Volatile anaesthetics also directly manipulate glucose homeostasis by affecting pancreatic insulin release and induce hyperglycaemia without surgical stress. We determined the preoperative application of transcutaneous electrical nerve stimulation (TENS) to the Chinese acupoints ST36 (Zusanli) and SP6 (Sanyinjiao) as a complementary therapy for controlling plasma glucose and improving insulin resistance during anaesthesia. We designed a single-blind, randomised controlled clinical study of female patients, scheduled for elective hysterectomy. SSIs often require extended hospitalisation and may worsen overall clinical outcomes. A retrospective database review of consecutive patients with traditional open lumbar spinal surgery was performed. SSIs patients were identified and reviewed for clinically relevant details, and postoperative SSIs' incidence was calculated for the entire cohort as well as for subgroups with or without spinal implants. In 15 years, 1,176 patients underwent open lumbar spinal surgery with spinal implants and 699 without. Thirty-eight developed postoperative SSIs. Total SSI rate for the entire group was 2.03%. The incidence of postoperative SSIs in the nonimplant group was relatively low. Patients received antibiotics, hyperbaric oxygen therapy, and wet dressing. Lumbar spinal surgery is a common procedure to treat low back pain. To remark that lower limb per... more Lumbar spinal surgery is a common procedure to treat low back pain. To remark that lower limb perfusion insufficiency may induce the similar syndrome as neuropathic pain, 1,076 cases of postspinal surgery was surveyed within 5 years, in which 51 cases had no obvious improvement 3 months after surgery; 27 cases received second operation because of instability, inadequate decompression, and wrong level; and the other 27 cases including 3 cases after twice spinal surgeries were enrolled. Physical examinations were presented with atypical neuropathic pain and muscle wasting at single lower limbs which deteriorated with long-distance walking. Computed tomography or magnetic resonance imaging scan was studied including the neural structure and lower limb angiography. The report showed degenerative change without definite neural structural compression or vascular occlusion. There was also no significant finding of electrodiagnostic study. The rest radionuclide study of lower limb muscle and myocardium was performed in these 27 cases using Thallium-201. All patients presented 9-24 % (~14%) muscle perfusion difference compared with two legs. Most of peripheral arterial occlusion disease cases presented the symptoms with ischemic pain and vascular claudication. In the study, the patients presented not only ischemic limb but also the bizarre symptoms, such as low back pain with or without radiating root pain and sensory disturbance. Physicians should be aware of the possibility of lower limb pain caused by lower perfusion syndrome.

Adiponectin, a 244-amino acid peptide hormone, is associated with insulin resistance and carcinogenesis. Curcumin (diferuloylmethane) is the principal curcuminoid of the popular Indian spice, turmeric. Curcumin possesses antitumor effects, including the inhibition of neovascularization and regulation of cell cycle and apoptosis. However, the effects of adiponectin and curcumin on non-small cell lung cancer (NSCLC) remain unclear. In this study, we evaluated the expression of adiponectin in paired tumors and normal lung tissues from 77 patients with NSCLC using real-time polymerase chain reaction, western blotting, and immunohistochemistry. Epidemiological studies have shown a strong association between dermatitis and stroke. Systemic c... more Epidemiological studies have shown a strong association between dermatitis and stroke. Systemic corticosteroid, the mainstay treatment for dermatitis, could enhance the atherosclerotic process. Traditional Chinese Medicine (TCM) has been used for dermatitis to decrease the side effects of corticosteroid. However, the different stroke risk in dermatitis patients treated with systemic corticosteroid or TCM remains unclear. This study identified 235,220 dermatitis patients and same comorbidity matched subjects between 2000 and 2009 from database of NHRI in Taiwan. The two cohorts were followed until December 31, 2011. The primary outcome of interest was new diagnosis of stroke. The crude hazard ratio (HR) for future stroke among dermatitis patients treated with systemic corticosteroid was 1.40 (95% CI, 1.34-1.45; P &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.0001) and TCM was 1.09 (95% CI, 1.05-1.13; P &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.0001). The log-rank test showed a higher cumulative incidence of ischemic stroke in the patient treated with only systemic corticosteroid group than that treated with systemic corticosteroid and TCM, only TCM, and neither systemic corticosteroid nor TCM in the matched cohort during the follow-up period (P &amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;lt; 0.0001). We demonstrated that patients treated with systemic corticosteroid had an increased risk of stroke and that the risk probably decreased by TCM treatment.

Resveratrol has excellent antioxidant and anti-inflammatory effects, and this study investigated its role in H/R-induced type II pneumocyte dysfunction. H/R conditions increased expression of inflammatory cytokines including interleukin (IL)-1β (142.3±21.2%, P < 0.05) and IL-6 (301.9±35.1%, P < 0.01) in a type II alveolar epithelial cell line (A549) while the anti-inflammatory cytokine IL-10 (64.6±9.8%, P < 0.05) and surfactant proteins (SPs) decreased. However, resveratrol treatment effectively inhibited these effects. We reported a case of ureteral perforation caused by a secondary amebiasis in a patient with acquired immunodeficiency syndrome (AIDS). Following bowel perforation and immunocompromised conditions, secondary right ureteral perforation was not easily to be treated well. He was treated with percutaneous drainage initially. Definite and successful treatment by a Boari flap was delayed until his underling disease was under control.

Ureteral perforation is a rare complication of abdominal infection, especially in a patient with ... more Ureteral perforation is a rare complication of abdominal infection, especially in a patient with human immunodeficiency virus (HIV) infection. We reported a case of ureteral perforation caused by a secondary amebiasis in a patient with acquired immunodeficiency syndrome (AIDS). Following bowel perforation and immunocompromised conditions, secondary right ureteral perforation was not easily to be treated well. He was treated with percutaneous drainage initially. Definite and successful treatment by a Boari flap was delayed until his underling disease was under control.
