Summary

Anorexia, malnutrition followed by cachexia is observed in up to 80% of cancer patients with advanced stages of their disease, particularly in head and neck cancer, gastrointestinal cancer and lung cancer. Malnutrition is associated with an unfavourable prognosis and has been demonstrated to be associated with an increased morbidity and an increased readmission rate. Cachexia itself can be the primary cause of death in cancer patients. Early intervention and nutritional support may be helpful in preventing anorexia and further weight loss. Successful approaches in treating anorexia have been undertaken with corticosteroids and gestagenes.

Zusammenfassung

Mangel- und Unterernährung betreffen bis zu 80 % der Patienten in fortgeschrittenen Stadien ihrer Krebserkrankung. Besonders häufig sind Patienten mit Kopf-Hals-Tumoren, gastrointestinalen Tumoren und Bronchial-Karzinomen betroffen. Ein schlechter Ernährungszustand ist mit einer ungünstigen Prognose assoziiert und kann eine erhöhte Morbidität oder sogar die primäre Todesursache darstellen. Ferner kann eine Mangelernährung zusätzliche Krankenhausaufenthalte bedingen. Frühzeitige ernährungstherapeutische Beratungen und gezielte Interventionen können den Ernährungszustand auch bei fortgeschrittener Krebserkrankung und palliativmedizinischen Patienten lange Zeit erhalten und damit Folgen einer Mangelernährung verhindern. Medikamentös kann die Anorexie mit Steroiden und Gestagenen behandelt werden, die beide eine nachgewiesene Appetitsteigerung bewirken.
