This study aimed to address the polyps’ detection rate and their prevalence. In addition, we analyzed some related variables among the patients referred to Afzalipour and Mehregan Hospitals of Kerman in 2015-2016. Methods: Data concerning colonoscopy and pathologic samples of patients aged over 40 years who referred for colonoscopy were collected and analyzed. The polyps’ detection rate and some related variables were assessed. Results: A total of 469 patients older than 40 who underwent colonoscopy were enrolled in this study. One hundred and two cases of polyps were found in which 45.3% of them had adenoma. The bowel preparation (0.03), higher age (0.007) and male gender (0.013) had significant relationship with the detection of polyps. Therefore, it was decided to conduct a study to evaluate the rs3746444 polymorphism of miR-499 in patients with colon cancer in comparison with healthy subjects. Methods: This case-control study was conducted to investigate rs3746444 polymorphism of miR-499 in blood samples of case and control groups. Patients with a confirmed diagnosis of cancer based on pathologic report were enrolled in the study as the case group and compared with healthy subjects. The level of significance was considered at p< 0.05. Result: The mean of DNA count in samples was 63.17± 23.51 that was significantly higher in the case group. Evidence from different practices intensely support this hypothesis and demonstrate that a common therapy for these pernicious disorders might be possible. However, H syndrome is related to the recessive mutations in SLC29A3, encoding the equilibrative nucleoside transporter Equilibrative nucleoside transporter 3 (hENT3) which is expressed in mitochondria. The aim of this study is to investigate the misfolding and aggregation of ENT3 protein regarding in H syndrome, which can be a potential target for therapeutic interference in this disorder. Therefore, the 3D structure of the ENT3 protein was assumed using the I-TASSER online server and the best-predicted structure with the maximum confidence score (C-Score) was selected. The reliability and quality of the structure were evaluated by Z-score. However, the structural model of the selected mutant was accomplished using the I-TASSER server. The comparison with the corresponding of non-mutated structure and structural difference between mutant and wild-type structures was demonstrated by PyMOL. In the present study, computational models combined with experimental approaches provide new insights into the ENT3 which could lead to expanding new classes of nucleobase analogs for treatment and quintessential therapeutic strategies for H syndrome and some other diseases like cancer and viral infection.

BACKGROUND Many studies have reported an association between periodontal infections and some syst... more BACKGROUND Many studies have reported an association between periodontal infections and some systemic diseases such as respiratory and cardiovascular diseases. Some studies found a direct association between chronic gastritis caused by Helicobacter pylori (HP) infection and poor periodontal health. METHODS In a cross-sectional study from November 2014 to December 2015 in Kerman, the largest province in southeast Iran, patients with dyspepsia who were candidate for diagnostic upper gastrointestinal (GI) endoscopy were included in our study. Decayed, Missing, and Filled Teeth (DMFT) index and Loe plaque index that are two popular indexes in dental epidemiology were used to assess the oral health by a dentist before the upper GI endoscopy. According to the Loe plaque index, score: 0= no plaque, score: 1= a film of plaque attaching to the free gingival border and near area of the tooth, score: 2= moderate reposition of deposits within the gingival pocket, score :3= plenty of soft matter within the gingival pocket±on the tooth and gingival border. Scores ≤1, 2 and 3 equal to good, moderate, and poor oral hygiene, respectively. During upper GI endoscopy a total of six biopsy samples were taken from fundus, body, and antrum. A pathologist reported these samples according to Sidney's classification into superficial gastritis, atrophic gastritis, intestinal metaplasia, and dysplasia. RESULTS According to Sidney's classification 77 (89.5%) patients had superficial gastritis, 3 (3.5%) had atrophic gastritis, and 6 (7%) had intestinal metaplasia. HP was found in 80.2% of the gastric mucosal biopsy samples. There were not statistically significant relationship between Sidney's classification, presence of HP in gastric mucosal biopsies, and hygiene indicators (p>0.05). No relation was found between the DMFT index and superficial gastritis, atrophic gastritis, and intestinal metaplasia (p>0.05). Gastric infection with HP was found in 70%, 75%, and 100% of patients with mild, moderate, and sever DMFT index, respectively. CONCLUSION Our study showed that there might be a relation between poor oral hygiene and gastric precancerous lesions. In addition, HP infection in gastric histopathology might be associated with periodontal disease.

