A 6 yr old female Labrador retriever was evaluated for an acute onset of difficulty walking and a... more A 6 yr old female Labrador retriever was evaluated for an acute onset of difficulty walking and a head tilt. Initial physical examination revealed bilateral retinal detachment, a left-sided head tilt, positional rotary nystagmus, and lumbar hyperpathia. Pertinent preliminary diagnostic findings included systemic hypertension, bony lysis and adjacent sclerosis of the vertebral endplates of the first and second lumbar vertebrae, and positive urine and blood cultures for a yeast identified as Candida spp. Concerned about disseminated candidiasis after subsequent subretinal aspirates confirmed the presence of a yeast-like organism, therapy with voriconazole was initiated. Because of progressive clinical deterioration and the poor prognosis for recovery, the dog was eventually euthanized. Postmortem histological examination of tissues, including the affected vertebral endplates, revealed numerous intralesional algae compatible with Prototheca spp. To the authors' knowledge, this report is the first to document a case of protothecal diskospondylitis with possible concurrent candidiasis in a dog. Although typically associated with signs referable to the gastrointestinal tract, this report underscores the importance of not excluding protothecosis as a differential diagnosis when such signs are absent. Lastly, the use of voriconazole appears ineffective for reversing the clinical course of late-stage disseminated protothecosis.

The mass was incompletely excised, and histological assessment resulted in a diagnosis of anal sac adenocarcinoma. The cat had a partial response to carboplatin therapy but a short overall duration of response. Necropsy confirmed the original diagnosis as well as metastasis to the regional lymph nodes and lungs.

Some patients under antiretroviral therapy (ART) do not reach immune recovery when the viral load... more Some patients under antiretroviral therapy (ART) do not reach immune recovery when the viral load becomes undetectable. This is called discordant immunologic and virologic responses. Its prevalence varies between 8% and 24%. This study describes its prevalence and the characteristics of the affected subjects in the outpatient clinic of a Brazilian specialized-care center. Of 934 patients on ART, 536 had undetectable viral loads. Prevalence was 51/536 or 9% (95% confidence interval: 6.6% to 11.4%). Median age at the beginning of ART was 37 years (interquartile range - IQR: 31 to 45). Male gender and mixed race predominated (76.5% and 47.1% respectively). AIDS-defining illnesses were absent at the beginning of ART in 60.8%. Fifty-one percent were taking protease inhibitors, 43.2% Efavirenz and 5.8% both. Background: A paradoxical immunologic response (PIR) to Highly Active Antiretroviral Therapy (HAA... more Background: A paradoxical immunologic response (PIR) to Highly Active Antiretroviral Therapy (HAART), defined as viral suppression without CD4 cell-count improvement, has been reported in the literature as 8 to 42%, around 15% in most instances. The present study aims to determine, in a cohort of HIV infected patients in Brazil, what factors were independently associated with such a discordant response to HAART. Methods: A case-control study (1:4) matched by gender was conducted among 934 HIV infected patients on HAART in Brazil. Cases: patients with PIR, defined as CD4 < 350 cells/mm 3 (hazard ratio for AIDS or death of at least 8.5) and undetectable HIV viral load on HAART for at least one year. Controls: similar to cases, but with CD4 counts ≥ 350 cells/mm 3 . Eligibility criteria were applied. Data were collected from medical records using a standardized form. Variables were introduced in a hierarchical logistic regression model if a p-value < 0.1 was determined in a bivariate analysis. Results: Among 934 patients, 39 cases and 160 controls were consecutively selected. Factors associated with PIR in the logistic regression model were: total time in use of HAART (OR 0.981; CI 95%: 0.96-0.99), nadir CD4-count (OR 0.985; CI 95%: 0.97-0.99), and time of undetectable HIV viral load (OR 0.969; CI 95%: 0.94-0.99). Conclusions: PIR seems to be related to a delay in the management of immunodeficient patients, as shown by its negative association with nadir CD4-count. Strategies should be implemented to avoid such a delay and improve the adherence to HAART as a way to implement concordant responses.

