Continuous evaluation of this environment will help in maximizing the learning opportunities of the training program. Objective: To assess the resident doctors' perception of their learning environment at a teaching hospital in Nigeria Methods: The DREEM questionnaire was administered to participants undergoing residency training in the faculty of Dental Surgery at the University College Hospital, Ibadan. Results: Thirty-nine resident doctors participated in the study (23 Males, 16 Females), mean age (±SD) was 35.7 (±4.22) ranging from 28 years to 46 years. Mean global score (±SD) was 105.3 (±26.8), 52.7%, out of a maximum of 200. The mean global score according to gender was significantly higher among males than females. The overall perception of the training environment by residents is more positive than negative and the male gender appears to have a more ...

Background: Exogenous factors occurring in the antenatal period could be contributory to the form... more Background: Exogenous factors occurring in the antenatal period could be contributory to the formation of orofacial cleft. This study sought to determine the antenatal events in mothers that may have contributed to orofacial cleft deformity of their children. Methodology: It was a prospective observational cross sectional study of consenting mothers of babies with orofacial cleft who met the inclusion criteria. The study instrument was a questionnaire. Results: Seventy-two mothers participated in the study. Most of these mothers were below 35 years of age and more than half, 43 (59.7%) were of the low-intermediate socioeconomic status. Although majority, 70 (97.2) of the mothers had antenatal care, the mean gestational age at commencement of antenatal care was 4 months. Almost all, 69 (95.8%) mothers had ultrasound scans however the detection of the orofacial cleft was in only 2 (2.8%) mothers. The commonest medication taken was haematinics, 26 (36.1%). Material and Methods: The surgical difficulty of third molar extraction was predicted in 79 Niger... more Material and Methods: The surgical difficulty of third molar extraction was predicted in 79 Nigerian patients based on the criteria of the Pederson index. The actual intraoperative experience of difficulty was determined by the operative time. Operations that lasted for 20 min or less were described as easy while those that lasted for more than 20 min were regarded as difficult. The surgeon was blinded to the pre-operative prediction which was done by a consultant oral surgeon. Results: Sensitivity and specificity of the index were determined as 94.9% and 49.0%, respectively, while the positive predictive (PPV) value was 62.7%. Conclusion: Although the Pederson index was highly sensitive for predicting difficult cases, the poor specificity suggested its inability to accurately identify easy cases and the high PPV confirmed a tendency towards false positive predictions alleged by some previous authors. Thus, we concluded that the Pederson index is an unreliable instrument for predicting the difficulty of third molar surgery. A retrospective review of all new cleft patients presenting to The UCH between January 2012 and June 2015. The data obtained were their local government area of residence, age of the patients, gender of the patients and the type of cleft. Descriptive statistics was used to analyze the distribution of patients seen while Chi square test was used to analyze the influence of gender and laterality on the type of cleft. Imaging of such objects is of paramount importance to their retrieval in order to assess their relation to vital structures as well as avoid disruption of such structures while retrieving the foreign bodies. We report a case of wooden foreign body which traversed the maxillofacial region, imaging of which posed a significant challenge. A sixteen year old female presented with a wooden foreign body impaction following a road traffic accident. Challenges in the course of investigation and events during the surgical retrieval of the foreign body are described. An unusual case of foreign body impaction transecting the right orbit, right antrum and the pharynx is presented. Standard medical note keeping is an important aspect of patient management, and the importance of... more Standard medical note keeping is an important aspect of patient management, and the importance of completeness of patients' records cannot be overemphasized, especially for the purpose of auditing, research, and medico-legal reasons. It is also an integral part of a good medical practice. However gross inadequacies are often noted. This may be partly due to the fact that until the recent past, cases of medical and professional negligence were uncommon in our environment. This audit exercise was embarked upon to assess the standard in the department using the CRABLE Scoring system with a view to standardizing and improving our practice. The result showed that the subsequent entry part of the medical notes assessed was marginally the best with a score of 66.5%, followed by initial entry which scored 65.3%, while consent had a score of 57.8%. This study was conducted between June 2007 and June 2008 at the Federal Medical Centre, Owo and State Specialist Hospital, Akure, Ondo State, Nigeria. The mean age of the dentists was 34.8 years, with a male/female ratio of 3.4:1. Most of the female dentists were found in the training institutions (p = 0.005). More than half of the dentists (58.7%) practiced in institutions with dental schools (training institutions), majority preferred the buccal approach (92%), and all the dentists (100%) preferred third molar disimpaction under local analgesia. Most dentists (88%) prescribe antibiotics following third molar surgery. Fifty dentists (66.7%) routinely reviewed all patients postoperatively while 12 dentists (16%) were of the opinion that all impacted third molars should be extracted. Maxillary Osteomyelitis is however rare compared with mandibular osteomyelitis. The extensive blood supply and the strut-like bones of the maxilla make it less prone to chronic infections. Systemic conditions that alter the host's resistance such as diabetes mellitus, autoimmune disorders, agranulocytosis, anaemia, especially sickle cell anaemia are predisposing factors for the development of this condition. An unusual case of chronic maxillary osteomyelitis induced by trauma in a diabetic, with subsequent atypical necrotising ulceration of palatal mucosa resulting in total maxillary sequestration is presented.

