Objective: Determination of the effectiveness of periarticular 5% dextrose prolotherapy in the tr... more Objective: Determination of the effectiveness of periarticular 5% dextrose prolotherapy in the treatment of knee pain due to osteoarthritis. Material and Methods: The clinical results of prolotherapy with 5% periarticular dextrose in adult female and male patients with knee pain and complaints related to osteoarthritis were evaluated retrospectively. During the examination, we identified the sensitive ligaments in the knee and injected 1 cc of 5% dextrose with a 0.6 * 60 mm sterican needle. In routine practice, stretching exercises are recommended to patients after prolotherapy and for analgesia, a combined analgesic containing paracetemol 500mg + codeine phosphate 10mg + caffeine 30mg is given for the first 3 days. In addition to the initial application, prolotherapy is performed 3 times with an interval of 20 days, and when the Visual Analogue Scale (VAS) score decreases by 80% or more after the first or second injection, prolotherapy is terminated. Results: 30 patients aged between 36-70 years were included in the study. Patients' mean age was 52,9±9,0 years. Nineteen patients received 3 sessions and 11 patients received 2 sessions of prolotherapy. A significant decrease was achieved in VAS scores of all patients except one patient. The VAS score was 2.9 at the end of the third session meanwhile the initial VAS average was 8.4. None of the patients had any side effects that would terminate the treatment. Conclusion: The data obtained in the study show that prolotherapy with periarticular 5% dextrose is effective in the treatment of knee pain and complaints related to osteoarthritis. However, more comprehensive studies will be guiding the clarification of its place in the osteoarthritis' treatment.

Retrorektal kistler gelişimsel kistler olup, kökenlerine ve histopatolojik özelliklerine göre; de... more Retrorektal kistler gelişimsel kistler olup, kökenlerine ve histopatolojik özelliklerine göre; dermoid kist, rektal duplikasyon kistleri ve kistik hamartomlar (Tailgut kistleri) olarak sınıflandırılabilirler. Dermoid kistler çoğunlukla overian yerleşim göstermekle beraber, mediastende, retroperitoneal bölgede ve nadiren retrorektal lokalizasyonda da görülebilmektedirler. Retrorektal kistler genellikle asemptomatik olsalar da rektal dolgunluk, defekasyon düzensizlikleri, karın ağrısı ve disparoni gibi şikayetlere neden olabilirler. Çoğunlukla rutin pelvik muayene ve görüntüleme yöntemleri ile rastlantısal olarak tanınırlar. Genellikle benign lezyonlar olmalarına rağmen malign transformasyon ve enfeksiyon gelişme riskleri nedeni ile tanı konulduğunda cerrahi olarak çıkarılmaları gerekmektedir. Bu yazıda erişkin bayan hastada, rutin pelvik muayene esnasında fark edilen ve cerrahi olarak çıkarılan retrorektal dermoid kistin tanı ve tedavisi irdelenmiştir.

AIM: Extended rapid virologic response (eRVR) predicts sustained virologic response (SVR) and pro... more AIM: Extended rapid virologic response (eRVR) predicts sustained virologic response (SVR) and provides to determine shortening response guided treatment duration. P2/MS<45, an effective and noninvasive marker reflecting the degree of hepatic fibrosis, demonstrates serious hepatic fibrosis. In a retrospective observational study, we studied the relationship between P2/MS and eRVR among HCV genotype 1 (G1) infected patients who were treated with triple therapy patients. P2/MS was evaluated whether it is a predictor for treatment response by foreseeing eRVR. METHOD: Thirty one patients infected with HCV-G1and were treated with triple therapy involving telaprevir, pegylated interferon-α and ribavirin, were enrolled in the study. P2/MS values were calculated before the treatment. HCV-RNA levels were measured at the initiation period, the 4 th and 12 th weeks. RESULTS: Nine patients of 31 had detectable HCV-RNA in the 4 th week. Among 6 of these 9 patients had P2/MS<45. HCV-RNA was undetectable in 22 patients at the 4 th week of the treatment. Sixteen of them had P2/MS>45. Among 30 of 31 patients, HCV-RNA was undetectable at the 12 th week of the treatment. We found that eRVR success was 85% in patients with P2/MS score>45 and 45.4% in patients with P2/MS<45. CONCLUSION: P2/MS is a noninvasive marker for detecting hepatic fibrosis in HCV patients. A positive correlation between P2/ MS score and eRVR is demonstrated. We suggest that P2/MS should be used as a pretreatment predictive marker for HCV-G1 infected patients, estimating SVR and the patients suitable for shortening therapy.

