This study aimed to investigate the impacts of fluctuations in agricultural production, consumpti... more This study aimed to investigate the impacts of fluctuations in agricultural production, consumption and marketing bottlenecks on main food prices in Gadarif State, east of Sudan in the period of 1996 to 2011. On the supply side, an adjusted Nerlovian model of supply response was used to calculate elasticities of prices of selected agricultural products. On the consumption side, the distribution of the agricultural supply among local consumption and external export was analyzed. The price margin of these products was calculated to explain how this margin is shared by the different market players. With reference to results of this study, supply response analysis of sorghum and sesame showed that production did not respond to finance or price factors, indicated by the low elasticities. Analysis of marketing channels of the above crops showed that the various fees and taxes imposed on different crops have weakened the competitiveness of export of these crops and hindered farmers from gaining reasonable revenues. Thus, prices of these crops in some seasons exceeded the world prices. Results of the price margin showed that the real producers receive the minimum price margin as compared to the other market players.

Based on state and local data of the periods (1998-2010) and (2008-2010) respectively, this study... more Based on state and local data of the periods (1998-2010) and (2008-2010) respectively, this study aimed at evaluating the fiscal decentralization experience of Gadarif state of eastern Sudan in the context of different aspects of fiscal decentralization, namely expenditure assignment, revenue assignment and intergovernmental transfers. The results on the state level reveal that the experience came out with a poor fiscal situations indicated by higher dependence on the federal transfers and higher vertical imbalance. The cross section and panel data analysis of the local level of the state indicates the overwhelming dependence of the localities on the state transfers and the failure of most of the localities to perform their responsibilities of delivering the basic services to their constituents.

This committee opinion paper summarizes available evidence about recurrent pelvic organ prolapse ... more This committee opinion paper summarizes available evidence about recurrent pelvic organ prolapse (POP) to provide guidance on management. A working subcommittee from the International Urogynecological Association (IUGA) Research and Development Committee was formed. The literature regarding recurrent POP was reviewed and summarized by individual members of the subcommittee. Recommendations were graded according to the 2009 Oxford Levels of Evidence. The summary was reviewed by the Committee. There is no agreed definition for recurrent POP and evidence in relation to its evaluation and management is limited. The assessment of recurrent POP should entail looking for possible reason(s) for failure, including persistent and/or new risk factors, detection of all pelvic floor defects and checking for complications of previous surgery. The management requires individual evaluation of the risks and benefits of different options and appropriate patient counseling. There is an urgent need for an agreed definition and further research into all aspects of recurrent POP.

Pelvic floor disorders (PFD), including urinary incontinence, anal incontinence, and pelvic organ... more Pelvic floor disorders (PFD), including urinary incontinence, anal incontinence, and pelvic organ prolapse, are common and have a negative effect on the quality of life of women. Treatment is associated with morbidity and may not be totally satisfactory. Prevention of PFDs, when possible, should be a primary goal. The purpose of this paper is to summarise the current literature and give an evidence-based review of the prevention of PFDs METHODS: A working subcommittee from the International Urogynecological Association (IUGA) Research and Development (R&D) Committee was formed. An initial document addressing the prevention of PFDs was drafted, based on a review of the English-language literature. After evaluation by the entire IUGA R&D Committee, revisions were made. The final document represents the IUGA R&D Committee Opinion on the prevention of PFDs. This R&D Committee Opinion reviews the literature on the prevention of PFDs and summarises the findings with evidence-based recommendations. Pelvic floor disorders have a long latency, and may go through periods of remission, thus making causality difficult to confirm. Nevertheless, prevention strategies targeting modifiable risk factors should be incorporated into clinical practice before the absence of symptomatology.

The aim of this study was to establish the number and trend of surgical procedures and pessary ma... more The aim of this study was to establish the number and trend of surgical procedures and pessary management of pelvic organ prolapse in England, using Hospital Episode Statistics (HES) data. An online search ( www.hesonline.nhs.uk ) was carried out. The number of various surgical procedures, as well as pessary insertions and removals for pelvic organ prolapse was obtained. Data were available for the 4-year period March 2002 to June 2005. Over 20,000 procedures performed and 600 pessaries inserted annually. The total number of patients having treatment for pelvic organ prolapse as well as the number of patients having surgery and pessary in English hospitals increased by < 10% over the previous year in 2 of the 4 years included and decreased by < 1% in one. Anterior vaginal wall repair was the commonest performed procedure for pelvic organ prolapse and sacrocolpopexy appears to be the most frequent procedure for post-hysterectomy vaginal vault prolapse. The aim of this study was to compare foundation doctors in obstetrics and gynaecology to the form... more The aim of this study was to compare foundation doctors in obstetrics and gynaecology to the former grade of senior house officer, as perceived by their consultant and middle grade colleagues. A self-construct questionnaire was sent to College District Tutors in obstetrics and gynaecology in all hospitals in the United Kingdom, for circulation to all consultants and middle grade doctors in obstetrics and gynaecology in their departments. A total of 155 questionnaires were returned, 143 completed and 12 indicating not having foundation doctors. Whilst about 50 % of respondents ranked the competencies of foundation year doctors as similar to that of the former senior house officer grade, the ratings were down in areas specific to obstetrics and gynaecology, such as seeing new patients in antenatal clinic, and up in general terms such as communication. General comments highlighted differences in ability and role and suggested longer attachments and less frequent changes in training structure. The study highlighted the need to focus on teaching generic skills in shorter durations and reflection on whole-patient management, which are relevant to all specialities. It underlined the impact of more frequent change of doctors on service delivery and recommended having a further look at foundation training programme before making any future changes.

Since women with MUI are a heterogeneous group, the treatment of MUI requires an individual assessment of the symptom components: stress urinary incontinence, urinary urgency, urgency urinary incontinence, urinary frequency, and nocturia. The purpose of this paper is to summarize the current literature and give an evidence-based review of the assessment and treatment of MUI. A working subcommittee from the International Urogynecological Association (IUGA) Research and Development (R&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;D) Committee was formed. The diagnosis of MUI encompasses a very heterogeneous group of women. The evaluation and treatment requires an individualized approach. The use of validated questionnaires is recommended to assess urinary incontinence symptoms and effect on quality of life. Conservative therapy is suggested as a first-line approach; if surgery is contemplated, urodynamic investigation is recommended. Women undergoing surgical treatment for MUI need to be counselled about the possibility of persistence of urinary urgency, frequency and urge incontinence even if stress urinary incontinence is cured.

The purpose of this paper is to summarize the current literature and give an evidence-based review of the assessment and treatment of MUI. A working subcommittee from the International Urogynecological Association (IUGA) Research and Development (R&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;D) Committee was formed. An initial document addressing the diagnosis and management of MUI was drafted based on a literature review. After evaluation by the entire IUGA R&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;D Committee, revisions were made, and the final document represents the IUGA R&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;D Committee Opinion on MUI. This R&amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;amp;D Committee Opinion reviews the literature on MUI and summarizes the assessment and treatment with evidence-based recommendations. The diagnosis of MUI encompasses a very heterogeneous group of women. The evaluation and treatment requires an individualized approach. The use of validated questionnaires is recommended to assess urinary incontinence symptoms and effect on quality of life. Conservative therapy is suggested as a first-line approach; if surgery is contemplated, urodynamic investigation is recommended. Women undergoing surgical treatment for MUI need to be counselled about the possibility of persistence of urinary urgency, frequency and urge incontinence even if stress urinary incontinence is cured.
