Primary closure techniques that have been updated and longterm follow-up for CBE (classic bladder... more Primary closure techniques that have been updated and longterm follow-up for CBE (classic bladder exstrophy) may be out of reach for many patients living in resource-limited settings. Late referrals to medical care and primary closures that lack the necessary skills and facilities for comprehensive treatment are still common. Alternative and long-term surgical solutions may improve the lives of these unfortunate patients. During surgical outreach missions, patients with CBE, either non-operated or with a previous unsuccessful bladder closure, who were referred from vast under-resourced rural areas to three Eastern African hospitals, were studied. The following information is provided: mode of presentation, clinical history, diagnostic workout, management, and outcome. There were 25 cases (M/F ratio 17/8) ranging in age from two days to twenty years. Narrative operative reports may fail to include some critical findings. Furthermore, standardized operative reports can form the basis of a local registry, which is often lacking in limited‐resource settings (LRSs). In adult literature, synoptic operative reports (SOR) contain more key findings than narrative operative reports. In the LRSs, where the capacity of diagnostic pathology services is typically suboptimal, the value of a thorough operative report is even greater. We report three cases of foreign body esophagus, in two of them the foreign body was a coin, and ... more We report three cases of foreign body esophagus, in two of them the foreign body was a coin, and the third child ingested a disc battery. In all three cases the foreign body was impacted in the mid esophagus. All were initially evaluated by chest X ray which confirmed the diagnosis.One underwent flexible endoscopic extraction initially followed by rigid esophagoscope later and in the other two extractions was performed using rigid esophagoscope, two of them ended with perforation of the esophagus and treated conservatively with only chest tube insertion and supportive management. In the third child who ingested a disc battery, esophagoscopy revealed necrosis and perforation at the site of impaction with formation of trachea-esophageal fistula, extraction was performed but the fistula necessitated surgical closure which failed and therefore underwent stent placement to end with complete cure. In this paper we describe the causes, patterns of presentation, and management of obstructive uro... more In this paper we describe the causes, patterns of presentation, and management of obstructive uropathy in Sudanese patients in a retrospective multi-center audit. All patients who presented with obstructive uropathy during 2005 were included in this study. All of the patients were subjected to serial investigations including imaging and tests of renal function. Diversion, stenting, and/or definitive surgery were performed in order to relieve the obstruction. Five hundred twenty patients were diagnosed with obstructive uropathy during this period; 345 (66%) patients presented with chronic obstruction and 175 (34%) with acute obstruction. Of the study patients, 210 (40%) presented with significant renal impairment; 50 (23%) of them required emergent dialysis. The patterns of clinical presentation of the obstructed patients included pain at the site of obstruction in 48%, lower urinary tract symptoms in 42%, urine retention in 36.5%, mass effect in 22%, and anuria in 4%. Tracheoesophageal fistula (TOF) is a congenital communication between the trachea and oesophagus.... more Tracheoesophageal fistula (TOF) is a congenital communication between the trachea and oesophagus. TOF often leads to severe and fatal pulmonary complications. It is usually combined with esophageal atresia. More than 95 types had been described in the literature up to 1976 (1) . Since then newer subtypes also were described (2-7) . The commonest one is type iii. The latter is diagnosed by suggestive clinical presentation and gaseous abdomen in the X-Ray. Gasless abdomen in this type of oesophageal atresia is an extremely rare condition and makes a diagnostic dilemma.

This study is designed to reflect our local experience in GNCPS regarding the contributing risk factors and outcomes of management of children presented with perforated appendicitis. Methods: This prospective study comprised 200 consecutive patients (age <16 yrs) who were admitted with acute appendicitis at GNCPS between 2016 and 2017. Patients were divided into two groups: Perforated appendicitis group and non-perforated appendicitis (including; simple appendicitis, appendicular abscess, and appendicular mass) group. Results: Two patient’s groups were analyzed. Among 200 patients, 31 patients (15.5%) were classified in the Perforated Appendicitis group, and 169 patients (84.5%) were in the NonPerforated Appendicitis group. ...

