The spectrum of alcohol related liver injury varies from simple steatosis to cirrhosis; patients with alcoholic cirrhosis have high prevalence of complications at the time of cirrhosis diagnosis. So there is a need to study the radiology and underlying biochemical changes for early diagnosis to reduce morbidity and mortality in case of alcoholic liver disease. Methodology: the patients visiting the OPD of Medicine and suffering from ALD underwent through medical examination and then the severity of ALD was determined by radiological and biochemical findings. Patients who fulfilled the inclusion and exclusion criteria were enrolled in the study if they are willing to give written informed consent. Clinically, the condition manifests as nasal itching (pruritus), sneezing, rhinorrhea, congestion and itchy eyes. The second generation antihistamines are commonly used either as nasal sprays (azelastine) or orally (levocetirizine). Studies have demonstrated equal efficacy of azelastine and levocetirizine, but the data on Indian population is lacking. Hence, we designed this study to evaluate the effectiveness and safety of azelastine in comparison to levocetirizine in patients with allergic rhinitis.

Purpose: The purpose of this study is to evaluate the effect of a single preoperative injection o... more Purpose: The purpose of this study is to evaluate the effect of a single preoperative injection of intravitreal bevacizumab (IVB) on the visual and anatomical outcome of patients undergoing 23-gauge pars plana vitrectomy (23G PPV) for proliferative diabetic retinopathy (PDR). Materials and Methods: Medical record from patients undergoing vitrectomy for PDR were retrospectively analyzed for the last 3 years. IVB has been a routine procedure for patients fulfilling eligibility criteria for the past 3 years. Patients who did not receive IVB preoperatively were assigned as control group (Group A). Patients who received a single IVB injection (1.25 mg in 0.05 ml) preoperatively were assigned to Group B. Medical record of 100 patients who had completed 12-month follow-up were included in each group. Results: The primary outcome measure was visual outcome which was better in Group B as compared to Group A, anatomical outcome observed was 65% patients in Group A and 85% patients in Group B. The secondary outcomes were intraoperative hemorrhage and postoperative vitreous hemorrhage (VH). Intraoperative hemorrhage was seen in 40% patients in Group A and 20% patients in Group B, whereas postoperative VH was seen in 45% patients in Group A and 15% patients in Group B. Conclusion: Preoperative use of bevacizumab achieves excellent anatomical and functional success in majority of patients undergoing PPV for PDR and significantly reduced occurrence of postoperative VH. The results are encouraging when combined with transconjunctival 23 GPPV.

Management involves using topical and oral antifungal drugs. Terbinafi ne has been used commonly for the treatment of tinea corporis to prevent the emergence of resistance combination of drugs is used. Hence, this study was designed to compare the safety and effi cacy of Sertaconazole and Amphotericin B when either is used with oral Terbinafi ne. Methodology: This open-label, randomized, prospective and parallel group study was conducted on patients suffering from tinea corporis visiting the Department of Dermatology. All patients underwent a thorough clinical examination to establish the diagnosis of tinea corporis and were divided into two groups. Patients in Group A received treatment with Sertaconazole and Terbinafi ne. Group B received treatment with Amphotericin B and Terbinafi ne. At the end of treatment phase patients were assessed clinically for the disease symptoms-erythema, pruritus, and scaling; adverse effects experienced and Dermatological life quality index (DLQI). Results: Fifty two (52) patients were enrolled in the study, both groups were comparable at baseline. After 2 weeks of treatment, complete clinical cure was found in 6 patients in Group A and 5 patients in Group B. The treatment also showed improvement in clinical disease activity as there was clinical improvement seen in 22 patients in group A as compared to 25 patients in Group B. Greater number of patients in Group B reported with symptomatic improvement. There was no serious adverse event reported in both groups and DLQI score was comparable. Conclusion: The use of topical antifungals along with systemic antifungals has been found to be useful in early achievement of clinical cure and improvement in clinical symptoms of tinea corporis.

Early clinical illustrations and actual clinical exposure enable students to associate basic science and real patient situations, probably increasing their retention of knowledge. The study was conducted to introduce an alternate method of teaching-learning in pharmacology in a large classroom setting to integrate pharmacology into clinical setting for better learning and understanding of the subject. Methods: Ninety-four students of second professional MBBS of a medical college in Punjab were divided into 2 groups and were taught a 2-hour topic in pharmacology using case-based learning (CBL) method and didactic lecture (DL) method using a crossover design. Objectives: Although antidepressant medications are effective, they have a delayed onset of effec... more Objectives: Although antidepressant medications are effective, they have a delayed onset of effect. Mirtazapine, an atypical antidepressant is an important option for add-on therapy in major depression. There is insufficient data on mirtazapine in Indian population; hence this study was designed to study the add-on effect of low-dose mirtazapine with selective serotonin reuptake inhibitors (SSRIs) in major depressive disorder (MDD) in Indian population. Materials and Methods: In an open, randomized study, 60 patients were divided into two groups. In Group A (n=30) patients received conventional SSRIs for 6 weeks. In Group B (n=30) patients received conventional SSRIs with low-dose mirtazapine for 6 weeks. Patients were evaluated at baseline and then at 1, 2, 3, 4, 5, and 6 weeks. Results: There was significant improvement in Hamilton Depression Rating Scale (HDRS), Montgomery and Asberg depression rating scale (MADRS) scores (P<0.05) in both groups. Mirtazapine in low dose as add on therapy showed improvement in scores, had earlier onset of action, and more number of responders and remitters as compared to conventional treatment (P<0.05). No serious adverse event was reported in either of the groups. Conclusion: Low-dose mirtazapine as add-on therapy has shown better efficacy, earlier onset of action and more number of responders and remitters as compared to conventional treatment in MDD in Indian patients.

