Although generally benign reflux can, if untreated, lead to severe complications. This article describes the symptoms and diagnosis of acid reflux and GORD, and reviews currently available treatments ranging from lifestyle modification to surgery.

Its cause and pathogenesis, however, remain poorly understood. Diagnosis is usually straightforward and detailed investigations are only required when the presentation is atypical. Treating IBS is always a challenge. The concerns and expectations of the patient must be met, along with any associated psychosocial issues. Medication on its own without addressing all of these issues is usually unsuccessful.

A specially designed questionnaire was administered to representatives of higher education institutions where nurse education took place and representatives of the NMC in all four countries of the UK. Quality assurance activity had taken place across the UK, mostly in the shape of validation events. Quality assurance events proceeded largely as expected by higher education and NMC representatives. There was a statistically significant trend towards greater preparedness and knowledge for validation and satisfaction with the new quality assurance procedures in Scotland, Wales and Northern Ireland compared with England. At the time of this study the new quality assurance procedures were less than 12 months old and there may have been resistance to change in England where the changes were greatest.

A 35-item version of the caring dimensions inventory was administered to a cohort of nursing students in a department of nursing in Scotland at entry to the programme, after 12 months and after 24 months on the programme. Caring was largely perceived through a technical dimension, demonstrated by factor analysis, but other dimensions such as intimacy, support and unnecessary and inappropriate aspects of nursing also became apparent as students progressed through the programme. #

Specific objectives of the study were to demonstrate the reliability of the instrument. DESIGN: The study, using a correlational design, was designed to test inter-and intra-rater reliability of the instrument using a sample of 45 patients and 45 pairs of assessors. Each pair of assessors assessed the same ...

The private sector is an important contributor to residential intermediate care, and the voluntary sector is also involved in a variety of activities. However, volunteers' contribution is little discussed, and its potential unknown. This article outlines features of intermediate care that may or may not be attractive to volunteers. A survey of volunteer perceptions of their role in one voluntary sector ...

Research paper thumbnail of Violations of local stochastic independence exaggerate scalability in Mokken scaling analysis of the Chinese Mandarin SF-36

However, the values of scalability of the subscales are very large, raising the possibility that these are artificially high and this may result from violations of local stochastic independence between items. To analyse selected items from the Chinese Mandarin form of the SF-36 scale using Mokken scaling and to investigate if violations of local stochastic independence exaggerate scalability. Exploratory Mokken scaling analysis was run using the online public domain software R by entering 19 items from the Chinese Mandarin form of the SF-36 items into the analysis. The items in the resulting scales, judged by the size of Loevinger's coefficient, were analysed for violations of monotony, 95% confidence intervals and invariant item ordering, including inspection of item pair plots. Two Mokken scales were obtained, one including items from the Physic...

Methods We administered in face-to-face interviews the MOS-SSS-CM to 200 patients with coronary heart disease. Data were analysed using Mokken scaling. Results The MOS-SSS-CM contains a set of items which meet the criteria of a reliable (Rho = 0.87) moderate Mokken scale (H = 0.40) with weak invariant item ordering (H = 0.39).

Research paper thumbnail of Investigating invariant item ordering in the Mental Health Inventory: An illustration of the use of different methods

Personality and Individual Differences, 2014

In the package 'mokken' in the statistical software R, the ability to analyse Mokken scales for invariant item ordering has recently been available and techniques for inspecting visually the item response curves of item pairs, have also been included. While methods to assess invariant item ordering are available, there have been indications that items representing extremes of distress in mental well-being scales, such as suicidal ideation, may lead to claiming invariant item ordering where it does not exist. We used the Mental Health Inventory to see if invariant item ordering was indicated in any Mokken scales derived and to see if this was being influenced by extreme items. A Mokken scale was derived indicating invariant item ordering. Visual inspection of the item pairs indicated that the most difficult item (suicidal ideation) was located far from the remaining cluster of items. Removing this item lowered invariant item ordering to an unacceptable level.

We conclude that Mokken scaling is an appropriate method for investigating the existence of hierarchical scales in general and, specifically, addresses the issue of invariant item ordering in polychotomous data.
