Introduction: Multiple foreign body aspiration (FBA) into the airway is an emergency condition, w... more Introduction: Multiple foreign body aspiration (FBA) into the airway is an emergency condition, which is frequently encountered in emergency services, requires immediate intervention, and could lead to troublesome situation in case of delayed diagnosis and treatment. A detailed anamnesis plays an important role in diagnosis. Overlooked or delayed diagnosis could lead to unintended situations that might result in loss of lung tissue. Case Presentation: We present a 71-year-old female patient who aspirated a wishbone 20 years ago and did not consult a physician due to regression of her complaints. The patient was referred to our clinic with hemoptysis. A foreign body was detected in her airway and accordingly she had to undergo left pneumonectomy, which suggested that delayed diagnosis of FBA leads to serious problems including loss of lung tissue, as was in the present case. Conclusion: In such cases, physical examination should be performed cautiously and a precise radiological scanning should be carried out. We recommend rigid or flexible bronchoscopy in the event of suspicious FBA into the airway.

This is a case report describing perforation of a Dacron aortic graft by the sharp margin of a displaced rib, which occurred after a thoracotomy. This case illustrates the importance of fixing fractured ribs.

It may be seen more common in patients who have primary underlying causes such as neurological diseases, mental retardation, alcohol or sedative misuse. FBA in adults can be difficult to diagnose because the symptoms are nonspecific and chest X-rays may be normal. Occult FBA may lead to a mistaken clinical diagnosis of asthma, bronchitis, chronic pneumonia, recurrent pneumonia and bronchiectasis (1, 2). In Turkey, olive consumption at breakfast is very common particularly among adults. We presented a case of olive aspiration which is extremely rare. A 64-year-old female adult patient was admitted to our clinic with a 1-week history of fever, productive cough and shortness of breath. Vital signs on admission revealed a pulse of 80 beats/min, blood pressure of 120/80 mmHg, respiratory rate of 14 breaths/min and a temperature of 37.5°C. Chest examination revealed crackles on the right lower area. The rest of the systemic examination was unremarkable. Chest X-ray revealed pneumonic infiltrates in the right middle and lower lung zones. The laboratory tests on admission showed a leucocyte count of 12 300/μL (4.0–10/μL) with 72% neutrophils. C-reactive protein was at 18 μmol/L (0–0.5 mg/dL). She was diagnosed with community acquired pneumonia involving the right lower lobe and parenteral antibiotic therapy was initiated. Objective: VATS (video-assisted thoracic surgery) is commonly used for variable indications in th... more Objective: VATS (video-assisted thoracic surgery) is commonly used for variable indications in thoracic surgery procedures such as pleural biopsy and lung resections. The use of VATS in our clinic for various diseases was reviewed. Methods: The files of 45 patients that underwent VATS were assessed retrospectively between May 2009 and March 2011. Cases consisted of 21 males and 24 females; the age range was 15 to 72 years and the mean age was 53.5 years. Results: Fifty-one operations were performed on the 45 cases. A total of six cases, including five cases of hyperhidrosis and one case of pleural effusion, underwent simultaneous bilateral operations. VATS was performed for pleural effusion and/or pleural thickening in 23 cases, for a lung nodule or lung mass in 13, for pneumothorax and prolonged air leak in four, and for hyperhidrosis-sympathetic hyperactivity in five cases. The pathological analyses of 16 patients with pleural effusion indicated malignant disease while seven patients had benign findings. Nine of the patients with pulmonary nodules were reported as benign while four were reported as malignant by pathology. In one patient who underwent wedge resection for pneumothorax, prolonged air leakage occurred and the patient underwent reoperation. Death occurred in one patient. Conclusion: VATS is used for diagnostic and therapeutic purposes in the practice of thoracic surgery. It is the preferred method for selected patients.

Klinigimizde cesitli hastaliklarda VATS uygulanilan cerrahiler gozden gecirildi. Yontemler : Mayis 2009-Mart 2011 tarihleri arasinda VATS uygulanilan 45 hastanin kayitlari retrospektif olarak degerlendirildi. Hastalarin 21’i erkek, 24’u kadin idi. Yaslari 15-72 arasinda degismekteydi (ortalama yas 53.5). Bulgular : Kirk bes hastaya 51 operasyon yapildi. Plevral efuzyon ve/veya plevral kalinlasma nedeniyle 23, akcigerde nodul veya kitle nedeniyle 13, pnomotoraks veya uzamis hava kacagi nedeniyle 4, hiperhidrozis ve sempatik hiperaktivite nedeniyle 5 olguya VATS yapildi. Yazımızda, akciğer kanseri nedeniyle pnömonektomi yapılan ve taburculuğunun 20. gününde ani başlayan göğüs ağrısı ve nefes darlığı ile kliniğimize başvuran 59 yaşında erkek hastayı sunduk. Toraks bilgisayarlı tomografi anjiografi incelemesinde, sol ana pulmoner arter distal kesiminde ve üst-orta lob segmenter-subsegmenter dallarında parsiyel ve yer yer total oklüzyona yol açan trombüs formasyonları izlendi. Tanı sonrasında hastaya günde iki defa olmak üzere düşük molekül ağırlıklı heparin(DMAH) tedavisi başlandı. Tedavinin altıncı günü hasta sorunsuz bir şekilde taburcu edildi.