Introduction: Knowledge and acceptance of COVID-19 vaccine among health-care professionals (HCPs)... more Introduction: Knowledge and acceptance of COVID-19 vaccine among health-care professionals (HCPs) are an important aspect in the success of the vaccination drive in India, as HCPs are the first group to receive the vaccine. Various factors affecting the attitude of getting COVID-19 vaccine and to better address those factors will help in widespread acceptance among the general public. Materials and Methods: This was a cross-sectional study which was conducted among HCPs by circulating Google form. The form was distributed among the medical students and physicians through social networking sites such as E-mail and WhatsApp. The questionnaire consisted of 18 questions focusing on the assessment of knowledge and acceptance about COVID-19 vaccine. It was a self-administered and pilot-validated questionnaire. Results: Out of total participants, 473 (78%) were willing to take vaccination and were in the opinion that getting vaccination is a good idea. Eighty-eight percent of the participants were worried about the effectiveness of the COVID-19 vaccine. Fifty-five percent of the participants were anxious regarding getting vaccine and its efficacy. Conclusion: Knowledge and acceptance were moderate, but overall attitude of getting vaccination was positive. Majority of the HCPs are still anxious about the long-term efficacy; therefore, appropriate steps need to be taken to address the factors contributing to the low acceptance of the COVID-19 vaccine.

Background: Suturing of caesarean incision leads to reduction in maternal mortality, suturing can... more Background: Suturing of caesarean incision leads to reduction in maternal mortality, suturing can be done in either single layer or double layer. Many studies have shown both of them to be effective, with no conclusive evidence of preference of any one of them. The objective of this study was to compare method of uterine closure by single-layer and double-layer closure keeping in mind the intraoperative and postoperative morbidity in index pregnancy and risk of uterine rupture in future pregnancy. Methods: This prospective randomized controlled study was done from March 2007 to January 2009, a total of 357 women were enrolled for lower segment caesarean section either to single layer (n=188) or double layer (n=169) closure of uterine incision. Primary outcome measures studied were operating time, intraoperative blood loss, and febrile morbidity in index pregnancy and chances of uterine rupture in subsequent pregnancy. Secondary outcome measures were cystitis, wound infection and hospital stay. Results were compared by Chi-square test. Results: Patients with single layer closure had significantly (p<0.05) less operative time and estimated blood loss as compared to double layer closure. There was also a statistically significant (p<0.05) difference in febrile morbidity and hospital stay in single layer closure of uterus. There was however no significant difference in other variables also, in subsequent pregnancy with previous caesarean section there was no difference in pregnancy outcome in both groups was not statistically significant (p-value>0.05). Operative findings in subsequent pregnancy don't differ much. Conclusions: Single layer closure was associated with lesser operating time, intra-operative blood loss, febrile morbidity and hospital stay in index pregnancy as compared to double-layer closure.

Methods: Ninety-four students of second professional MBBS of a medical college in Punjab were divided into 2 groups and were taught a 2-hour topic in pharmacology using case-based learning (CBL) method and didactic lecture (DL) method using a crossover design. Their
attendance and written test score at the end of teaching session were compared. Feedback from students and faculty was taken by prestructured questionnaires.
Results : There was an increase in students’ attendance (P = .008) in CBL sessions but insignificant difference in their performance (P = .98) in the tests. Most (84%) of the students felt that CBL is a better method of teaching-learning than traditional DL. The teaching faculty
felt that the students looked more interested and were themselves more motivated for the newer method of teaching.
Conclusions: Case-based learning led to improvement in student motivation, satisfaction, and engagement. Most students and faculty accepted that CBL was an effective learning tool for pharmacology teaching in a large group setting and supported the incorporation of CBL into traditional DL teaching.

At the end of treatment phase patients were assessed clinically for the disease symptoms-erythema, pruritus, and scaling; adverse effects experienced and Dermatological life quality index (DLQI). Results: Fifty two (52) patients were enrolled in the study, both groups were comparable at baseline. After 2 weeks of treatment, complete clinical cure was found in 6 patients in Group A and 5 patients in Group B. The treatment also showed improvement in clinical disease activity as there was clinical improvement seen in 22 patients in group A as compared to 25 patients in Group B. Greater number of patients in Group B reported with symptomatic improvement. There was no serious adverse event reported in both groups and DLQI score was comparable. Conclusion: The use of topical antifungals along with systemic antifungals has been found to be useful in early achievement of clinical cure and improvement in clinical symptoms of tinea corporis.

