States must have in effect policies and procedures to ensure seamless transitions for children and their families as they leave Part C and other early childhood programs, so they have timely access to appropriate services. Families need to move smoothly from one program or system to another, or among services within a program or system. State and local structures, policies, interagency agreements, personnel development processes, and other mechanisms must be in place to support the transition process.

• April 2024: The Center for IDEA Early Childhood Data Systems (DaSy) produced this Automated Data Sharing or Linking Use Case: IDEA Part C to Part B 619 Transition Notification resource to support Part C and Part B, 619 program staff in sharing or linking their IDEA Part C to Part B, 619 transition notification data.
• November 28, 2023: OSEP has released the 2023 Early Childhood Transition Questions and Answers document. This document incorporates longstanding policy and provides further clarification on early childhood transition requirements as children transition from the IDEA Part C early intervention program to the IDEA Part B preschool program. This resource has been added to the collection of OSEP Resources on Transition.