Adenoid cystic carcinoma of the lower respiratory tract is an uncommon tumor that can arise in th... more Adenoid cystic carcinoma of the lower respiratory tract is an uncommon tumor that can arise in the mainstem bronchus and often presents as an endobronchial mass lesion causing bronchial obstruction with post obstructive atelectasis and pneumonia. Exfoliative cytology is seldom useful in the diagnosis of primary bronchial adenoid cystic carcinoma, because these neoplasms usually have a submucosal location with often intact mucosa. Since most endobronchial adenoid cystic carcinomas are endoscopically visible, bronchoscope-guided fine-needle aspiration constitutes an excellent approach to establish a pathologic diagnosis. The fine-needle aspiration cytology of primary pulmonary adenoid cystic carcinoma has been rarely described. We report a case of primary adenoid cystic carcinoma of the lung having characteristic cytologic features and correlate with computed tomography, bronchoscopic, and histological findings. Bronchoscope-guided aspiration cytology provided a conclusive diagnosis of adenoid cystic carcinoma, which was further corroborated by histology in the pneumonectomy specimen.

Olgular yaş, cinsiyet, semptom,lezyon lokalizasyonu, girişim yolu, seçilen cerrahi yöntem açısından retrospektif olarak değerlendirildi ve literatür bilgileri ile değerlendirildi. Akciğer kist hidatikleri en sık sağ akciğer alt loba yerleşmekteydi. En sık kullanılan yöntem kistotomi ve kapitonajdı. Sağ akciğer ve karaciğer kist hidatik birlikteliğinde sağ torakotomiden transtorasik olarak frenotomi ile her iki bölgedeki kiste de müdahale edildi. Akciğer kist hidatiği ön tanısı olan hiçbir hastaya iğne biopsisi ya da medikal tedavi uygulanmamıştır. Akciğer ya da karaciğer kist hidatik tanısı alan hastalar bu iki organ açısından mutlaka taranmalıdır. Akciğer kist hidatiği gelişmekte olan ülkeler için halen ciddi bir sağlık sorunudur. In patients with pulmonary embolism (PE), a pulmonary radiograph may reveal oligemic fields (the ... more In patients with pulmonary embolism (PE), a pulmonary radiograph may reveal oligemic fields (the Westermark sign) associated with sites of occlusion of the pulmonary arteries, interruption or loss of the artery line (the knuckle sign), and even unilateral hyperlucency attributable to reduced overall lung vascularity. In Swyer–James–Macleod syndrome (SJMS), which develops as a result of bronchiolitis obliterans, unilateral hyperlucency is evident because of emphysema and hypoplasia of the pulmonary artery and its branches. Therefore, SJMS cases with clinical and laboratory data compatible with PE may in fact be confused with PE. The cases of six adult patients who were initially presumed to have PE but on further investigation were diagnosed with SJMS are presented in this report, which thus can serve as a guide for diagnosis of similar cases in future.

