Özet: A¤›r konjenital iflitme kayb›nda glutatyon peroksidaz ve katalaz enzim gen polymorfizmleri ... more Özet: A¤›r konjenital iflitme kayb›nda glutatyon peroksidaz ve katalaz enzim gen polymorfizmleri Amaç: Bu çal›flman›n amac› konjenital a¤›r iflitme kayb› olan koklear implantl› hastalarda glutatyon peroksidaz 1 geni (GPX1) üzerindeki kodon 200 polimorfizmini ve katalaz geni (CAT) üzerindeki A/T de¤iflimlerini araflt›rmak idi. Yöntem: Do¤ufltan iflitme kayb› olan 65 koklear implant hastas› ve yafl ve cinsiyet eflleflmeli 100 gönüllü 2011 ila 2013 y›llar› aras›nda de¤erlendirilmifltir. Tuzla çökeltme yöntemi kullan›larak periferik kan numunelerinden genomik DNA ekstrakte edilmifltir. Polimeraz zincir reaksiyonu ve restriksiyon fragment uzunluk polimorfizmleri kullan›larak CAT geninin promoter bölgesindeki T/A polimorfizmi (rs7943316) ve GPX1 gen kodon 200 prolinin lösine çevrimi (rs1050450) belirlenmifltir. Bulgular: GPX1 üstündeki kodon 2000 içindeki CC ve CT genotipleri aras›nda istatistiksel aç›dan anlaml› herhangi bir istatistiksel aç›dan anlaml› farkl›l›k bulunmam›flt›r (CC, p=0.10; CT, p=0.48). Ancak TT genotipi aç›s›ndan istatistiksel aç›dan anlaml› bir farkl›l›k vard› (p=0.04). CAT promoter bölgesi aç›s›ndan hastalarla kontrol grupla-r› aras›nda istatistiksel aç›dan anlaml› bir farkl›l›k yoktu (AA, p=0.41; TA, p=0.16; TT, p=0.08). Sonuç: Sonuç olarak GPX1 kodon 200 üzerindeki TT genotipi konjenital a¤›r sensorinöral iflitme kayb›yla iliflkili olabilir.

Congenital choanal atresia (CCA) is a very rare abnormality of the nose, but in the case of bilat... more Congenital choanal atresia (CCA) is a very rare abnormality of the nose, but in the case of bilateral presence, it becomes a life-threatening malformation. Various surgical treatment options, such as transpalatal, transseptal, and open rhinoplasty techniques, as well as the transnasal approach, have been defined for the repair of CCA. In this study, the authors intended to evaluate the outcomes of transnasal endoscopic surgery for CCA, and stent implementation's impact on surgical success. Patients who were admitted to the Otorhinolaryngology Department of Gaziantep University and patients who had not undergone CCA surgery before were included in the study. Patients who underwent transnasal endoscopic choanal atresia surgery (TECAS) were advised to have regular nasal endoscopic examinations performed at check-ups; after a 6-month follow-up period, surgical results were evaluated concerning whether stenosis had occurred or not. Of the 48 patients who underwent TECAS after a minimum 6-month follow-up period, 34 of patients revealed no stenosis, so the overall surgical success rate was 70.8%. Fourteen (29.2%) patients who underwent TECAS developed stenosis and required revision surgery. Transnasal endoscopic choanal atresia surgery is the most preferred approach for CCA repair and has many advantages, such as excellent vision, shorter operative time, minimal bleeding, and minimum complication. Despite advances in endovision systems and surgical instruments, stenosis is the most challenging problem after TECAS, so new treatment strategies should be developed to prevent stenosis.

