Many studies examining Latino health-seeking behavior have focused on comparing Latinos with othe... more Many studies examining Latino health-seeking behavior have focused on comparing Latinos with other ethnic groups, pri-marily with Whites and African Americans. However, without the benefit of intragroup or intracultural comparisons, such studies fail to identify the subtle variation in health-seeking strategies and the range of needs within the Latino ethnic group, and thus are compromised in their capacity to guide and improve practice and policy. This article reviews the lit-erature regarding health-seeking behavior and Latino men. Important factors identified include gender, occupation, and responsiveness of the health care system along with charac-teristics of the individual.

Culturally and linguistically appropriate services should be integrated throughout an organization and in collaboration with the communities served. Cultural differences will exist but providers and patients should create an environment based on trust and honesty to dialogue about differences and share similarities.

Propelled by a national concern with social justice and health disparities, the notion of cultura... more Propelled by a national concern with social justice and health disparities, the notion of cultural competency is being incorporated into both government regulation and professional standards. Although most of the standards that are being developed nationally apply at the institutional level, it is in the clinical setting where the expectation of cultural competency is the most demanding. The recommendations for clinicians to become culturally competent generally fall into 2 major categories. The first focuses on the content and structure of the clinical encounter between provider and patient. The second category charges providers with becoming knowledgeable about the cultures of their constituent patients and learning their lifestyles, health beliefs, and behaviors. Although individuals may belong to the same cultural group, the assumption that they are, in fact, the same, is an ecological fallacy. The health care system has nested the accountability for cultural competence with the clinician who provides direct services to individuals, where the application of cultural information is likely to be least useful. We contend that cultural competence is really nursing competence. "If ever you hear anyone. .. speak of the East,. .. hold your judgment. If you are told 'they are all this' or 'they do this' or 'their opinions are these,' withhold your judgment until all the facts are upon you. Because that land they call 'India' goes by a thousand names and is populated by millions, and if you think you have found 2 men the same among that multitude, then you are mistaken. One would be hard pressed to deny that the flurry of activity around cultural competency is a very good thing. Recognition that the ethnic composition of the population of the United States is changing dramatically and challenging a health care system that is narrowly based in a white, male, middle-class, biomedical model is long overdue. As the United States continues to evolve as a multi-ethnic, culturally diverse society, a standard of cultural competency in all human services is wholesome, desirable, and consistent with the democratic principles on which this nation was founded. In this study, an emergency care unit (ECU) worked with consultants to adapt the hospital patient classification system to provide more accurate ECU service charges for nursing care.

Several states in the United States have been experiencing an influx of migrants from an area of ... more Several states in the United States have been experiencing an influx of migrants from an area of the world that most people have only heard of when learning about the atomic bomb and World War II. This area is the former U.S. Trust Territory of Pacific Islands now called the Freely Associated States. At the end of World War II, the United States took possession of many of these islands and in 1948, the United States formally took over administration of the Marshalls, the Carolines, Palau, and the Northern Marianas islands. Collectively this area is known as Micronesia. Micronesians come from areas that have high prevalence of several communicable diseases and there is growing concern that Micronesian immigrants may enable the spread of infectious disease to the United States from Asia. Data concerning Hansen's disease and tuberculosis support this claim. According to data from the Hawai'i State Department of Health, a 5-year trend examining new cases of tuberculosis in Hawai'i identified that 65 out of 77 new cases came from the Freely Associated States of Micronesia. Presented is an overview of the health concerns and health status of the people from the Federated States of Micronesia.