Neuroblastoma (NBL) is the most common malignant solid extracranial tumour in children. It accoun... more Neuroblastoma (NBL) is the most common malignant solid extracranial tumour in children. It accounts for about 8%-10% of overall childhood malignancies in developed countries and only 1%-3% in developing countries. We aim to study the clinical presentation and outcome of NBL in children treated at the National Cancer Institute (NCI), University of Gezira, Wad Madani, Sudan. This is a retrospective, descriptive, hospital-based study, where patients' records were reviewed from the years 2004-2015. During this period, 38 paediatric patients were treated in the Paediatric Oncology Unit at the NCI, accounting for about 4% of the overall childhood malignancies. Of them, 24 (63%) were males and 14 (37%) were females with a ratio of 1.7: 1. The age at presentation ranged between 2 months and 7 years with a mean age at the presentation of 3 years. Twenty-nine (76%) of them were classified as high-risk disease, no one received highdose chemotherapy, 33(87%) died and only 5 (13%) achieved complete remission. This study reflects the poor outcome of NBL among Sudanese children which can be explained by the late presentation of the patients, lack of the diagnostic modalities and lack of the sophisticated treatment modalities for highrisk NBL.

The objectives of this study were to determine the indications for uretero-neocystostomy and to evaluate the complications of different techniques. This retrospective descriptive study was conducted in the Gezira Hospital for Renal Diseases and Surgery, Sudan between January 2001 and January 2004. A total of 65 patients were enrolled in this study. All of them underwent reimplantation of either one or both ureters for various indications. The methods of reimplantation were direct, Lich Gregoire, Cohen, Boari's and ileum substitution. The mean follow-up period was18 months. The 65 patients underwent a total of 77 reimplanted ureters. Of them, 39 ureters were gynecological ureters, 21 were renal transplant and 17 others had miscellaneous indications. Wilms tumour is one of the commonest childhood solid tumours which has an excellent outlook in th... more Wilms tumour is one of the commonest childhood solid tumours which has an excellent outlook in the developed world with 5-year overall survival exceeding 90%. There is little information from Sudan regarding Wilms tumour. Records of patients with Wilms tumour diagnosed and treated at Institute of Nuclear Medicine, Molecular Biology and Oncology (INMO) in the University of Gezira from May 1999 to June 2007 were reviewed. Thirty-seven children presented at a mean age of 4.1 years (range 2 months-13 years). The male to female ratio was 0.9-1. Abdominal swelling or mass was the commonest symptom. There was 1 child with Stage I (2.7%), 7 with stage II (18.9%), 25 with Stage III (67.6%) and 4 with Stage IV (10.8%). Following diagnosis 27% of children did not receive further treatment (5.4% died prior to treatment, 5.4% were not able to finance treatment and for the rest 16.2% no cause was identified). More than half of the children did not have a nephrectomy and only 4 (11%) completed treatment. The poor outlook is related to several factors. Delayed presentation, poor awareness of treatment options, lack of finances, no provision of food, lodging and transport, absence of paediatric trained staff are the obstacles to better outcomes. Empowering parents with information, giving chemotherapy prior to nephrectomy, training staff and establishing links with a tertiary cancer centre in the developed world are some of the options to improve survival.