[1] Among the etiological factors affecting prelingual deaf patients, 60% are hereditary, while 40% are environmental or iatrogenic. [1,2] The etiology of congenital sensorineural hearing loss remains largely unknown. One hypothesis is that immuno-logical factors are responsible for disease development. [3,4] Moreover, single nucleotide polymorphisms (SNPs) at position-308 of the TNF-α gene, at position-174 of the IL-6 gene, and at position +874 of the interferon-gamma (IFN-γ) gene were previously shown to be associated with inflammatory and autoimmune disorders. [5,6]

We evaluated outcomes of otologic surgeries with a special interest in learning curves of residen... more We evaluated outcomes of otologic surgeries with a special interest in learning curves of residents in a tertiary referral setting. A retrospective review of the outcomes of 811 otologic operations was performed. The outcomes of faculty staff (group 1), senior residents (fifth year; group 2), and residents (fourth year) under supervision of the faculty staff (group 3) were assessed and compared. The faculty staff were available for consultation if needed and were ready to intervene at any stage for group 2. There were 397 female patients (48.95%) and 414 male patients (51.04%) from 17 to 71 years of age. There was no statistically significant difference between the groups regarding graft take rate or hearing outcome (p > .05). The otologic training of residents appears to be successful in Turkey, as the surgical outcomes of residents were comparable to those of faculty staff.

More than half of the head and neck GCTs originate from the tongue. It is most frequently seen in the fourth to sixth decades of life. We presented an interesting case of a giant GCT in a 2.5-year-old patient. It was located under the anterior tongue, filled the floor of the mouth and was resected by the transoral route.

We evaluated the results of endoscopic repair for choanal atresia. Ten patients (2 males, 8 femal... more We evaluated the results of endoscopic repair for choanal atresia. Ten patients (2 males, 8 females) underwent transnasal endoscopic repair for choanal atresia. Involvement was unilateral in five cases and bilateral in five cases. Unilateral patients were treated at a mean age of 22.6 years (range 1 to 44 years), while bilateral cases were treated within the first six days of life. Two patients with bilateral involvement had associated polydactyl and tracheoesophageal fistula, respectively. The mean follow-up period was 26 months (range 8 to 56 months). No postoperative complications such as significant hemorrhage, injury to the skull base, cerebrospinal fluid fistula, septal perforation, or granulation tissue formation were encountered. All the patients had choanal patency at the end of six months. Endoscopic repair of choanal atresia is effective in both unilateral and bilateral cases.

Tuberculous involvement of the neck is also possible. Patients mostly present with masses or draining fistulas in the neck, which are mostly located in the posterior cervical, submandibular, and supraclavicular regions. We report on a soldier who presented with a tuberculous deep neck infection, followed by miliary tuberculosis. The clinical presentation and diagnostic dilemmas are discussed.

The magnetic resonance imaging revealed that the dimensions of the mass were 29×19×17 mm. The most probable diagnosis with the magnetic resonance imaging findings was dermoid cyst. The mass was a cyst and marsupialized by functional endonasal endoscopic sinus surgery without any complication. Intraoperative findings of the mass were thought to be a hydatid cyst, but the diagnostic tests were negative, and the pathologic result was consistent with mucocele.

Material and Methods: Nucleus ® 24 Contour Advance™ electrodes were inserted into a human scala tympani model filled with different intermediates: glycerin, sodium hyaluronate (healon), water or soap solution. Additionally, electrodes were coated with BDNF and GFP producing fibroblasts and were examined with fluorescence microscopy. Force measurements were performed during the insertions by a force measurement system (Instron 5542) equipped with a 10 N load cell. Results: The lowest forces were documented with the soap solution followed by healon and glycerin, and highest values with water. Coating of the electrodes with cells resulted in decreased forces, too. Fibroblasts were removed from the lateral electrode surface but remained intact on the surface facing the modiolus. Conclusions: Modification of the electrode surface or the environment can result in a significant decrease of the insertion forces.