We assessed the inter- and intra-observer variability of the Los Angeles classification. Still images (n = 254) of the lower esophagus were presented to 9 gastroenterologists (6 experts and 3 trainees) and they were asked to report the images according to the Los Angeles classification. After 2 weeks the images were reordered and they were asked to report them again. Kappa statistic was calculated for intra- and inter-observer variability. The kappa for intra-observer agreement was 0.54 (attendings: 0.54; trainees; 0.55; P = not significant) and the inter-observer agreement was 0.22 (attendings: 0.20; trainees: 0.31; P = 0.027). Some of medicinal plants traditionally have been used for gastrointestinal problems. In the present work, the inhibitory effect of the essential oils of some medicinal plants was evaluated against clinical isolate of H. pylori. H. pylori was isolated from gastric biopsy of patients with gastric complications. Agar diffusion and agar dilution methods were used for evaluating the anti-H. pylori effect and minimum inhibitory concentration (MIC) determination of tested plants. The results were reported as mean±SD and differences considered significant at a P value <0.05. The essential oils of Cinnamomum zeylanicum and Zataria multiflora demonstrated potent anti-H. pylori effect with inhibition zone diameter of 24.8 mm and 23.6 mm, respectively. The MIC of both two essential oils was estimated to be 0.3 µl/ml. The essential oils of Heracleum persicum, Syzygium aromaticum and Citrus aurantium exhibited more than 88% inhibition in concentration of 0.3 µl/ml. The essential oils of C. zeylanicum and Z. multiflora might be good candidate for treatment of gastrointestinal disorders caused by H. pylori and it is needed to do further study about these essential oils.

ABSTRACT Background &amp;amp; Objective: Evaluating research indicators amongst clinical and ... more ABSTRACT Background &amp;amp; Objective: Evaluating research indicators amongst clinical and basic science faculty members reveals that such indicators are lower in clinical faculty members. Hence, we decided to assess the effect of providing education-research services, by founding a clinical research development center, on the research performance of clinical faculty members of Kerman University of Medical Sciences. Methods: In this intervention field study, 66 faculty members of Afzalipour teaching hospital and 45 ones in Shafa teaching hospital were recruited as cases and controls respectively. After founding the research center in cases’ work place, education-research services were provided for two years. The research activities of cases and controls were compared two years before and two years after founding the center. Results: 39 and 33 percent increase were observed in the cases’ total and per capita number of published articles respectively; similar indicators showed 21% and 21.6% decrease in the controls accordingly. The differences between two groups regarding both indicators were significant (P&amp;lt;0.01). Indicator of tutoring in dissertations and the total number and per capita number of presenting abstracts in the congresses showed a higher increase among cases. An increase in the total number and per capita number of accepted research proposals was 76% and 68% in cases and 120% and 122% in controls, respectively. Conclusion: Founding a clinical research center and providing education-research services in clinical faculty members’ workplace has positively influenced their performance and promoted their research indicators. Hence, establishing similar centers in other teaching hospitals is recommended.

Problem statement: Helicobacter pylori (H. pylori) is a microaerophilic, gram negative bacillus, ... more Problem statement: Helicobacter pylori (H. pylori) is a microaerophilic, gram negative bacillus, which can cause peptic ulcer and gastric cancer. Recurrent infection with this agent is considered as one of the reasons for failure of peptic ulcer treatment. Some studies have reported colonization of H. pylori in dental plaques, tonsils and adenoid tissues. Therefore oral cavity could be the source of H. pylori and it might be the reason for unsuccessful eradication. Approach: In this cross sectional study, 95 patients with the average age of 12.1±7.5 years undergoing adenotonsillectomy were chosen consecutively. Intra-operatively a 2 mm section of the tonsils were removed and investigated for H. pylori by Rapid Urease Test (RUT). Post-operatively the removed tonsils were stained by hematoxylin-eosin (H and E) and Gimsa for direct investigation of H. pylori bacterium. Serum samples of the patients were also tested for the presence of H. pylori IgG antibody. Results: Overall 70 patients (73.7%) had positive anti-H. pylori IgG antibody in their sera. The results of RUT on adeno-tonsils showed that 42.1% of the specimens were positive for H. pylori. In histology examination, 9 patients (9.5%) were positive for the presence of bacterium. Conclusion: Based on our findings it seems that tonsils and adenoid tissues are the candidate places for the growth of H. pylori. Further studies about the role of tonsillar colonization of H. pylori in re-infection after treatment are recommended.