Elevated alpha-fetoprotein levels in hepatocellular carcinoma is related with recurrence. We aimed to examine the efficacy of P2/MS and alpha-fetoproteinin early prediction of hepatocellular carcinoma recurrence and to determine a cutoff value for P2/MS and alpha-fetoprotein. Methods: Sixty nine patients with hepatocellular carcinoma were retrospectively scanned for recurrence existence. The recurrence of hepatocellular carcinoma was diagnosed radiologically, via 3-phase tomography and magnetic resonance investigation. The formula of P2/MS was platelet count (10 9 /L) 2 /[monocyte count (%)x neutrophil count(%)]. Results: Forty two patients had hepatocellular carcinoma recurrence. For the recurrence prediction, the cutoff value of P2/MS was ≤59.9 with 100% specificity and 95.24% sensitivity. The cutoff value of alpha-fetoprotein was found >57.5 with 83.3% sensitivity and 92.6% specificity. Conclusions: With these findings, P2/MS and alpha-fetoprotein seems as reliable markers for the prediction of hepatocellular carcinoma recurrence. We suggest that they can be used as non-invasive tools for the early estimation of hepatocellular carcinoma recurrence.

BACKGROUND: In early breast cancer patients, the effects of hormonal therapy (tamoxifen and aroma... more BACKGROUND: In early breast cancer patients, the effects of hormonal therapy (tamoxifen and aromatase inhibitors) on plasma fibroblast growth factor 21 (FGF-21), lipid levels and body composition have not yet been investigated. Therefore, we aimed to analyze the relationship between FGF-21 and body composition as well as the effects of tamoxifen and aromatase inhibitors on plasma lipid levels, FGF-21, and body composition. METHODS: A total of 72 patients were treated with either tamoxifen or aromatase inhibitors due to their menopausal status after adjuvant radiotherapy. Each patient was followed-up over a period of 1 year. Changes in body composition and serum lipid profile, glucose and FGF-21 levels were evaluated. We recorded the type of hormonal therapy, body mass index, waist-to-hip ratio, lipid profile, and FGF-21 levels both at the beginning and after 12 months. RESULTS: There was a statistically significant decrease in serum FGF-21 levels after 12 months of adjuvant endocrine therapy (46 ± 19.21 pg/ml vs. 30.99 ± 13.81 pg/ml, p < 0.001). Total body water (p < 0.001), serum glucose (p = 0.036) and triglyceride levels (p < 0.001) also exhibited a significant decrease. The decreases in total cholesterol and low-density lipoprotein were not statistically significant. Likewise, high-density lipoprotein increased after adjuvant endocrine therapy, although it did not reach statistical significance. The changes in body composition, glucose, lipid profile and FGF-21 were similar in tamoxifen and aromatase inhibitor groups. A positive correlation was found between basal weight, fat mass, fat-free mass and serum FGF-21 levels; however, the correlation was maintained only for the fat-free mass at the 12 th month. CONCLUSION: As part of the present study, we suggest that both tamoxifen and aromatase inhibitors can reduce FGF-21 levels independently of body compositions, and these drugs can provide antihyperlipidemic, antidiabetic and cardio-protective effects. We also recommend that serum FGF-21 level can be utilized as a tumor biomarker in early-stage breast cancer and for monitoring purposes. FGF-21 levels may help physicians estimate prognosis, too. Further studies with larger populations may shed light on the role of FGF-21 in breast cancer.