The present study was designed to demonstrate the regional distribution prevalence and management... more The present study was designed to demonstrate the regional distribution prevalence and management differences of certain pediatric neck masses in this region of the country. Fifty-five patients with pediatric neck masses, who were diagnosed and treated surgically were retrospectively analyzed. Analysis of 55 patients showed that congenital neck masses were the most common pathology in pediatric neck masses (47%), followed by infectious and tuberculous lymphadenopathies (24%). It can be concluded from the study that besides the age group and the characteristics of the neck mass, life standards of the region is also an important factor to be considered.

The aim of this study was to detect cysteinyl leukotriene (Cys-LT) receptors in aspirin-sensitive... more The aim of this study was to detect cysteinyl leukotriene (Cys-LT) receptors in aspirin-sensitive nasal polyposis patients, and compare them with nasal polyposis and chronic rhinosinusitis patients without aspirin sensitivity. The formalin-fixed, paraffin-embedded tissue blocks of a total of 52 patients undergoing endoscopic sinus surgery were reviewed by an experienced head and neck pathologist. Of these 52 cases, there were 22 nasal polyposis cases with aspirin sensitivity, 15 nasal polyposis cases without aspirin sensitivity and 15 chronic rhinosinusitis cases without aspirin sensitivity. Immunohistochemical analyses for Cys-LT(1) receptors were performed with polyclonal rabbit antibody. Eosinophil receptor intensity levels were 27.68, 21.67, and 14.40, respectively. This was statistically insignificant (p <0.089). Mast cell receptor intensity levels were 8.41, 5.27, and 4.27, respectively. In the aspirin-sensitive group, mast cell receptor intensity was significantly higher when compared to the nasal polyposis and chronic rhinosinusitis group (p < 0.0001 and p < 0.0001, respectively). The difference in Cys-LT(1) receptor immunoreactivity in both eosinophils and mast cells among the 3 groups was also found to be statistically significant (p < 0.001). Our study revealed that nasal polyps of aspirin-sensitive patients were infiltrated with eosinophils and mast cells, Cys-LT(1) receptor proportions in these inflammatory cells were found to be higher and Cys-LT(1) receptor immunoreactivity in eosinophils and mast cells was increased.

A case of cerebral venous infarction (CVI) as a complication of acute otitis media (AOM) was pres... more A case of cerebral venous infarction (CVI) as a complication of acute otitis media (AOM) was presented in a 16-month-old male patient. The patient admitted with AOM in the right ear, ipsilateral facial paralysis and contralateral hemiplegia. Computerized tomography of the brain showed low density areas involving both the cortex and subcortical white matter in the right frontoparietal region, and there were patchy and multifocal enhancing areas with intravenous contrast enhancement. These findings disclosed the diagnosis of venous infarctions involving the superficial cortical veins on the right side. Complete recovery was achieved with 2 weeks of sulbactam-ampicilline, amikacin and prednisolone treatment. Although it is rather rare, CVI should also be remembered among the otogenic intracranial complications.

Ten patients (2 males, 8 females) underwent transnasal endoscopic repair for choanal atresia. Involvement was unilateral in five cases and bilateral in five cases. Unilateral patients were treated at a mean age of 22.6 years (range 1 to 44 years), while bilateral cases were treated within the first six days of life. Two patients with bilateral involvement had associated polydactyl and tracheoesophageal fistula, respectively. The mean follow-up period was 26 months (range 8 to 56 months). No postoperative complications such as significant hemorrhage, injury to the skull base, cerebrospinal fluid fistula, septal perforation, or granulation tissue formation were encountered. All the patients had choanal patency at the end of six months. Endoscopic repair of choanal atresia is effective in both unilateral and bilateral cases.

Tuberculous involvement of the neck is also possible. Patients mostly present with masses or draining fistulas in the neck, which are mostly located in the posterior cervical, submandibular, and supraclavicular regions. We report on a soldier who presented with a tuberculous deep neck infection, followed by miliary tuberculosis. The clinical presentation and diagnostic dilemmas are discussed.

