Background: The problem of correct inpatient scheduling is extremely significant for healthcare m... more Background: The problem of correct inpatient scheduling is extremely significant for healthcare management. Extended length of stay can have negative effects on the supply of healthcare treatments, reducing patient accessibility and creating missed opportunities to increase hospital revenues by means of other treatments and additional hospitalizations. Methods: Adopting available national reference values and focusing on a Department of Internal and Emergency Medicine located in the NorthWest of Italy, this work assesses prediction models of hospitalizations with length of stay longer than the selected benchmarks and thresholds. The prediction models investigated in this case study are based on Artificial Neural Networks and examine risk factors for prolonged hospitalizations in 2018. With respect current alternative approaches (e.g., logistic models), Artificial Neural Networks give the opportunity to identify whether the model will maximize specificity or sensitivity. Results: Our sample includes administrative data extracted from the hospital database, collecting information on more than 16,000 hospitalizations between January 2018 and December 2019. Considering the overall department in 2018, 40% of the hospitalizations lasted more than the national average, and almost 3.74% were outliers (i.e., they lasted more than the threshold). According to our results, the adoption of the prediction models in 2019 could reduce the average length of stay by up to 2 days, guaranteeing more than 2000 additional hospitalizations in a year. Conclusions: The proposed models might represent an effective tool for administrators and medical professionals to predict the outcome of hospital admission and design interventions to improve hospital efficiency and effectiveness.

Emergency departments are characterized by the need for quick diagnosis under pressure. To select... more Emergency departments are characterized by the need for quick diagnosis under pressure. To select the most appropriate treatment, a series of rules to support decision-making has been offered by scientific societies. The effectiveness of these rules affects the appropriateness of treatment and the hospitalization of patients. Analyzing a sample of 1844 patients and focusing on the decision to hospitalize a patient after a syncope event to prevent severe short-term outcomes, this work proposes a new algorithm based on neural networks. Artificial neural networks are a non-parametric technique with the well-known ability to generalize behaviors, and they can thus predict severe short-term outcomes with pre-selected levels of sensitivity and specificity. This innovative technique can outperform the traditional models, since it does not require a specific functional form, i.e., the data are not supposed to be distributed following a specific design. Based on our results, the innovative model can predict hospitalization with a sensitivity of 100% and a specificity of 79%, significantly increasing the appropriateness of medical treatment and, as a result, hospital efficiency. According to Garson's Indexes, the most significant variables are exertion, the absence of symptoms, and the patient's gender. On the contrary, cardio-vascular history, hypertension, and age have the lowest impact on the determination of the subject's health status. The main application of this new technology is the adoption of smart solutions (e.g., a mobile app) to customize the stratification of patients admitted to emergency departments (ED)s after a syncope event. Indeed, the adoption of these smart solutions gives the opportunity to customize risk stratification according to the specific clinical case (i.e., the patient's health status) and the physician's decision-making process (i.e., the desired levels of sensitivity and specificity). Moreover, a decision-making process based on these smart solutions might ensure a more effective use of available resources, improving the management of syncope patients and reducing the cost of inappropriate treatment and hospitalization.

This paper presents an analysis of relationships between collaborations and scientific outputs of... more This paper presents an analysis of relationships between collaborations and scientific outputs of the Italian National Research Council (CNR). In order to evaluate collaborations among CNR institutes and between CNR institutes and universities, social network metrics have been applied with the aim to measure relationships and to understand if to cooperate allows researchers to publish higher quality outputs, improving their labour productivity. Research institutes are considered as nodes of the internal collaboration network, following the main aim of recent reform. Collaborations are stimulated not only by governments with the aim to have knowledge spillovers but they can improve citations and also their reputation. This last is extremely relevant for winning competitions, calls or grants. In this paper authors used data of scientific publications related to all institutes of CNR for the 2007 year and they ask to the question if researchers that publish more and better are those that collaborate more.

This work investigates the Italian judicial system and discusses the designing of a policy reform... more This work investigates the Italian judicial system and discusses the designing of a policy reform to boost the efficiency of courts, considering the human resources available as well as the expenditure generated by the process to deliver justice. The authors present the benchmarks and shed light on how policy makers embarking on such a process of reform may be misled by inappropriate model definition. In detail, the authors propose a comparative analysis of technical efficiency (TE) scores computed using data envelopment analysis, adopting the same output (number of settled cases) and two alternative sets of inputs (judicial expenditure and human resources). According to the results, without considering the information extracted from the two complementary benchmark analyses, policy makers might be misled in the reform process. Regarding the elements that affect the performance of courts, it may be possible to improve the efficiency of judicial districts by working on judicial procedures. Hence, these are the domains where interventions by policy makers may prove successful. As for policy implications, the models and benchmarks presented here could be used to devise a reform of the judicial system aimed at enhancing its TE, while also keeping public expenditure under control.

