Here, we report that plasma protein functional effector sncRNAs (pfeRNAs) serve as non-invasive biomarkers for determining both the existence and the nature of pulmonary nodules in a three-stage study that included the healthy group, patients with benign pulmonary nodules, patients with suspicious nodules, and patients with malignant nodules. Following the standards required for a clinical laboratory improvement amendments (CLIA)-compliant laboratory-developed test (LDT), we identified a pfeRNA classifier containing 8 pfeRNAs in 108 biospecimens from 60 patients by sncRNA deep sequencing, deduced prediction rules using a separate training cohort of 198 plasma specimens, and then applied the prediction rules to another 230 plasma specimens in an independent validation cohort. For controlling dynamical systems certain controllers directly observe the physical response of t... more For controlling dynamical systems certain controllers directly observe the physical response of the controlled systems and feed back the observed response for stabilization purposes. For instance, in robotics, where the rules of Classical Mechanics govern the behavior of the controlled system, this is the acceleration that is measured and fed back by the Acceleration Feedback Controllers. In quite different manner, the Fixed Point Iteration (FPI)-based adaptive controllers also feed back such observed information. If the driving forces act on a robot link through some deformable, elastic component, the relative order of the control task can be higher than two, and the system's response can be a higher order time-derivative of the observable coordinate. In this case the numerical derivatives of the noise-burdened measurement signals suffer from huge fluctuations due to which the need for the application of noise filtering techniques arises. Generally, any noise filtering corresponds to some “averaging” of the fresh, recent dynamic information with older, more or less “obsolete” one so it may corrupt the operation of the FPI-based control. The extent of this degradation depends on the “dynamics” of the nominal trajectory to be tracked, the nonlinearities in the dynamic model of the controlled system and cannot be tackled at high level generality. In the present paper a new, simple noise filtering technique is recommended for use in the FPI-based adaptive controller. Its corruption effect is investigated for a chirp signal independently of any dynamic control issue, then its application in the FPI-based adaptive control of a popular, nonlinear benchmarking system, the Duffing oscillator is studied via numerical simulations.

The development of a capacitive microphone with an integrated detection circuit is described. The... more The development of a capacitive microphone with an integrated detection circuit is described. The condenser microphone is made by micromachining of polyimide on silicon. Therefore, the structure can be realized by postprocessing on substrates containing integrated circuits (IC's), independently of the IC process. Integrated microphones with excellent performances have been realized on a CMOS substrate containing dc-dc voltage converters and preamplifiers. The measured sensitivity of the integrated condenser microphone was 10 mV/Pa, and the equivalent noise level (ENL) was 27 dB(A) re. 20 Pa for a power supply voltage of 1.9 V, which was measured with no bias voltage applied to the microphone. Furthermore, a back chamber of infinite volume was used in all reported measurements and simulations. [338]

Upper Indus Basin (UIB) region has faced seasonal and sometimes unpredictable disastrous flow in ... more Upper Indus Basin (UIB) region has faced seasonal and sometimes unpredictable disastrous flow in their tributaries and contributing one of the world's largest Indus River System. As these streams emerged from high mountains of Hindukush, Karakorum and Himalaya ranges, and formed as a lifeline for the local population of which 90% is accommodate by Indus River system source. A little change in the regional climate may cause floods and outburst flows in the river and affects the lives, regional ecosystem and long part of the Karakoram highway. On the other hand, the shortage of water in Pakistan can create an alarming condition in future because a huge amount of eastern glaciers is shrinking. According to UN-ICC 2011 report Pakistan is in top of the four risky countries which adversely affected by climate change and especially worst hit by bi-catastrophes in 2010.During summer, intensifications of temperature may dissimilar for different locations/altitudes but it affects the glaciated areas. Moreover, the summer river flow and precipitation in previous winter and spring seasons has significant correlation shows their influence in the UIB region. Consequently, it may also be responsible for fluctuation in the seasonal/regular flows of UIB Rivers. To study these variations this paper analyses two types of data, mean monthly and 4-times moving average of monthly for long-term forecast. They belong to two different rivers Ghizer-Gilgit at Gilgit and Ghizer-Gilgit-Hunza at Alam stations. Both types of data illustrate a strong seasonal cycle. Therefore, seasonal autoregressive integrated moving average (SARIMA) models of time series method have been used. The five selected SARIMA models explore 90% and more river flow forecast. Moreover, the result with 4-times moving average is more accurate than simple data.

Patients with non-cyclical mastalgia were assessed for rib tenderness (lateral chest wall tenderness-LCWT). The tenderest spot was injected with a combined preparation of 1 ml 2% lignocaine and 1 ml 40 mg depomedrone. Those who declined injection therapy were advised on topical or oral NSAIDs or reassurance. All patients were reassessed 6 weeks later. A successful outcome was taken as either a complete response (CR) or partial response (PR), i.e. pain although present is tolerated. One hundred and thirty eight women were diagnosed with LCWT. One hundred and four women were injected. Thirty-four women declined and were managed appropriately. At assessment 83 injected cases were successful (83%; CR n = 61, PR n = 22) compared to 13 non-injected cases (44.8%; CR n = 5, PR n = 8, P < 0.0001). No side effects were recorded. A prospective audit of the outcome of tamoxifen routinely given to men with physiological gynaecomastia was carried out at Nottingham. Men referred with gynaecomastia had clinical signs recorded, e.g., type (diffuse &#39;fatty&#39; or retro-areolar &#39;lump&#39;), size and possible aetiology. They were offered oral tamoxifen 20mg once daily for 6-12 weeks. On follow-up patients were assessed for complete resolution (CR), partial resolution where patient is satisfied with outcome (PR) or no resolution (NR). Success was either CR or PR. Thirty-six men accepted tamoxifen for physiological gynaecomastia. Median age was 31 (range 18-64). Tenderness was present in 25 (71%) cases. Sixteen men (45%) had &#39;fatty&#39; gynaecomastia and 20 had &#39;lump&#39; gynaecomastia. Tamoxifen resolved the mass in 30 patients (83.3%; CR=22, PR=8) and tenderness in 21 cases (84%; CR=0, PR=0). Lump gynaecomastia was more responsive to tamoxifen than the fatty type (100% vs. 62.5%; P=0.0041). Oral tamoxifen is an effective treatment for physiological gynaecomastia, especially for the lump type.

ABSTRACT This research presents a novel hand exoskeleton rehabilitation device to facilitate tend... more ABSTRACT This research presents a novel hand exoskeleton rehabilitation device to facilitate tendon therapy exercises. The exoskeleton is designed to assist fingers flexion and extension motions in a natural manner. The proposed multi-Degree Of Freedom (DOF) system consists of a direct-driven, optimized and underactuated serial linkage mechanism having capability to exert extremely high force levels perpendicularly on the finger phalanges. Kinematic and dynamic models of the proposed device have been derived. The device design is based on the results of multi-objective optimization algorithm and series of experiments conducted to study capabilities of the human hand. To permit a user-friendly interaction with the device, the control is based on minimum jerk trajectory generation. Using this control system, the transient response and steady state behavior of the proposed device are analyzed after designing and fabricating a two-fingered prototype. The pilot study shows that the proposed rehabilitation system is capable of flexing and extending the fingers with accurate trajectories.
