The first confirmed case of 2019-nCoV infection in Albania was reported in Tirana on 08 March 2020, when a patient and his adult son who had come from Florence, Italy tested positive. Patients with COVID-19 can be presented with a series of signs and symptoms. Acute abdomen as a presentation of COVID-19 is rare. The diagnosis of COVID-19 should be suspected and investigated in every case of acute abdomen. Case presentation: We report a case of a 42-year-old male who presented with features of acute appendicitis. SARS-CoV-2 polymerase chain reaction test result was positive for COVID-19. Abdominal ultrasonography and his computed tomography of the chest and abdomen showed a perforated appendix and no infiltrates or abnormalities of COVID-19. We presently report the case of hantavirus infection in a 45-year-old male who was hospitalized t... more We presently report the case of hantavirus infection in a 45-year-old male who was hospitalized to our clinic of infectious diseases, with fever, myalgia, vomiting, nausea, headache, and abdominal pain. The physical findings included hepatomegaly, splenomegaly, rash, and conjunctival injection. Eight days before the start of complaints, the patient has cut trees in the mountain. An acute renal failure was observed with an oliguria and an increase of serum creatinine and blood urea nitrogen. Urinalysis shows albuminuria and hematuria. Elevations of amylase, lipase, and liver enzymes levels, low serum albumin level, and thrombocytopenia were observed. A positive ELISA test for hantavirus IgM/IgG antibodies confirmed hemorrhagic fever with renal syndrome. On the third day of hospitalization, the patient had seizures. The unenhanced head computed tomography (CT) performed after seizures showed subcortical bilateral hypodensities within frontal, parietal, and occipital regions correspond...

We presently report the case of renal infarction in a 31 years old woman who represented in Surgi... more We presently report the case of renal infarction in a 31 years old woman who represented in Surgical Emergency Department with severe right flank pain, nausea with 2-3 episodes of vomiting, high temperature and fatigue for over 3 days. She has had one episode with dark urine. On initial investigations including a total blood count, urinary analysis and renal tract ultrasound there was only hematuria and anemia. A CT angiogram was performed due to persisting pain, which demonstrated multiple segmental infarctions of her right kidney. For further evaluation, this patient was hospitalized in Infectious Diseases Department. After all the laboratory, radiological examinations and multidisciplinary medical consultations we excluded thromboembolic, surgical, infectious, hematological, oncological, urological, nephrological and rheumatological causes. The only abnormal finding was a positive antinuclear antibody test. The patient was discharged on the third week of hospitalization.

Measles is a serious respiratory disease that causes a maculopapular rash and fever. It is transm... more Measles is a serious respiratory disease that causes a maculopapular rash and fever. It is transmitted by the respiratory route and is highly infectious. Measles virus infects multiple organs and targets epithelial, reticuloendothelial and white blood cells, including monocytes, macrophages and T-lymphocytes which are vital components of our body defense mechanism [1]. Pathological studies of children dying during acute measles have found multinucleated giant cells typical of measles virus infection throughout the respiratory and gastrointestinal tracts and in most lymphoid tissues [2,3]. Measles causes a systemic illness [4]. After an incubation period of 8–12 days, measles begins with increasing fever (39–40.5°C), cough, coryza and conjunctivitis [5]. The rash is usually first noted on the face and neck, appearing as discrete erythematous patches 3–8 mm in diameter. To describe the clinical, laboratory, microbiological, and echocardiographic findings in four int... more To describe the clinical, laboratory, microbiological, and echocardiographic findings in four intravenous drug users with endocarditis hospitalized and followed in our Infectious Disease Service, a tertiary university hospital as well as to determine the efficacy of medical treatment. Methods: From a database of 35 subjects with endocarditis during five years, we made a retrospective analysis of data for four cases between the age of 24-33 years old which were intravenous drug users. Results: Infective endocarditis was encountered in four drug users with positive blood cultures (Staphylococcus aureus was present in all the cases), vegetations in the tricuspid native valve in ultrasound, high fever (more than 38 o C). The four cases were male and the mean age was 29 years (range 24-33 years). Three out of the four cases presented with pulmonary involvement and only one with femoral and popliteal vein thrombosis. Two out of four cases had acute renal and hepatic failure and only one had acute cutaneous vasculitis. Transesophageal Echocardiography (TEE) was also performed in two cases. For all of them medical management consisted of antibiotic therapy and two out of them underwent surgery because of the persistence of valvular vegetations after antibiotic therapy. The prognosis was good with 0% mortality. Conclusion: Infective endocarditis should be considered in the differential diagnosis of intravenous drug users presenting with various clinical scenarios. Echocardiography remains the main modality and should be used serially to facilitate early diagnosis. The successful management of a complicated case often requires the close cooperation of an infectious disease physician, a cardiologist, an addiction physician and occasionally a cardiac surgeon.

r i We presently report the case of hydronephrosis, urinoma formation and urosepsis resulting fro... more r i We presently report the case of hydronephrosis, urinoma formation and urosepsis resulting from ureter obstruction caused by an ingested fishbone that has passed spontaneously without any symptom through intestinal loop to the right ureter. Computed tomography indicated a high-attenuation sharp elongated object located transversally to the right ureter that we thought it was a fish bone. The foreign body was removed surgically and was proven a fishbone. To our knowledge, there has been no previous report of such ureteral obstruction published in the English literature mproves the diagnostic accuracy and may help in management.