The aim of this study was to compare survival and mortality rates of two groups with different ages, younger and older than 70 years, who went curative resection for nonsmall-cell lung cancer. We analyzed the patients who underwent curative lung cancer surgery in the Department of Thoracic Surgery of Gaziantep University Research Hospital between January 1997 and November 2014. Patients were divided into 2 groups according to their ages. A total of 497 patients were included in data analysis (381 were under 70 years old and 116 of them were ≥70 years old). Bu çalışmanın amacı, eş zamanlı akciğer ve karaciğer kist hidatikli hastalarımızdaki tek aşamalı ... more Bu çalışmanın amacı, eş zamanlı akciğer ve karaciğer kist hidatikli hastalarımızdaki tek aşamalı operasyon deneyimlerimizi ve sonuçlarımızı incelemektir. Gereç ve Yöntem: 2002 ile 2011 yılları arasında akciğerin yanı sıra, eş zamanlı karaciğer kist hidatiği de mevcut olan 29 olgu incelendi. Bu olguların 21'inde, karaciğer kistleri kubbe yerleşimliydi. Bu olgular sağ torakotomi ve frenotomi ile opere edildiler. Yirmibir olguda, yaş, cinsiyet, semptomlar, kist boyutları, cerrahi yöntem, operasyon süresi ve komplikasyonlar retrospektif olarak incelendi. Bulgular: Ortalama yaş 60,0±16,72 yıl olarak hesaplandı. Operasyon uygulanan 21 hastanın 8'i erkek,13'ü kadın idi. Öksürük ve yan ağrısı en sık karşılaşılan semptom iken, 2 hastada izole karın ağrısı görüldü. 4 hasta ise asemptomatikti. Akciğer kistleri için, kist çapı ortalama 7,50±1,29 cm , karaciğer için 7,0±1,76 cm olarak bulundu. Akciğerdeki kistler kistotomi ve/veya kapitonaj yada wedge yöntemi ile tedavi edildiler. Karaciğerdeki kistler için seçilen tedavi yöntemi ise kistotomi ve kavitenin inversiyonu ile subdiyafragmatik bölgenin drene edilmesiydi. Operasyon süresi ortalama 145,0±15,26 dakika idi. İki hastada uzamış safra drenajı gözlendi. Mortalite izlenmedi. Tartışma: Eş zamanlı sağ akciğer ve subdiyafragmatik karaciğer hidatik kistli hastalarda, sağ torakotomi ve frenetomi ile transdiyafragmatik olarak karaciğer kistine müdahale, güvenilir ve efektif bir yöntemdir.

Gereç ve Yöntem: Olgular yaş, cinsiyet, semptom, kistin lokalizasyonu, girişim yolu, seçilen cerrahi yöntem açısından retrospektif olarak değerlendirildi ve literatür bilgileri ile karşılaştırıldı. Bulgular: Çalışmada 73 hasta incelendi. Hastaların yaş aralığı 6-73 idi,ortalama yaş 38,2 olarak hesaplandı. Olguların 36 sı kadın 37 si erkek hastaydı. En sık görülen semptom öksürük olarak izlendi. Akciğer kist hidatikleri en sık sağ akciğer alt loba yerleşmekteydi. En sık kullanılan yöntem kistotomi ve kapitonajdı. Girişim yolu olarak tüm hastalara posterolateral torakotomi uygulandı. Perikard yerleşimli kist hidatik nedeniyle bir hastaya median sternotomi uygulandı. Sağ akciğer ve karaciğer kist hidatik birlikteliğinde sağ torakotomiden transtorasik olarak frenotomi ile her iki bölgedeki kiste de müdahale edildi. Tartışma: Akciğer kist hidatiği gelişmekte olan ülkeler için halen ciddi bir sağlık sorunudur. Akciğer ya da karaciğer kist hidatik tanısı alan hastalar bu iki organ açısından mutlaka taranmalıdır. Günümüzde akciğer veya akciğer karaciğer kistlerinde en etkin tedavi olan cerrahi, düşük morbidite ve mortalite ile yapılmaktadır.

Olgular yaş, cinsiyet, semptom,lezyon lokalizasyonu, girişim yolu, seçilen cerrahi yöntem açısından retrospektif olarak değerlendirildi ve literatür bilgileri ile değerlendirildi. Akciğer kist hidatikleri en sık sağ akciğer alt loba yerleşmekteydi. En sık kullanılan yöntem kistotomi ve kapitonajdı. Sağ akciğer ve karaciğer kist hidatik birlikteliğinde sağ torakotomiden transtorasik olarak frenotomi ile her iki bölgedeki kiste de müdahale edildi. Akciğer kist hidatiği ön tanısı olan hiçbir hastaya iğne biopsisi ya da medikal tedavi uygulanmamıştır. Akciğer ya da karaciğer kist hidatik tanısı alan hastalar bu iki organ açısından mutlaka taranmalıdır. Akciğer kist hidatiği gelişmekte olan ülkeler için halen ciddi bir sağlık sorunudur. Swyer–James–Macleod syndrome mimicking an acute pulmonary embolism: a report of six adult cases and a retrospective analysis
In patients with pulmonary embolism (PE), a pulmonary radiograph may reveal oligemic fields (the ... more In patients with pulmonary embolism (PE), a pulmonary radiograph may reveal oligemic fields (the Westermark sign) associated with sites of occlusion of the pulmonary arteries, interruption or loss of the artery line (the knuckle sign), and even unilateral hyperlucency attributable to reduced overall lung vascularity. In Swyer–James–Macleod syndrome (SJMS), which develops as a result of bronchiolitis obliterans, unilateral hyperlucency is evident because of emphysema and hypoplasia of the pulmonary artery and its branches. Therefore, SJMS cases with clinical and laboratory data compatible with PE may in fact be confused with PE. The cases of six adult patients who were initially presumed to have PE but on further investigation were diagnosed with SJMS are presented in this report, which thus can serve as a guide for diagnosis of similar cases in future.