Sand fly saliva has been shown to have proteins with potent biological activities, salivary prote... more Sand fly saliva has been shown to have proteins with potent biological activities, salivary proteins that can be used as biomarkers of vector exposure, and salivary proteins that are candidate vaccines against different forms of leishmaniasis. Sand fly salivary gland transcriptomic approach has contributed significantly to the identification and characterization of many of these salivary proteins from important Leishmania vectors; however, sand fly vectors in some regions of the world are still neglected, as Bichromomyia olmeca (formerly known as Lutzomyia olmeca olmeca), a proven vector of Leishmania mexicana in Mexico and Central America. Despite the importance of this vector in transmitting Leishmania parasite in Mesoamerica there is no information on the repertoire of B. olmeca salivary proteins and their relationship to salivary proteins from other sand fly species. A cDNA library of the salivary glands of wild-caught B. olmeca was constructed, sequenced, and analyzed. We identified transcripts encoding for novel salivary proteins from this sand fly species and performed a comparative analysis between B. olmeca salivary proteins and those from other sand fly species. With this new information we present an updated catalog of the salivary proteins specific to New World sand flies and salivary proteins common to all sand fly species. We also report in this work the anti-Factor Xa activity of Lofaxin, a salivary anticoagulant protein present in this sand fly species. This study provides information on the first transcriptome of a sand fly from Mesoamerica and adds information to the limited repertoire of salivary transcriptomes from the Americas. This comparative analysis also shows a fast degree of evolution in salivary proteins from New World sand flies as compared with Old World sand flies.
ABSTRACT We evaluated the use of polymerase chain reaction (PCR) for diagnosis of American cutane... more ABSTRACT We evaluated the use of polymerase chain reaction (PCR) for diagnosis of American cutaneous leishmaniasis (ACL) in an area in Bahia, Brazil, where Leishmania braziliensis is endemic. Leishmania DNA was detected in 50 cases, yielding a positivity rate of 100%, which was higher than the rates for all of the other diagnostic methods studied—namely, the Montenegro skin test, anti-Leishmania serological testing, and microscopic examination of lesion biopsy specimens. These findings have led us to propose guidelines for the diagnosis of ACL that use PCR as the principal means of parasitological confirmation of cases. Leishmania, a protozoan parasite, is the cause of leish-maniasis, a human disease with diverse clinical mani-festations. An estimated 12 million people are currently infected with Leishmania species, whereas another 350 million people live at risk of infection [1]. American cutaneous leishmaniasis (ACL) is characterized by a cutaneous ulcer with elevated borders and a sharp cra-ter, which, when caused by parasites of the Leishmania braziliensis complex, may spontaneously heal, dissem-inate, or metastasize to the nasopharyngeal tract. When metastasis occurs, severe disfigurement and secondary complications may also occur, with a fatal outcome [2]. Epidemiological, clinical and pathological aspects of an unreported plant poisoning are described... more Epidemiological, clinical and pathological aspects of an unreported plant poisoning are described in sheep from southern Brazil. The disease occurred during the summer of 2003/2004 in 173 crossbred sheep with morbidity, mortality and lethality rates respectively of 31.8%, 8.1%, and 25.5%. Affected sheep were reluctant to move; when forced to walk they had stiff uncoordinated gaits, dragged their toes and bumped into objects in their path. When standing they assumed a wide base position with the legs widely spread apart. Increased respiratory rates, laborious breathing and cyanosis were marked in affected sheep that died. The experimental feeding of 2 sheep with the fruits of Eythroxylum argentinum reproduced the clinical disease. Except for consistent findings of distended bladder and the presence of E. argentinum undigested seeds in the abomasum and rumen in 4 necropsied sheep, no significant necropsy and histopathological findings were found. It was concluded that the ingestion of fruits of E. argentinum was the cause of this disease in sheep.

The clinical course lasted a few hours and was characterized by apathy and staggering disorders. The main postmortem findings occurred in livers as enhanced lobular patterns and hepatocellular necrosis that ranged from centrilobular to massive. An estimated 12 million people are currently infected with Leishmania species, whereas another 350 million people live at risk of infection [1]. American cutaneous leishmaniasis (ACL) is characterized by a cutaneous ulcer with elevated borders and a sharp cra-ter, which, when caused by parasites of the Leishmania braziliensis complex, may spontaneously heal, dissem-inate, or metastasize to the nasopharyngeal tract. When metastasis occurs, severe disfigurement and secondary complications may also occur, with a fatal outcome [2]. Epidemiological, clinical and pathological aspects of an unreported plant poisoning are described... more Epidemiological, clinical and pathological aspects of an unreported plant poisoning are described in sheep from southern Brazil. The disease occurred during the summer of 2003/2004 in 173 crossbred sheep with morbidity, mortality and lethality rates respectively of 31.8%, 8.1%, and 25.5%. Affected sheep were reluctant to move; when forced to walk they had stiff uncoordinated gaits, dragged their toes and bumped into objects in their path. When standing they assumed a wide base position with the legs widely spread apart. Increased respiratory rates, laborious breathing and cyanosis were marked in affected sheep that died. The experimental feeding of 2 sheep with the fruits of Eythroxylum argentinum reproduced the clinical disease. Except for consistent findings of distended bladder and the presence of E. argentinum undigested seeds in the abomasum and rumen in 4 necropsied sheep, no significant necropsy and histopathological findings were found. It was concluded that the ingestion of fruits of E. argentinum was the cause of this disease in sheep.

The clinical course lasted a few hours and was characterized by apathy and staggering disorders. The main postmortem findings occurred in livers as enhanced lobular patterns and hepatocellular necrosis that ranged from centrilobular to massive.
