colaizzi model approach. The sampling technique is purposive sampling. The results obtained are for input components such as human resources are quite good, but for SP2TP officers there is no information technology department, there is no special facility for SP2TP, and no special funds for SP2TP. For the process component, the manual recording and reporting of the SP2TP implementation is not yet available, and there is no monitoring and evaluation at the Nanggalo health center. Based on the above conclusions, the researcher suggested to the Puskesmas that there should be additional human resources for D3 graduates medical records and health information in accordance with Permenpan No. 30 of 2013, providing facilities and funds to support the implementation of SP2TP, in the process of recording and reporting should use the e-Puskesmas application and need the existence of monitoring and evaluation in the implementation of SP2TP.

Penilain prestasi merupakan salah satu petunjuk untuk menilai kualitas pelayanan rumah sakit. Penelitian ini bertujuan untuk menganalisa kepuasan pelanggan sebelum dan sesudah RSUD Kab Sidoarjo menjadi badan layanan umum daerah (BLUD). Penelitian ini menggunakan pendekatan observasi. Responden adalah 150 orang pasien rawat jalan dan rawat inap yang menggunakan jasa rumah sakit pada saat penelitian dilaksanakan, sebelum (sebelum 1998) dan sesudah rumah sakit menjadi BLUD. Penelitian ini dilaksanakan tgl 28 agustus 2013 sampai 2 september 2013 dengan metode observasi dan wawancara dan menggunakan questionnaire. Hasil-nya mayoritas pelanggan merasa puas , selebihnya tidak memuaskan.

It was conducted at Kendangsari Merr Mother and Child Hospital from 27 to 31 October 2014. The respondents were taken from a total population of all inpatients and outpatients during the survey. The rate of exclusive breastfeeding in Indonesia did not meet the national target which was set a... more The rate of exclusive breastfeeding in Indonesia did not meet the national target which was set at 80% in 2014 but reduced to 50% in 2019. Based on Basic Health Research (2013), only 38% of babies aged 0-5 months were exclusively breastfed. The Indonesian Ministry of Health (Indonesian Health Profile, 2016) reported that only 55.7% of babies were exclusively breastfed. Though it seems that the national target rate was reached, in fact some provinces had low exclusive breastfeeding rates. Indonesia has published regulations for breastfeeding such as 'Government Regulation Number 33 Year 2012 on Exclusive Breastfeeding'. The objective of this research was to analyse breastfeeding policy and its implementation in Indonesia. The research methodology was a literature review on Indonesian regulation. The review demonstrates that policy implementation still needs to be monitored and evaluated. Recommendations are made for the medical professional association and the government.
