The aim of this study was to determine whether low values of VLPP (Valsava Leak Point Pressure) a... more The aim of this study was to determine whether low values of VLPP (Valsava Leak Point Pressure) and low values of MUCP (Maximal Urethral Closure Pressure) before operation can predict quality of life (QOL) after anti-incontinence surgery. 72 stress incontinent women were included in this study. All women underwent anti-incontinent surgery. We compared the quality of life using I-QOL (Incontinence Quality of Life) assessment with parameters of urodynamic measurement VLPP and MUCP The women filled in an I-QOL questionnaire before surgery and three month afterwards. As in other studies, low preoperative value of the VLPP < or =60 cmH2O was not related to a statistically lower quality of life (average I-QOL for VLPP < or =60 cmH2O was 38.4 and 48.9 for VLPP > 60 cmH2O). We did not find statistically significant lower quality of life in women with MUCP < or =20 cmH2O (average I-QOL 38.2, and for MUCP > 20 39.7). Prospective observational study. Department of Gynecology and Obstetrics, First Faculty of Medicine, Charles University and the General University Hospital, Prague, EuroMISE centre of the Academy of Science, Prague. 111 women with urodynamically proven stress incontinence who had undergone a TVT operation were included in the study; patients suffering with the mixed type of incontinence were included as well while a major prolapse of anterior vaginal wall (POP-Q II and more) was applied as the exclusion criterion. An overall pre-operation examination was carried out, including urodynamic and ultrasound examination, while the patients also completed an i-QOL quality of life questionnaire. We present the technique employed in the operation, the examination procedure including US examination before and after the operation, at rest and during Valsalva manoeuvre, and the subjective and objective outcome in our group. This study is intended to determine the curative effect and the complications associated with TVT S procedure. The clinical study included 15 women with previously untreated stress urinary incontinence. Their mean age was 53.1, mean body mass index (BMI) 27.73, and mean parity 1.86. This syndrome requires longterm therapy but the persistence of this treatment in comparison with other chronic diseases as diabetes, hypertension, osteoporosis etc. is low. Accorging many studies the persistence variability is high and could be influenced by regional circumstanses. We desided to analyze real situation in parasympatolytics persitance in two years period in Czech Republic based on registry of health insurance company. We analyzed data between 2009 and 2010 of all clients of health insurance company where they fullfil their prescribtion. We analyzed the persistence in one year period in different periods of the year and within one year. In comparison of the same period between those two year there was a trend of increase of prescription but it was not significant: 1306(SD 56.3) vs 1410 (SD 68.1) - p = 0.056. Levator ani avulsion injury is one of the main factors associated with vaginal prolapse of all co... more Levator ani avulsion injury is one of the main factors associated with vaginal prolapse of all compartments. Levator avulsion is the main link between vaginal delivery and subsequent vaginal prolapse - cause and sequel. The risk for prolapse is doubled in women with levator injury (RR = 1,9). The risk for prolapse is mainly expressed for anterior and central compartment. In retrospective analysis of women after vaginal repair without use of implants the recurrence rate was 29%. In subanalysis of only of women with recurrence the avulsion injury was detected in 85%. The risk of recurrence of prolapse after native tissue repair is four times higher if the avulsion is present to those where not. Levator avulsion injury is main etiological and risk factor associated with uterine and vaginal prolapse as the sequel of delivery trauma. This injury also increases significantly the risk of prolapse recurrence after native tissue vaginal repair.

Presentation of the standards in use of ultrasound imaging in urogynecology and presentation of our practical skills in ultrasound diagnoses in the management of lower urinary tract dysfunctions and in monitoring of the effect of surgical treatment and its complications. Ultrasound is an excellent method for the investigation of the lower urinary tract, not only as a diagnostic tool but also for the monitoring of the effect of surgical procedures.