This study aimed to evaluate the distribution of Vit D3 deficiency in individuals with non-alcoholic fatty liver disease. In this cross-sectional study, 122 individuals with nonalcoholic fatty liver disease were selected by a simple sampling method. After collecting demographic data, serum Vit 25(OH) D3 level was measured by the ELFA method. Blood lipids level (TG, cholesterol, HDL, LDL), FBS, AST, ALT, alkaline phosphatase, total and direct bilirubin, albumin, and PT were measured by the enzymatic method. To analyze the data, descriptive and analytical methods and SPSS software version 16 were used. The study cases are comprised of 122 individuals (57.4% male). Background: Chronic constipation is a common problem worldwide that negatively impacts quality of... more Background: Chronic constipation is a common problem worldwide that negatively impacts quality of life. Because of the effects of psychological and cultural factors on the function of the bowel tract and the vast variety of ethnicities and diet among the Iranian people, the present study describes the occurrence of constipation and its associated factors in Kerman, the largest city in Southeast Iran. Materials and Methods: This was a cross-sectional study conducted in Kerman, Iran from 2011 to 2012 that included 2191 adults who were randomly selected and interviewed face to face using a validated questionnaire based on Rome III criteria. Demographics, cigarette smoking, opium dependence, amount of fiber consumption, physical activity and psychological factors were assessed. Inclusion criteria was ages 15 to 85 years old. Exclusion criteria was the presence of any metabolic and/ or neuro-muscular disease that caused constipation. All data were analyzed with Stata11 software, with a confidence interval of 95%. A p-value of ≤0.05 was chosen for statistical significance. Results: Particiants had a mean age of 44.9 ± 16.2 years. There were 57.42% female participants. The prevalance of chronic constipation was estimated at 9.4% (7.7%-10.6%). Constipation was more prevalent in females, older individuals, those with decreased physical activity, people who were anxious and depressed, and opium addicted individuals (p<0.05). There was no significant difference in terms of cigarette smoking and amount of dietary fiber between constipated and non-constipated individuals (p>0.2). Conclusion: Although the prevalance of constipation in Iran is estimated to be lower than Western countries, new changes in life style can cause an increase in the burden of constipation in the future.

The aim of this study is to determine the incidence rate of IBD in Kerman, a city in Southeast Iran. METHODS All medical records that indicated a new diagnosis of ulcerative colitis (UC) or Crohn's disease (CD) were retrieved from the gastrointestinal endoscopy and pathology departments of 12 centers from October 2011 to September 2012. RESULTS The incidence rate of UC in Kerman was estimated at 4.98/100,000 (95% CI: 2.44-6.94). The mean age of patients was 39.4 years and male to female ratio was 0.89:1.0. UC patients had the following prevalence: cigarette smoking (30%), appendectomy (36%) and oral contraceptive (OCP) use (78.9%). The incidence rate of CD was 0.8/100,000 (95% CI: 0.6 4-1.14). So, the aim of this study was to assess the association between SIBO and IBS by using glucose breath test (GBT) in Kerman city as the first study in Iranian population. METHODS 107 patients with IBS and 107 healthy individuals were enrolled in our study. All the participants underwent GBT. A peak of H2 values >20 p.p.m above the basal value after glucose ingestion was considered suggestive of SIBO. SPSS software version 17 was used for data analysis. P value < 0.05 was considered as statistically significant. RESULTS Of the 107 patients with IBS, 40 had positive GBT (37.4%) compared with 14 (12.1%) out of the 107 control participants(p< 0.001). Patients with a confirmed diagnosis of cancer based on pathologic report were enrolled in the study as the case group and compared with healthy subjects. Evidence from different practices intensely support this hypothesis and demonstrate that a common therapy for these pernicious disorders might be possible. However, H syndrome is related to the recessive mutations in SLC29A3, encoding the equilibrative nucleoside transporter Equilibrative nucleoside transporter 3 (hENT3) which is expressed in mitochondria. The aim of this study is to investigate the misfolding and aggregation of ENT3 protein regarding in H syndrome, which can be a potential target for therapeutic interference in this disorder. Therefore, the 3D structure of the ENT3 protein was assumed using the I-TASSER online server and the best-predicted structure with the maximum confidence score (C-Score) was selected. The reliability and quality of the structure were evaluated by Z-score. However, the structural model of the selected mutant was accomplished using the I-TASSER server. The comparison with the corresponding of non-mutated structure and structural difference between mutant and wild-type structures was demonstrated by PyMOL. In the present study, computational models combined with experimental approaches provide new insights into the ENT3 which could lead to expanding new classes of nucleobase analogs for treatment and quintessential therapeutic strategies for H syndrome and some other diseases like cancer and viral infection.