This study was a retrospective and prospective descriptive hospital-based study involving (131) child with Acquired anorectal conditions (AARC) who presented to GNCPS during the two-year study period. Data regarding the presentation, management, and post-operative course to look after complications and outcomes were collected. Data were collected using a formulated questionnaire and analyzed using (SPSS 17). Data considered statistically significant when P-value is less than 0.05. The study involved a total number of 131 children with AACS. The mean age of presentation is (5.78 years) with an overall 1.4:1 male to female ratio. Materials and Methods: In this study the records of 73 patients with endemic vesical stones were reviewed at Gezira National Center For Pediatric Surgery (GNCPS). The data were analyzed for age, sex, locality, clinical features the diagnostic tools, the treatment offered and outcomes. Results: A total of 73 patients were included in this study, males were 64 (87.7%), < 5 years were 54 cases ( 74%), the majority from rural areas of low socio-economic status. This is a retrospective, descriptive, hospital-based study, where patients' records were reviewed from the years 2004-2015. During this period, 38 paediatric patients were treated in the Paediatric Oncology Unit at the NCI, accounting for about 4% of the overall childhood malignancies. Of them, 24 (63%) were males and 14 (37%) were females with a ratio of 1.7: 1. The age at presentation ranged between 2 months and 7 years with a mean age at the presentation of 3 years. Twenty-nine (76%) of them were classified as high-risk disease, no one received highdose chemotherapy, 33(87%) died and only 5 (13%) achieved complete remission. This study reflects the poor outcome of NBL among Sudanese children which can be explained by the late presentation of the patients, lack of the diagnostic modalities and lack of the sophisticated treatment modalities for highrisk NBL.

The objectives of this study were to determine the indications for uretero-neocystostomy and to evaluate the complications of different techniques. This retrospective descriptive study was conducted in the Gezira Hospital for Renal Diseases and Surgery, Sudan between January 2001 and January 2004. A total of 65 patients were enrolled in this study. All of them underwent reimplantation of either one or both ureters for various indications. The methods of reimplantation were direct, Lich Gregoire, Cohen, Boari's and ileum substitution. The mean follow-up period was18 months. The 65 patients underwent a total of 77 reimplanted ureters. Wilms tumour is one of the commonest childhood solid tumours which has an excellent outlook in th... more Wilms tumour is one of the commonest childhood solid tumours which has an excellent outlook in the developed world with 5-year overall survival exceeding 90%. There is little information from Sudan regarding Wilms tumour. Records of patients with Wilms tumour diagnosed and treated at Institute of Nuclear Medicine, Molecular Biology and Oncology (INMO) in the University of Gezira from May 1999 to June 2007 were reviewed. Thirty-seven children presented at a mean age of 4.1 years (range 2 months-13 years). The male to female ratio was 0.9-1. Abdominal swelling or mass was the commonest symptom. There was 1 child with Stage I (2.7%), 7 with stage II (18.9%), 25 with Stage III (67.6%) and 4 with Stage IV (10.8%). Following diagnosis 27% of children did not receive further treatment (5.4% died prior to treatment, 5.4% were not able to finance treatment and for the rest 16.2% no cause was identified). More than half of the children did not have a nephrectomy and only 4 (11%) completed treatment. The poor outlook is related to several factors. Delayed presentation, poor awareness of treatment options, lack of finances, no provision of food, lodging and transport, absence of paediatric trained staff are the obstacles to better outcomes. Empowering parents with information, giving chemotherapy prior to nephrectomy, training staff and establishing links with a tertiary cancer centre in the developed world are some of the options to improve survival.

This study was a retrospective and prospective descriptive hospital-based study involving (131) child with Acquired anorectal conditions (AARC) who presented to GNCPS during the two-year study period. Data regarding the presentation, management, and post-operative course to look after complications and outcomes were collected. Data were collected using a formulated questionnaire and analyzed using (SPSS 17). Data considered statistically significant when P-value is less than 0.05. Materials and Methods: In this study the records of 73 patients with endemic vesical stones were reviewed at Gezira National Center For Pediatric Surgery (GNCPS). The data were analyzed for age, sex, locality, clinical features the diagnostic tools, the treatment offered and outcomes. Results: A total of 73 patients were included in this study, males were 64 (87.7%), < 5 years were 54 cases ( 74%), the majority from rural areas of low socio-economic status. Conclusion: Endemic vesical stones in children is common in males below 5 years and low socio-economic status, with UTI like symptoms and penile itching