Therefore, we aimed to analyze the relationship between adipocytokines and body composition as well as the effects of TMX and AIs on plasma adiponectin, leptin, leptin/adiponectin ratio (LAR) and body composition. Patients were treated with either TMX or AI according to their menopausal status after adjuvant radiotherapy. Changes in body composition and serum leptin and adiponectin levels were evaluated. We recorded the type of hormonal therapy, BMI, waist/hip ratio (WHR), leptin and adiponectin levels at study entry, and after 6 and 12 months. Half of the cases had a familiar history of idiopathic psoriasis: these were the most severe and invalidating forms and required definitive interruption of the therapy. Conclusions: This study shows that long-term treatment with IFX and ADA was safe and well tolerated in most cases. Considering the remarkable benefits of this new form of medical therapy for IBD in pediatric age, the decision to use an anti-TNFalpha agent should be individualized and based upon specific clinical features and unique risk profile of a given patient.

Background: Minor salivary gland biopsy from the lip region is commonly used for primary Sjögren'... more Background: Minor salivary gland biopsy from the lip region is commonly used for primary Sjögren's syndrome (SS) diagnosis. It is accepted that changes of histopathology is the most important component of diagnostic criteria. In this study, we analyzed the relationship between histopathological changes and laboratory findings/clinical features in SS patients and control group. Objectives: The aim of this study was to determine the histopathological changes in minor salivary tissue for SS diagnosis Methods: A total of 69 minor salivary gland biopsies (29 Sjögren's syndrome and 40 controls) were included in the study. Biopsies were evaluated by a blind pathologist unaware of the diagnosis. Histopathological findings were noted as ductal dilatation, fat tissue percentage (<10% or >10%), acinar atrophy, ductal/ acinar ratio and lobules/foci ratio. It was evaluated whether there was a relationship between histological changes and age, gender, ANA, anti-SSA, anti-SSB, RF and Chisholm scores. Results: The lobules/foci ratio of patients with Sjögren's syndrome was significantly higher than the control group (p<0.001). The cutoff value of the lobules/foci ratio for SS diagnosis was 0.8. Furthermore, there was a significant correlations between the lobules/foci ratio greater than 0.8 and antibodies (ANA, RF, anti-Ro) positivity and Chisholm score (p<0.01). Other histopathological findings such as ductal dilatation, fat tissue percentage, acinar atrophy, and ductal/atherine ratio do not distinguish SS from non-SS. Conclusions: The results of this study revealed that only the lobules/foci ratio from histopathological changes has a strong relation with SS diagnosis. It can be important for pathologists to distinguish SS.

Aim: The outbreak of the novel coronavirus disease (COVID-19) has been affecting the world day by... more Aim: The outbreak of the novel coronavirus disease (COVID-19) has been affecting the world day by day. The definitive diagnosis of COVID-19 is made using the real-time polymerase chain reaction (RT-PCR) method. Although the RT-PCR method is the gold standard for confirming infection, it requires a specialized laboratory, expensive equipment, and trained personnel. Thus, simple and alternative laboratory tests are needed to predict PCR positivity and disease. Material and Methods: We analyzed laboratory parameters of 147 patients who were hospitalized with a pre-diagnosis of COVID-19 and sent RT-PCR samples. Of these, 76 tests were positive and 71 were negative. In addition to routine laboratory parameters, we also examined ratios derived from them, such as the De Ritis ratio (AST to ALT ratio) and Neutrophil-to-Lymphocyte Ratio (NLR). We investigated whether these parameters can predict the positivity or negativity of the PCR test. Results: CRP, D-dimer, Ferritin, AST values, NLR, and De Ritis ratio were found to be significantly higher in the PCR-positive group. It was observed that the lymphocyte count was lower in the positive group. Discussion: Our results suggest that routine laboratory tests that can be performed easily may also have the potential to predict COVID-19 positivity and negativity. These parameters can especially be used as an alternative in areas with a shortage of laboratories, equipment and personnel for PCR testing. Combining some hematological parameters and specific ratios derived from hematological parameters can help in identifying false positive/negative RT-PCR tests.

