On the mutation rate of neurofibromatosis (original) (raw)
Summary
A genetic study of 124 cases of neurofibromatosis was performed. The contingent of probands was mainly represented by a Russian population, most of the individuals being born in the European part of the RSFSR.
Both parents of the probands were examined in only 58 cases, the proportion of sporadic cases in this group being 0.79, as compared to 0.77 for the whole group under study.
The existing data evaluated by a direct method are not yet sufficient for a decisive estimation of the penetrance, which, however, cannot be under 80%. Segregation analysis of descendants from particular marriages showed a good correspondance to the hypothesis of Mendelian dominance (32 affected children out of 65). These results analyzed together with those obtained by other authors permit an inference on the full penetrance of neurofibromatosis. The genetic interpretation of sporadic cases as a result of new mutations is presented.
The prevalence of neurofibromatosis among the 16-year-old youths was evaluated as 12.8 · 10-5. This value is suggested to be an estimation of the incidence of the condition in the general population, the mutation rate evaluated by a direct method being equal to 4.4 · 10-5÷4.9 · 10-5.
The increased birth order of probands in sporadic cases (against the theoretical expectation) as well as increased paternal age (as compared with controls) were found to be statistically significant (_P_=0.004 and_P_=0.03, respectively) while the difference in maternal ages was statistically insignificant (_P_=0.08). No statistical relationship between sporadic cases and occupational exposure of parents to deleterious chemical and physical factors was found.

Es wurde eine genetische Forschung an 124 Fällen von Neurofibromatose durchgeführt. Das Kontingent der Probanden entstammte hauptsächlich der russischen Bevölkerung, wobei die meisten auf den europäischen Teil der Russischen Sozialistischen Föderativen Sowjetrepublik (RSFSR) entfielen.
Beide Eltern der Probanden wurden nur in 58 der Fälle untersucht. Dabei betrug die für diese Gruppe ausgerechnete Häufigkeit der sporadischen Fälle 0.79 während die betreffende Ziffer für das ganze Material 0.77 war.
Zwar sind die zur Verfügung stehenden Angaben betreffs der direkten Auswertung von Penetranz unzureichend für zuverlässige Abschätzungen, jedoch kann die Penetranz nicht geringer als 80% sein. Die Segregationsanalyse der Nachkommenschaft von bestimmten Ehetypen zeigte eine recht gute Übereinstimmung mit der Hypothese der Mendelschen Dominanz: von 65 Nachkommen waren 32 mit Neurofibromatose behaftet. Diese Ergebnisse, ebenso wie diejenigen von anderen Verfassern, lassen die Schlußfolgerung zu, daß sporadische Fälle Folgen einer neuentstandenen Mutation sind.
Das Vorkommen der Neurofibromatose unter 16jährigen Jugendlichen wurde auf 12,8·10-5 geschätzt. Vorausgesetzt, daß dies der Häufigkeit der Krankheit in der allgemeinen Bevölkerung entspricht, wurde die mit direkter Methode ausgewertete Mutationsrate auf 4,4·10-5 ÷4,9·10-5 geschätzt.
Es wurde festgestellt, daß sowohl die erhöhte Geburtenordnung der Probanden von sporadischen Fällen (gegenüber der theoretischen Erwartung) als auch das erhöhte väterliche Alter (verglichen mit der Kontrolle) statistisch gesichert sind (_P_=0,004 bzw._P_=0,03), während keine statistischen Unterschiede für das mütterliche Alter gefunden wurden (_P_=0,08). Es konnten keine sicheren Befunde über eine statistische Beziehung zwischen sporadischen Fällen und der Exposition der Eltern zu chemischen und physischen Berufsschäden erhoben werden.