Mirtazapine, an atypical antidepressant is an important option for add-on therapy in major depression. There is insufficient data on mirtazapine in Indian population; hence this study was designed to study the add-on effect of low-dose mirtazapine with selective serotonin reuptake inhibitors (SSRIs) in major depressive disorder (MDD) in Indian population. Materials and Methods: In an open, randomized study, 60 patients were divided into two groups. In Group A (n=30) patients received conventional SSRIs for 6 weeks. In Group B (n=30) patients received conventional SSRIs with low-dose mirtazapine for 6 weeks. Patients were evaluated at baseline and then at 1, 2, 3, 4, 5, and 6 weeks. Results: There was significant improvement in Hamilton Depression Rating Scale (HDRS), Montgomery and Asberg depression rating scale (MADRS) scores (P<0.05) in both groups. Mirtazapine in low dose as add on therapy showed improvement in scores, had earlier onset of action, and more number of responders and remitters as compared to conventional treatment (P<0.05). No serious adverse event was reported in either of the groups. Conclusion: Low-dose mirtazapine as add-on therapy has shown better efficacy, earlier onset of action and more number of responders and remitters as compared to conventional treatment in MDD in Indian patients.

Introduction: Knowledge and acceptance of COVID-19 vaccine among health-care professionals (HCPs)... more Introduction: Knowledge and acceptance of COVID-19 vaccine among health-care professionals (HCPs) are an important aspect in the success of the vaccination drive in India, as HCPs are the first group to receive the vaccine. Various factors affecting the attitude of getting COVID-19 vaccine and to better address those factors will help in widespread acceptance among the general public. Materials and Methods: This was a cross-sectional study which was conducted among HCPs by circulating Google form. The form was distributed among the medical students and physicians through social networking sites such as E-mail and WhatsApp. The questionnaire consisted of 18 questions focusing on the assessment of knowledge and acceptance about COVID-19 vaccine. It was a self-administered and pilot-validated questionnaire. Results: Out of total participants, 473 (78%) were willing to take vaccination and were in the opinion that getting vaccination is a good idea. Eighty-eight percent of the participants were worried about the effectiveness of the COVID-19 vaccine. Fifty-five percent of the participants were anxious regarding getting vaccine and its efficacy. Conclusion: Knowledge and acceptance were moderate, but overall attitude of getting vaccination was positive. Majority of the HCPs are still anxious about the long-term efficacy; therefore, appropriate steps need to be taken to address the factors contributing to the low acceptance of the COVID-19 vaccine.

Background: Suturing of caesarean incision leads to reduction in maternal mortality, suturing can... more Background: Suturing of caesarean incision leads to reduction in maternal mortality, suturing can be done in either single layer or double layer. Many studies have shown both of them to be effective, with no conclusive evidence of preference of any one of them. The objective of this study was to compare method of uterine closure by single-layer and double-layer closure keeping in mind the intraoperative and postoperative morbidity in index pregnancy and risk of uterine rupture in future pregnancy. Methods: This prospective randomized controlled study was done from March 2007 to January 2009, a total of 357 women were enrolled for lower segment caesarean section either to single layer (n=188) or double layer (n=169) closure of uterine incision. Primary outcome measures studied were operating time, intraoperative blood loss, and febrile morbidity in index pregnancy and chances of uterine rupture in subsequent pregnancy. Secondary outcome measures were cystitis, wound infection and hospital stay. Results were compared by Chi-square test. Results: Patients with single layer closure had significantly (p<0.05) less operative time and estimated blood loss as compared to double layer closure. There was also a statistically significant (p<0.05) difference in febrile morbidity and hospital stay in single layer closure of uterus. There was however no significant difference in other variables also, in subsequent pregnancy with previous caesarean section there was no difference in pregnancy outcome in both groups was not statistically significant (p-value>0.05). Operative findings in subsequent pregnancy don't differ much. Conclusions: Single layer closure was associated with lesser operating time, intra-operative blood loss, febrile morbidity and hospital stay in index pregnancy as compared to double-layer closure.

Results : There was an increase in students’ attendance (P = .008) in CBL sessions but insignificant difference in their performance (P = .98) in the tests. Most (84%) of the students felt that CBL is a better method of teaching-learning than traditional DL. The teaching faculty
felt that the students looked more interested and were themselves more motivated for the newer method of teaching.
Conclusions: Case-based learning led to improvement in student motivation, satisfaction, and engagement. Most students and faculty accepted that CBL was an effective learning tool for pharmacology teaching in a large group setting and supported the incorporation of CBL into traditional DL teaching.

Hence, this study was designed to compare the safety and efficacy of Sertaconazole and Amphotericin B when either is used with oral Terbinafine. Methodology: This open-label, randomized, prospective and parallel group study was conducted on patients suffering from tinea corporis visiting the Department of Dermatology. All patients underwent a thorough clinical examination to establish the diagnosis of tinea corporis and were divided into two groups. Patients in Group A received treatment with Sertaconazole and Terbinafine. Group B received treatment with Amphotericin B and Terbinafine.
