Research paper thumbnail of Mother-friendly hospital programme of Turkey: national intervention to improve the quality of maternity services

Eastern Mediterranean Health Journal, Feb 1, 2021

Background: The mother-friendly care model and mother-friendly programmes are powerful responses ... more Background: The mother-friendly care model and mother-friendly programmes are powerful responses to solve problems in maternity care services including high rates of caesarean section, low breastfeeding rates, and women's rights issues. Aims: This paper describes the development and implementation of the mother-friendly hospital programme of Turkey. Methods: The Ministry of Health initiated the mother-friendly hospital programme in 2010. National mother-friendly hospital standards, guidelines and evaluation tools were developed. Implementation began in 2015. Results: Training activities were carried out to enable health care workers (e.g. physicians, midwives, nurses) to train staff of hospitals applying to the programme, and to evaluate and monitor their maternity services; 455 health care workers were trained. Participation in the programme is voluntary and hospitals apply for certification. To apply, the hospital's caesarean section rate must be lower than the country average. As of January 2020, 73 hospitals have been certified as mother-friendly hospitals. Conclusion: As a practical implementation of the mother-friendly approach, Turkey's experience may be useful to other health systems.

In this study it was aimed to examine the knowledge, smoking patterns, and the perceptions of narghile-(Waterpipe) smokers in Turkey. Methodology: In this descriptive survey, the data was collected through a structured questionnaire. The questions focused on smoking patterns of narghile smokers who were the clients (n=460) of narghile cafes. The perceptions of participants were identified using a 10 point visual scale. Results: In all, 76.7% was male and 9.6% was younger than 18 years, and the mean duration of narghile smoking was 27.1 months. Peer influence (38.4%), and curiosity (18.4%) were the two most common reasons for starting narghile smoking. Aromatic narghile tobacco was preferred (86.3%) mostly, and 50.1% of them did not believe the addictive pattern of narghile smoking. One third of narghile smokers rated the negative health consequences of narghile smoking between 8.1 and 10 point on the visual scale versus 74.9% for cigarette. Conclusions: Interventions are needed to inform people in Turkey about the health hazards of narghile (Waterpipe) smoking.

Anahtar Kelimeler: Kadın hakları, toplumsal cinsiyet ayrımcılığı, evlilik yasağı, bekleme süresi, soybağı Evaluation of 132th Article of Civil Code on Waiting Period (Iddet Duration) in the Light of Women's Rights and Modern Medical Developments Article 132 of the Turkish Civil Code does not give any restriction for the male if the marriage is final, but does not allow women for marriage within 300 days after the end of marriage. If a woman wants to marry someone other than her divorced husband during this duration called as "waiting period ", she can get a marriage permit by proving that she is not medically pregnant by applying to the court. Waiting period after divorce for women in the Civil Code is discriminative for women. This article aims to examine the waiting period in terms of patriarchy, women rights in relation to paternity and current medical developments.

Method: The study included a total of 8407 students enrolled as first-(4392) and third-(4015) year students. A self-administered questionnaire was used for data collection. In data analysis, respecting sampling weights, models were formed by logistic regression method to determine factors that affect the risky behaviors. Results: 731 male-1114 female students from the first year and 560 male-1096 female students from the third year were interviewed. Male students were found to be engaged in risky behaviors more frequently than females. Logistic models of the study indicated that gender, place of residence, relationship with parents, and socialization with friends have profound effects on risky behaviors. Conclusion: After leaving home, young people develop their own lifestyles, and this study demonstrates that lifestyle is the main effective factor for risky behaviors in this group. Universities need to assume more responsibility to guide students' lives and to provide the facilities and opportunities that encourage and facilitate their adoption of a healthy lifestyle.

Although there is comprehensive disaster legislation in Turkey, it has not been well studied in terms of gender sensitivity. This study aimed to evaluate the disaster legislation of Turkey in the context of gender equality. We investigated 17 non-technical disaster legislations to determine in what context and to what level disaster management is gender-sensitive in Turkey. We also carried out a workshop with experts from governmental bodies and nongovernmental organizations (NGOs), and academics to discuss the gender sensitivity of the legislations. We found that only two of the 17 legislations included a gender perspective. One legislation addressed gender equality and equity very well, while one was partially gender-sensitive and the others 5 were gender blind. The workshop was conducted in collaboration with the Disaster and Emergency Management Presidency (AFAD) and 33 participants from 19 organizations. The participants agreed that the disaster legislation did not include a gender perspective and acknowledged the importance of integrating a gender perspective into the disaster policies of Turkey. They recommended that the AFAD and emergency response units should have at least one female president or vice president, gender equality training should be provided to officers and managers of government and non-governmental disaster institutions, and a working group should be established to develop strategies that integrate gender perspectives into disaster and emergency policies. The disaster legislation of Turkey is not gender sensitive. It is important to incorporate language terminologies, understanding, and approaches that are sensitive to gender into disaster-related legislation and regulations.

Knowledge of chronic pain and its relationship to depression among primary health care patients i... more Knowledge of chronic pain and its relationship to depression among primary health care patients in Turkey is inadequate. The aim of this study was to evaluate the frequency of chronic pain and its relationship to depression. Materials and methods: In this descriptive study, data was collected with a questionnaire based on sociodemographic and pain-related characteristics, which was administered to a study population (n = 259) of primary care patients, 18-64 years old. The Visual Analogue Scale (VAS) (min-max = 0-10) and Beck Depression Inventory (BDI) (min-max = 0-63) were used for the assessment of pain severity and depression. Results: Of the participants, 50.6% complained of chronic pain and 48.8% had only one type of chronic pain. According to the VAS, 64.9% of patients' chronic pain severity scores were between 4 and 8. Among those without chronic pain, 24.2% got a score equal to 17 or higher on the BDI (increased risk for depression), as compared with 38.3% of the patients who had chronic pain complaints (P = 0.015). Conclusion: This study is one of the few that have investigated chronic pain and considered it as a serious health problem among primary health care patients in Turkey. Physicians' attention should be drawn to the effects of chronic pain, causing mental health disability, within the health system of Turkey.