BACKGROUND Many studies have reported an association between periodontal infections and some syst... more BACKGROUND Many studies have reported an association between periodontal infections and some systemic diseases such as respiratory and cardiovascular diseases. Some studies found a direct association between chronic gastritis caused by Helicobacter pylori (HP) infection and poor periodontal health. METHODS In a cross-sectional study from November 2014 to December 2015 in Kerman, the largest province in southeast Iran, patients with dyspepsia who were candidate for diagnostic upper gastrointestinal (GI) endoscopy were included in our study. Decayed, Missing, and Filled Teeth (DMFT) index and Loe plaque index that are two popular indexes in dental epidemiology were used to assess the oral health by a dentist before the upper GI endoscopy. According to the Loe plaque index, score: 0= no plaque, score: 1= a film of plaque attaching to the free gingival border and near area of the tooth, score: 2= moderate reposition of deposits within the gingival pocket, score :3= plenty of soft matter within the gingival pocket±on the tooth and gingival border. Scores ≤1, 2 and 3 equal to good, moderate, and poor oral hygiene, respectively. During upper GI endoscopy a total of six biopsy samples were taken from fundus, body, and antrum. A pathologist reported these samples according to Sidney's classification into superficial gastritis, atrophic gastritis, intestinal metaplasia, and dysplasia. RESULTS According to Sidney's classification 77 (89.5%) patients had superficial gastritis, 3 (3.5%) had atrophic gastritis, and 6 (7%) had intestinal metaplasia. HP was found in 80.2% of the gastric mucosal biopsy samples. There were not statistically significant relationship between Sidney's classification, presence of HP in gastric mucosal biopsies, and hygiene indicators (p>0.05). No relation was found between the DMFT index and superficial gastritis, atrophic gastritis, and intestinal metaplasia (p>0.05). Gastric infection with HP was found in 70%, 75%, and 100% of patients with mild, moderate, and sever DMFT index, respectively. CONCLUSION Our study showed that there might be a relation between poor oral hygiene and gastric precancerous lesions. In addition, HP infection in gastric histopathology might be associated with periodontal disease.

We assessed the inter- and intra-observer variability of the Los Angeles classification. Still images (n = 254) of the lower esophagus were presented to 9 gastroenterologists (6 experts and 3 trainees) and they were asked to report the images according to the Los Angeles classification. After 2 weeks the images were reordered and they were asked to report them again. Kappa statistic was calculated for intra- and inter-observer variability. The kappa for intra-observer agreement was 0.54 (attendings: 0.54; trainees; 0.55; P = not significant) and the inter-observer agreement was 0.22 (attendings: 0.20; trainees: 0.31; P = 0.027). Some of medicinal plants traditionally have been used for gastrointestinal problems. In the present work, the inhibitory effect of the essential oils of some medicinal plants was evaluated against clinical isolate of H. pylori. H. pylori was isolated from gastric biopsy of patients with gastric complications. Agar diffusion and agar dilution methods were used for evaluating the anti-H. pylori effect and minimum inhibitory concentration (MIC) determination of tested plants. The results were reported as mean±SD and differences considered significant at a P value <0.05. The essential oils of Cinnamomum zeylanicum and Zataria multiflora demonstrated potent anti-H. pylori effect with inhibition zone diameter of 24.8 mm and 23.6 mm, respectively. The MIC of both two essential oils was estimated to be 0.3 µl/ml. The essential oils of Heracleum persicum, Syzygium aromaticum and Citrus aurantium exhibited more than 88% inhibition in concentration of 0.3 µl/ml. The essential oils of C. zeylanicum and Z. multiflora might be good candidate for treatment of gastrointestinal disorders caused by H. pylori and it is needed to do further study about these essential oils.

ABSTRACT Background &amp;amp; Objective: Evaluating research indicators amongst clinical and ... more ABSTRACT Background &amp;amp; Objective: Evaluating research indicators amongst clinical and basic science faculty members reveals that such indicators are lower in clinical faculty members. Hence, we decided to assess the effect of providing education-research services, by founding a clinical research development center, on the research performance of clinical faculty members of Kerman University of Medical Sciences. Methods: In this intervention field study, 66 faculty members of Afzalipour teaching hospital and 45 ones in Shafa teaching hospital were recruited as cases and controls respectively. After founding the research center in cases’ work place, education-research services were provided for two years. The research activities of cases and controls were compared two years before and two years after founding the center. Results: 39 and 33 percent increase were observed in the cases’ total and per capita number of published articles respectively; similar indicators showed 21% and 21.6% decrease in the controls accordingly. The differences between two groups regarding both indicators were significant (P&amp;lt;0.01). Indicator of tutoring in dissertations and the total number and per capita number of presenting abstracts in the congresses showed a higher increase among cases. An increase in the total number and per capita number of accepted research proposals was 76% and 68% in cases and 120% and 122% in controls, respectively. Conclusion: Founding a clinical research center and providing education-research services in clinical faculty members’ workplace has positively influenced their performance and promoted their research indicators. Hence, establishing similar centers in other teaching hospitals is recommended.

