Medical language contains many faults. One of them is the use of cultured and elegant words witho... more Medical language contains many faults. One of them is the use of cultured and elegant words without knowing its proper significance. A second error is the recourse to using foreign words or phrases (foreignisms), particularly Anglicisms, both in their original spelling (raw foreignisms) and Hispanicised (adapted word); an overlapping mode of foreignism are so-called "calques" or loan translations. Thirdly, there is the use of words that do not exist in Spanish, palabros. Finally, the words are not correctly pronounced. In this article some examples of these errors are demonstrated and it is directed towards the appropriate use of language.

There is much variability regarding time to start of enteral nutrition in patients undergoing col... more There is much variability regarding time to start of enteral nutrition in patients undergoing colorectal surgery. In many instances such patients are postoperatively maintained with nasogastric intubation with the aim of preventing complications such as dehiscence, evisceration or eventration. We examine the clinical evidence regarding nasogastric tube placement and early feeding with reference to the PubMed, Embase, and Cochrane databases.Ia and Ib evidence was obtained from meta-analyses and prospective randomized studies, where the systematic use of a gastric decompression catheter is advised against and initiation of early feeding for colorectal surgery is recommended. Fasting does not provide any benefit after gastrointestinal surgery, and the use of nasogastric tubes does not decrease postoperative complications. However, less invasive surgery and new advances in anesthesia and analgesia are contributing to a reduction in postoperative ileus.

We have attempted to investigate whether such an immunity modulating substance as Pentoxiphylline (PX) might diminish or inhibit BT. We have chosen PX because it has immunity modulation actions and inhibits the synthesis and action of TNF-alpha, which seems to be linked to the progress of these conditions towards multiple organ failures. An experimental study was performed with 4 groups of 20 Wistar rats subjected to lipid-free parenteral nutrition (PN) over 7 days. Group A: PN; group B: PN + 50 mg/kg of PX; group C: PN + 100 mg/kg; and group D: PN + 134 mg/kg. Actinomycosis is a rare chronic infectious disease that usually develops abscesses and fistulas. ... more Actinomycosis is a rare chronic infectious disease that usually develops abscesses and fistulas. It can also mimic other diseases such as cancer, inflammatory bowel diseases or diverticulitis. We present the case of a 67-year-old woman with an unusual form of actinomycosis. Clinically, the disease simulated an inflammatory bowel disease. Computerized axial tomography, intestinal transit and colonoscopy were performed and the suspected 1 diagnosis continued to be inflammatory bowel disease. The definitive diagnosis was made after histopathological study of the surgical specimen. We highlight the diagnostic difficulty of this rare disease and the importance of prolonged antibiotic treatment.

Failure to diagnose synchronous tumors leads to errors in patient treatment and prognosis. The ex... more Failure to diagnose synchronous tumors leads to errors in patient treatment and prognosis. The existence of metachronous tumors requires strict patient follow-up to ensure early identification of the second tumor. The present study evaluates the results obtained in the application of a structured procedure for the diagnosis and follow-up of multiple colorectal carcinoma. A structured procedure was used to follow for 5 years a group of 12 patients with multiple colorectal tumors (7 synchronous and 5 metachronous) of a series of 450 colorectal neoplasms. Six synchronous tumors were diagnosed preoperatively and one intraoperative. Of the 5 metachronous neoplasms, 4 strictly adhered to the follow-up protocol, as a result of which the second tumor was detected at an early stage. The remaining case involved no follow-up, and the second tumor was diagnosed in an advanced stage as a result of bowel occlusion. The authors present a study in two groups of Wistar rats, which were given two different parenter... more The authors present a study in two groups of Wistar rats, which were given two different parenteral nutritions with two different amounts of proteins, during different periods of time (4, 7 and 11 days), with the objective of evaluating their influence on the healing of a colonic anastomosis which was performed prior to the initiation of the TPN. To asses the condition of the colonic anastomosis, we studied the rupture pressure of the colon containing the anastomosis, the hydroxyproline concentration of said anastomosis, and the histological study of the same. The results indicate that the group of animals which were fed with a hyperproteinic diet during a longer period of time, showed a better degree of healing than the rest of the study groups.

The syndrome of duodenal obstruction by the mesenteric artery is an infrequent condition that nev... more The syndrome of duodenal obstruction by the mesenteric artery is an infrequent condition that nevertheless was too often diagnosed in the past. The development of modern imaging techniques has presently restricted the diagnosis of this syndrome. We report the case of a 21-year-old patient with superior mesenteric artery-induced duodenal compression syndrome secondary to malnutrition and loss of weight. Diagnosis was confirmed by the evaluation of bowel transit, CT Scan, NMR and angio-MR. Conservative treatment with total parenteral nutrition proved ineffective. Surgical sectioning of the ligament of Treitz and extensive mobilization of the duodenum (Strong's operation) was performed; outcome was unsatisfactory, however. A duodenojejunostomy was performed later, which finally solved the condition. We emphasize the diagnostic usefulness of CT Scan and angio-MR, and the favorable results afforded by duodenojejunostomy.

We describe the medical-surgical management of a patient with a complex inflammatory bowel diseas... more We describe the medical-surgical management of a patient with a complex inflammatory bowel disease who developed 2 acute episodes of pyoderma gangrenosum and enterocutaneous fistulas after ileal pouch-anal anastomosis for ulcerative colitis. The rarity of this postsurgical complication is emphasized. A good response to topical tacrolimus was achieved in cutaneous wounds. A less favorable response to infliximab was achieved in the abdominal fistulas, requiring surgical excision of the pouch.

La actinomicosis es una rara enfermedad infecciosa crónica que suele desarrollar abscesos y fístu... more La actinomicosis es una rara enfermedad infecciosa crónica que suele desarrollar abscesos y fístulas y que puede simular otras entidades como el cáncer, la enfermedad inflamatoria intestinal o la diverticulitis. Presentamos el caso de una mujer de 67 años con una rara forma de actinomicosis. Clínicamente simulaba una enfermedad inflamatoria intestinal y tras realizar tomografía axial computarizada, tránsito intestinal y colonoscopia el diagnóstico de sospecha continuó siendo enfermedad inflamatoria intestinal. El diagnóstico definitivo se realizó tras el estudio anatomopatológico de la pieza de resección. Resaltamos la dificultad diagnóstica de esta rara entidad y la importancia del tratamiento antibiótico prolongado.

Esta pauta postoperatoria retrasaba el inicio de la alimentación oral hasta que aparecía el peristaltismo intestinal, lo que contribuía, a alargar las estancias.

Methods Colonic occlusion was induced in two groups of Wistar rats. One group was administered octreotide. At 48 hours, the animals underwent relaparotomy. The degree of occlusion and intestinal contents were evaluated and an intestinal fragment was resected for histological study. Termino-terminal anastomosis was performed. At 7 days, the animals again underwent surgery to evaluate complications and to determine the bursting strength of the anastomosis, collagen content, and histology. Results The octreotide-treated group presented greater intestinal radius and fecal load in the occluded loop, as well as greater bursting pressure and bursting wall tension and these differences were statistically significant. Dehiscence, leakage, and complications were similar in both groups. Hydroxyproline value was higher in the treated group but this difference was not statistically significant. The absence of ischemia in the occluded loop was greater in the treated group and this value predicts greater bursting wall tension of the subsequent anastomotic scar. Conclusions Octreotide treatment in experimental colonic occlusion improves the intestinal wall before anastomosis.