The aim of this study was to compare survival and mortality rates of two groups with different ages, younger and older than 70 years, who went curative resection for nonsmall-cell lung cancer. We analyzed the patients who underwent curative lung cancer surgery in the Department of Thoracic Surgery of Gaziantep University Research Hospital between January 1997 and November 2014. Patients were divided into 2 groups according to their ages. A total of 497 patients were included in data analysis (381 were under 70 years old and 116 of them were ≥70 years old). Bu çalışmanın amacı, eş zamanlı akciğer ve karaciğer kist hidatikli hastalarımızdaki tek aşamalı ... more Bu çalışmanın amacı, eş zamanlı akciğer ve karaciğer kist hidatikli hastalarımızdaki tek aşamalı operasyon deneyimlerimizi ve sonuçlarımızı incelemektir. Gereç ve Yöntem: 2002 ile 2011 yılları arasında akciğerin yanı sıra, eş zamanlı karaciğer kist hidatiği de mevcut olan 29 olgu incelendi. Bu olguların 21'inde, karaciğer kistleri kubbe yerleşimliydi. Bu olgular sağ torakotomi ve frenotomi ile opere edildiler. Yirmibir olguda, yaş, cinsiyet, semptomlar, kist boyutları, cerrahi yöntem, operasyon süresi ve komplikasyonlar retrospektif olarak incelendi. Bulgular: Ortalama yaş 60,0±16,72 yıl olarak hesaplandı. Operasyon uygulanan 21 hastanın 8'i erkek,13'ü kadın idi. Öksürük ve yan ağrısı en sık karşılaşılan semptom iken, 2 hastada izole karın ağrısı görüldü. 4 hasta ise asemptomatikti. Akciğer kistleri için, kist çapı ortalama 7,50±1,29 cm , karaciğer için 7,0±1,76 cm olarak bulundu. Akciğerdeki kistler kistotomi ve/veya kapitonaj yada wedge yöntemi ile tedavi edildiler. Karaciğerdeki kistler için seçilen tedavi yöntemi ise kistotomi ve kavitenin inversiyonu ile subdiyafragmatik bölgenin drene edilmesiydi. Operasyon süresi ortalama 145,0±15,26 dakika idi. İki hastada uzamış safra drenajı gözlendi. Mortalite izlenmedi. Tartışma: Eş zamanlı sağ akciğer ve subdiyafragmatik karaciğer hidatik kistli hastalarda, sağ torakotomi ve frenetomi ile transdiyafragmatik olarak karaciğer kistine müdahale, güvenilir ve efektif bir yöntemdir.

Gereç ve Yöntem: Olgular yaş, cinsiyet, semptom, kistin lokalizasyonu, girişim yolu, seçilen cerrahi yöntem açısından retrospektif olarak değerlendirildi ve literatür bilgileri ile karşılaştırıldı. Bulgular: Çalışmada 73 hasta incelendi. Hastaların yaş aralığı 6-73 idi,ortalama yaş 38,2 olarak hesaplandı. Olguların 36 sı kadın 37 si erkek hastaydı. En sık görülen semptom öksürük olarak izlendi. Akciğer kist hidatikleri en sık sağ akciğer alt loba yerleşmekteydi. En sık kullanılan yöntem kistotomi ve kapitonajdı. Girişim yolu olarak tüm hastalara posterolateral torakotomi uygulandı. Perikard yerleşimli kist hidatik nedeniyle bir hastaya median sternotomi uygulandı. Sağ akciğer ve karaciğer kist hidatik birlikteliğinde sağ torakotomiden transtorasik olarak frenotomi ile her iki bölgedeki kiste de müdahale edildi. Tartışma: Akciğer kist hidatiği gelişmekte olan ülkeler için halen ciddi bir sağlık sorunudur. Akciğer ya da karaciğer kist hidatik tanısı alan hastalar bu iki organ açısından mutlaka taranmalıdır. Günümüzde akciğer veya akciğer karaciğer kistlerinde en etkin tedavi olan cerrahi, düşük morbidite ve mortalite ile yapılmaktadır.