The lifetime risk of undergoing a single operation for prolapse or incontinence by age 80 is 11.1%. Recurrence rates for classical prolapse surgery are as high as 30%. For this reason various graft materials have been proposed to improve the long-term surgical outcomes. The aim of our study was to investigate the safety and efficacy of posterior colporrhaphy incorporating Vypro II (polyglactin 910-polypropylene) mesh in the treatment of posterior vaginal wall prolapse. To evaluate and quantify early and late changes in mesh length after anterior vaginal repair with... more To evaluate and quantify early and late changes in mesh length after anterior vaginal repair with implants with ultrasound. Prospective interventional study. Ultrasound assessment of 35 patient randomized in two groups both with Gynemesh insertion. In one group with Gynemesh Prolift anterior kit and second group with individualized size of the Gynemesh. Ultrasound measurement of the mesh in sagittal plane was performed postoperative day 4 and after 3-4 month. Measured values were put in proportions, together with the original size of the mesh. Results are expressed as a percentage of shortening. In comparison of measurements from the late ultrasound scans with original size the shortening of the Prolift was 45% vs. 25% in Mesh group. When we compared the late and early ultrasound scans, there was no difference in the shrinking of the mesh in both group 16% vs 20%. After the TVT procedure 90 women were evaluated. Prospective study. The Department of Obstetric and Gynaecology, the 1st Medical Faculty of Charles University and General Hospital, Prague. 73 women were initially involved in the study. After one year of control we could only evaluate 51 of them (69.9%). Their average age was 56, BMI over 25 had 70% of them and 47% of them underwent a gynaecologic surgery in history. With these 51 women we managed to get complete questionnaire before the surgery on GSI and during the following checking after 3 months and 1 year. All women underwent anti-incontinent surgery. We compared the quality of life using I-QOL (Incontinence Quality of Life) assessment with parameters of urodynamic measurement VLPP and MUCP The women filled in an I-QOL questionnaire before surgery and three month afterwards. This syndrome requires longterm therapy but the persistence of this treatment in comparison with other chronic diseases as diabetes, hypertension, osteoporosis etc. is low. Accorging many studies the persistence variability is high and could be influenced by regional circumstanses. We desided to analyze real situation in parasympatolytics persitance in two years period in Czech Republic based on registry of health insurance company. We analyzed data between 2009 and 2010 of all clients of health insurance company where they fullfil their prescribtion. We analyzed the persistence in one year period in different periods of the year and within one year. Levator ani avulsion injury is one of the main factors associated with vaginal prolapse of all co... more Levator ani avulsion injury is one of the main factors associated with vaginal prolapse of all compartments. Levator avulsion is the main link between vaginal delivery and subsequent vaginal prolapse - cause and sequel. The risk for prolapse is doubled in women with levator injury (RR = 1,9). The risk for prolapse is mainly expressed for anterior and central compartment. In retrospective analysis of women after vaginal repair without use of implants the recurrence rate was 29%. In subanalysis of only of women with recurrence the avulsion injury was detected in 85%. The risk of recurrence of prolapse after native tissue repair is four times higher if the avulsion is present to those where not. Levator avulsion injury is main etiological and risk factor associated with uterine and vaginal prolapse as the sequel of delivery trauma. This injury also increases significantly the risk of prolapse recurrence after native tissue vaginal repair.

Prospective interventional study. Ultrasound assessment of 35 patient randomized in two groups both with Gynemesh insertion. In one group with Gynemesh Prolift anterior kit and second group with individualized size of the Gynemesh. Ultrasound measurement of the mesh in sagittal plane was performed postoperative day 4 and after 3-4 month. Measured values were put in proportions, together with the original size of the mesh. Results are expressed as a percentage of shortening. In comparison of measurements from the late ultrasound scans with original size the shortening of the Prolift was 45% vs. 25% in Mesh group. Prospective pilot study. Obstet. Gynecol Department, General Teaching Hospital, 1st Medical Faculty, Charles University, Prague; EuroMISE Centre of the Charles University and Academy of Sciences, Prague, Czech Republic. 101 women with proven stressed urinary incontinence were included in the study. After the TVT procedure 90 women were evaluated. As a part of the complex urogynecological investigation before surgery the transperineal ultrasound scan was performed in supine position, urinary bladder was filled to 300 ml. The aim of the study is to show the changes in quality of life before and after surgery for genui... more The aim of the study is to show the changes in quality of life before and after surgery for genuine stress urinary incontinence by means of the subjective assessment. Prospective study. The Department of Obstetric and Gynaecology, the 1st Medical Faculty of Charles University and General Hospital, Prague. 73 women were initially involved in the study. After one year of control we could only evaluate 51 of them (69.9%). Their average age was 56, BMI over 25 had 70% of them and 47% of them underwent a gynaecologic surgery in history. With these 51 women we managed to get complete questionnaire before the surgery on GSI and during the following checking after 3 months and 1 year. The most frequent type of surgery was colposuspension (76%). The quality of life in connection with the incontinence is measured by the help of a specific standardized questionnaire IQoL from Donald L. Patrick.