The magnetic resonance imaging revealed that the dimensions of the mass were 29×19×17 mm. The most probable diagnosis with the magnetic resonance imaging findings was dermoid cyst. The mass was a cyst and marsupialized by functional endonasal endoscopic sinus surgery without any complication. Intraoperative findings of the mass were thought to be a hydatid cyst, but the diagnostic tests were negative, and the pathologic result was consistent with mucocele.

Material and Methods: Nucleus ® 24 Contour Advance™ electrodes were inserted into a human scala tympani model filled with different intermediates: glycerin, sodium hyaluronate (healon), water or soap solution. Additionally, electrodes were coated with BDNF and GFP producing fibroblasts and were examined with fluorescence microscopy. Force measurements were performed during the insertions by a force measurement system (Instron 5542) equipped with a 10 N load cell. Results: The lowest forces were documented with the soap solution followed by healon and glycerin, and highest values with water. Coating of the electrodes with cells resulted in decreased forces, too. Fibroblasts were removed from the lateral electrode surface but remained intact on the surface facing the modiolus. Conclusions: Modification of the electrode surface or the environment can result in a significant decrease of the insertion forces.

Fifty-five patients with pediatric neck masses, who were diagnosed and treated surgically were retrospectively analyzed. Analysis of 55 patients showed that congenital neck masses were the most common pathology in pediatric neck masses (47%), followed by infectious and tuberculous lymphadenopathies (24%). It can be concluded from the study that besides the age group and the characteristics of the neck mass, life standards of the region is also an important factor to be considered.

The aim of this study was to detect cysteinyl leukotriene (Cys-LT) receptors in aspirin-sensitive... more The aim of this study was to detect cysteinyl leukotriene (Cys-LT) receptors in aspirin-sensitive nasal polyposis patients, and compare them with nasal polyposis and chronic rhinosinusitis patients without aspirin sensitivity. The formalin-fixed, paraffin-embedded tissue blocks of a total of 52 patients undergoing endoscopic sinus surgery were reviewed by an experienced head and neck pathologist. Of these 52 cases, there were 22 nasal polyposis cases with aspirin sensitivity, 15 nasal polyposis cases without aspirin sensitivity and 15 chronic rhinosinusitis cases without aspirin sensitivity. Immunohistochemical analyses for Cys-LT(1) receptors were performed with polyclonal rabbit antibody. Eosinophil receptor intensity levels were 27.68, 21.67, and 14.40, respectively. This was statistically insignificant (p <0.089). Mast cell receptor intensity levels were 8.41, 5.27, and 4.27, respectively. In the aspirin-sensitive group, mast cell receptor intensity was significantly higher when compared to the nasal polyposis and chronic rhinosinusitis group (p < 0.0001 and p < 0.0001, respectively). The difference in Cys-LT(1) receptor immunoreactivity in both eosinophils and mast cells among the 3 groups was also found to be statistically significant (p < 0.001). Our study revealed that nasal polyps of aspirin-sensitive patients were infiltrated with eosinophils and mast cells, Cys-LT(1) receptor proportions in these inflammatory cells were found to be higher and Cys-LT(1) receptor immunoreactivity in eosinophils and mast cells was increased.

A case of cerebral venous infarction (CVI) as a complication of acute otitis media (AOM) was pres... more A case of cerebral venous infarction (CVI) as a complication of acute otitis media (AOM) was presented in a 16-month-old male patient. The patient admitted with AOM in the right ear, ipsilateral facial paralysis and contralateral hemiplegia. Computerized tomography of the brain showed low density areas involving both the cortex and subcortical white matter in the right frontoparietal region, and there were patchy and multifocal enhancing areas with intravenous contrast enhancement. These findings disclosed the diagnosis of venous infarctions involving the superficial cortical veins on the right side. Complete recovery was achieved with 2 weeks of sulbactam-ampicilline, amikacin and prednisolone treatment. Although it is rather rare, CVI should also be remembered among the otogenic intracranial complications.