Recurrent infection with this agent is considered as one of the reasons for failure of peptic ulcer treatment. Some studies have reported colonization of H. pylori in dental plaques, tonsils and adenoid tissues. Therefore oral cavity could be the source of H. pylori and it might be the reason for unsuccessful eradication. Approach: In this cross sectional study, 95 patients with the average age of 12.1±7.5 years undergoing adenotonsillectomy were chosen consecutively. Intra-operatively a 2 mm section of the tonsils were removed and investigated for H. pylori by Rapid Urease Test (RUT). Post-operatively the removed tonsils were stained by hematoxylin-eosin (H and E) and Gimsa for direct investigation of H. pylori bacterium. Serum samples of the patients were also tested for the presence of H. pylori IgG antibody. Results: Overall 70 patients (73.7%) had positive anti-H. pylori IgG antibody in their sera. The results of RUT on adeno-tonsils showed that 42.1% of the specimens were positive for H. pylori. In histology examination, 9 patients (9.5%) were positive for the presence of bacterium. Conclusion: Based on our findings it seems that tonsils and adenoid tissues are the candidate places for the growth of H. pylori. Further studies about the role of tonsillar colonization of H. pylori in re-infection after treatment are recommended.

In this cross-sectional study, 122 individuals with nonalcoholic fatty liver disease were selected by a simple sampling method. After collecting demographic data, serum Vit 25(OH) D3 level was measured by the ELFA method. Blood lipids level (TG, cholesterol, HDL, LDL), FBS, AST, ALT, alkaline phosphatase, total and direct bilirubin, albumin, and PT were measured by the enzymatic method. Because of the effects of psychological and cultural factors on the function of the bowel tract and the vast variety of ethnicities and diet among the Iranian people, the present study describes the occurrence of constipation and its associated factors in Kerman, the largest city in Southeast Iran. Materials and Methods: This was a cross-sectional study conducted in Kerman, Iran from 2011 to 2012 that included 2191 adults who were randomly selected and interviewed face to face using a validated questionnaire based on Rome III criteria. Demographics, cigarette smoking, opium dependence, amount of fiber consumption, physical activity and psychological factors were assessed. Inclusion criteria was ages 15 to 85 years old. Exclusion criteria was the presence of any metabolic and/ or neuro-muscular disease that caused constipation. All data were analyzed with Stata11 software, with a confidence interval of 95%. A p-value of ≤0.05 was chosen for statistical significance. Results: Particiants had a mean age of 44.9 ± 16.2 years. There were 57.42% female participants. The prevalance of chronic constipation was estimated at 9.4% (7.7%-10.6%). Constipation was more prevalent in females, older individuals, those with decreased physical activity, people who were anxious and depressed, and opium addicted individuals (p<0.05). There was no significant difference in terms of cigarette smoking and amount of dietary fiber between constipated and non-constipated individuals (p>0.2). Conclusion: Although the prevalance of constipation in Iran is estimated to be lower than Western countries, new changes in life style can cause an increase in the burden of constipation in the future.

The aim of this study is to determine the incidence rate of IBD in Kerman, a city in Southeast Iran. METHODS All medical records that indicated a new diagnosis of ulcerative colitis (UC) or Crohn's disease (CD) were retrieved from the gastrointestinal endoscopy and pathology departments of 12 centers from October 2011 to September 2012. RESULTS The incidence rate of UC in Kerman was estimated at 4.98/100,000 (95% CI: 2.44-6.94). The mean age of patients was 39.4 years and male to female ratio was 0.89:1.0. So, the aim of this study was to assess the association between SIBO and IBS by using glucose breath test (GBT) in Kerman city as the first study in Iranian population. METHODS 107 patients with IBS and 107 healthy individuals were enrolled in our study. All the participants underwent GBT. A peak of H2 values >20 p.p.m above the basal value after glucose ingestion was considered suggestive of SIBO. SPSS software version 17 was used for data analysis. P value < 0.05 was considered as statistically significant. RESULTS Of the 107 patients with IBS, 40 had positive GBT (37.4%) compared with 14 (12.1%) out of the 107 control participants(p< 0.001).