The authors extract data on directors and top managers as well as rating classes and credit score indicators, and using a fixed-effects model, the authors analyze the relationship between credit risk mitigation and the inclusion of women among top managers, consistently with the rising empirical literature focused on risk perceptions. However, several aspects remain unclear, including if and how, corporate sustainability approaches are implemented and aligned within such networks, mostly created for competitiveness and resource efficiency purpose. With the aim of filling this gap in the literature over Small Business Social Responsibility, an empirical investigation is proposed by analyzing 389 Formalized Network Contracts and their original legal document, following Bocken et al. (2014) business model archetypes and NBS (2012) studies on sustainable innovation and, normative-making perspectives. The European Commission has recently highlighted the necessity to evaluate the performance of these sectors due to the decline of fish stocks. For this reason, strict rules oblige the Member States to manage fish stocks safeguarding natural resources and, at the same time, the productivity of the industry. Abstract This manuscript focuses on the Italian judicial system and on how to shape a policy refo... more Abstract This manuscript focuses on the Italian judicial system and on how to shape a policy reform aimed at increasing court efficiency, taking the budget externalities generated by this production process into account. On the one hand, the authors identify the benchmarks and main drivers of judicial inefficiency, while, on the other hand, they show how incorrect model definition may mislead policy makers tackling this reform process, based on an analysis of the Directional Distance Function with and without bad outputs. According to the results, incorrect model definition causes a type I error equal to 17.99% and a type II error equal to 25.18%. As for the determinants of court performance, the current staff structure and civil procedures are the main drivers of inefficiency and, consequently, the areas in which policy makers could successfully take action. In view of the EU’s recovery fund aimed at alleviating the economic impact of Covid-19, policy implications concern the opportunity to adopt these results and the collected benchmarks to drive one of the most important structural reforms in Italy, by leveraging the financial resources of the Next Generation EU package. Such a reform might be able to improve courts’ technical efficiency, while also maintaining the public budget under control.
This study proposes a cost-effectiveness analysis concerning the implementation of a tool to chec... more This study proposes a cost-effectiveness analysis concerning the implementation of a tool to check for ventilator-associated pneumonia (VAP) in Intensive Care. VAP is a sub-type of hospital-acquired pneumonia which occurs in people receiving mechanical ventilation. An empirical analysis is performed to estimate the effectiveness of a specific procedure to prevent VAP (i.e. Ventilator Bundle), as well as to evaluate cost savings related to not developing the infection. The relevance of the results thus obtained is not only clinical but also managerial and economic, since a reduction in costs improves the economic performance of hospitals and a lower incidence of VAP improves their reputation on the national market of healthcare services.

In particular, this work aims to address the impact of COVID-19 on the Italian manufacturing industry, and evaluation of the adopted recovery measures. Considering the current situation, with the war in Ukraine and the related gas crisis across the European Union, such investigation on policy relief is even more relevant, contributing to the current debate. Reports of the European Commission registered that women are disadvantaged subjects on the labour market but, at the same time, they are more ambitious and are at their best on the educational side. This paper aims at analysing data of Piedmont Region on vocational training policies, focusing on the role of women into the labour market. Data refer to subjects that accomplished their training course during 2011. Analyses have been performed on interviews, in order to evaluate the effects of training on medium-term employment outcomes of trainees. A control sample has been selected with the aim to evaluate the effect of training, with a special focus on women. The relevance of the results thus obtained is not only clinical but also managerial and economic, since a reduction in costs improves the economic performance of hospitals and a lower incidence of VAP improves their reputation on the national market of healthcare services.

In particular, this work aims to address the impact of COVID-19 on the Italian manufacturing industry, and evaluation of the adopted recovery measures. Considering the current situation, with the war in Ukraine and the related gas crisis across the European Union, such investigation on policy relief is even more relevant, contributing to the current debate. This manuscript focuses on the Italian judicial system and on how to shape a policy reform aimed ... more This manuscript focuses on the Italian judicial system and on how to shape a policy reform aimed at increasing court efficiency, taking the financial negative externalities generated by this production process into account. On the one hand, the authors identify the benchmarks and main drivers of judicial inefficiency, while, on the other hand, they show how incorrect model definition may mislead policy makers tackling this reform process, based on an analysis of the Directional Distance Function with and without bad outputs. According to the results, incorrect model definition causes a type I error equal to 10.37% and a type II error equal to 3.66%. Policy implications concern the opportunity to adopt the proposed model and the collected benchmarks to reform the judicial system, improving its technical efficiency and maintaining the public budget under control.

Reports of the European Commission registered that women are disadvantaged subjects on the labour market but, at the same time, they are more ambitious and are at their best on the educational side. This paper aims at analysing data of Piedmont Region on vocational training policies, focusing on the role of women into the labour market. Data refer to subjects that accomplished their training course during 2011. Analyses have been performed on interviews, in order to evaluate the effects of training on medium-term employment outcomes of trainees. A control sample has been selected with the aim to evaluate the effect of training, with a special focus on women.
