Major Aircraft Accident
CH-53 S/N 68-10933
15 May 1975
This is the collateral investigation report regarding the root cause of the loss of this aircraft and the resultant fatalities of all 23 Airmen on board. The aircraft (call sign Knife 01-3) was on operational deployment from Nakhon Phanom RTAFB to U-Tapao in support of the USS Mayaguez rescue operation. The loss occurred on 13 May 1975.
The root cause is stated beginning on page 130 of the PDF file. Personnel SSN's have been redacted for privacy.

Subject: I have the same report
I’ve also got an original copy of this report along with hundreds of other reports that contradict the final conclusion of rotor blade; spindle sleeve issues. I’ve even got photos of the crash site and the hole in the tail from a missile. My uncle was on that helo. Myself and the other families are contacting the AF as to how this document became an open source document.