OBJECTIVES To describe the mortality trends and associated factors among older patients (>60 years) at the Geriatric Centre, University College Hospital, Ibadan between January 2013 and December 2017. METHODS Hospital records of older patients admitted were analysed. Data extracted included socio-demographic, diagnoses, length of stay from date of admission to discharge or death. Results of vital signs, anthropometric measurements and laboratory tests carried out at admission were also obtained. RESULTS The mean age of the 1,091 older patients admitted was 73.6±8.6 years. The overall crude proportion of in-hospital deaths was 9.0% (males=11.3% > females=7.2%, p=0.024) and it increased from 4.1% in 2013 to 12.1% in 2017....

It is also an integral part of a good medical practice. However gross inadequacies are often noted. This may be partly due to the fact that until the recent past, cases of medical and professional negligence were uncommon in our environment. This audit exercise was embarked upon to assess the standard in the department using the CRABLE Scoring system with a view to standardizing and improving our practice. The result showed that the subsequent entry part of the medical notes assessed was marginally the best with a score of 66.5%, followed by initial entry which scored 65.3%, while consent had a score of 57.8%. The mean age of the dentists was 34.8 years, with a male/female ratio of 3.4:1. Most of the female dentists were found in the training institutions (p = 0.005). More than half of the dentists (58.7%) practiced in institutions with dental schools (training institutions), majority preferred the buccal approach (92%), and all the dentists (100%) preferred third molar disimpaction under local analgesia. Most dentists (88%) prescribe antibiotics following third molar surgery. Fifty dentists (66.7%) routinely reviewed all patients postoperatively while 12 dentists (16%) were of the opinion that all impacted third molars should be extracted. Only 21 dentists (28%) were aware of any protocol on the management of impacted teeth. Systemic conditions that alter the host's resistance such as diabetes mellitus, autoimmune disorders, agranulocytosis, anaemia, especially sickle cell anaemia are predisposing factors for the development of this condition. An unusual case of chronic maxillary osteomyelitis induced by trauma in a diabetic, with subsequent atypical necrotising ulceration of palatal mucosa resulting in total maxillary sequestration is presented.

OBJECTIVES To describe the mortality trends and associated factors among older patients (>60 years) at the Geriatric Centre, University College Hospital, Ibadan between January 2013 and December 2017. METHODS Hospital records of older patients admitted were analysed. Data extracted included socio-demographic, diagnoses, length of stay from date of admission to discharge or death. Results of vital signs, anthropometric measurements and laboratory tests carried out at admission were also obtained. RESULTS The mean age of the 1,091 older patients admitted was 73.6±8.6 years.