Abstract Ineffective esophageal motility (IEM), defined as minor esophageal motility disorder, is... more Abstract Ineffective esophageal motility (IEM), defined as minor esophageal motility disorder, is also the most common esophageal motility disorder. The relationship between gastro-esophageal reflux disease is still controversial. Our aim in this study is to evaluate whether there are differences in terms of demographic, endoscopic, or motility findings between IEM patients with pathological esophageal acid reflux and physiological reflux. Patients diagnosed with IEM according to the Chicago classification v3 with high-resolution manometry (HRM) before acid monitoring constituted the study group of our investigation. The patients were divided into 2 groups as patients with pathological esophageal reflux and patients with physiological reflux according to 24-hour acid monitoring. Demographic data, endoscopic findings, and HRM findings were compared between 2 groups. A total of 62 patients who were diagnosed with IEM according to the Chicago classification v3 were included in the study. Patients in the physiological reflux group were 7 years younger on average than the pathological reflux group. Esophagitis rates were significantly higher in the pathological reflux group (P = .033). Lower esophageal sphincter resting pressure, integrated relaxation pressure, and the presence of hernia were found to be similar in the 2 groups (P = 392, P = 182, P = 657, respectively). The rate of severe IEM was also similar between the 2 groups (P = .143). The fact that the physiological reflux patient group is younger may suggest that the IEM develops in the early period and then reflux accompanies the picture with advancing age.

One third of all extranodal lymphomas are located at the gastrointestinal tract, and 10-20% of ga... more One third of all extranodal lymphomas are located at the gastrointestinal tract, and 10-20% of gastrointestinal lymphomas are located in the colon. Primary colorectal lymphomas constitute 0.1-0.5% of all large bowel malignancies. The most common type of lymphoma is diffuse large B cell lymphoma. It is twice as common in males than females and generally is encountered in 6 th and 7 th decade of life. Abdominal pain and weight loss are the most common symptoms, and because these symptoms are non-specific, time of diagnosis is usually delayed. Isolated surgical therapy or chemotherapy can be employed, and combination treatment might be preferred. In this case report, we present a colonic non-Hodgkin lymphoma patient who was admitted with weight loss and abdominal pain. This case reports on a patient who was younger than what has generally been reported in the literature.

BACKGROUND: In early breast cancer patients, the effects of hormonal therapy (tamoxifen and aroma... more BACKGROUND: In early breast cancer patients, the effects of hormonal therapy (tamoxifen and aromatase inhibitors) on plasma fibroblast growth factor 21 (FGF-21), lipid levels and body composition have not yet been investigated. Therefore, we aimed to analyze the relationship between FGF-21 and body composition as well as the effects of tamoxifen and aromatase inhibitors on plasma lipid levels, FGF-21, and body composition. METHODS: A total of 72 patients were treated with either tamoxifen or aromatase inhibitors due to their menopausal status after adjuvant radiotherapy. Each patient was followed-up over a period of 1 year. Changes in body composition and serum lipid profile, glucose and FGF-21 levels were evaluated. We recorded the type of hormonal therapy, body mass index, waist-to-hip ratio, lipid profile, and FGF-21 levels both at the beginning and after 12 months. RESULTS: There was a statistically significant decrease in serum FGF-21 levels after 12 months of adjuvant endocrine therapy (46 ± 19.21 pg/ml vs. 30.99 ± 13.81 pg/ml, p < 0.001). Total body water (p < 0.001), serum glucose (p = 0.036) and triglyceride levels (p < 0.001) also exhibited a significant decrease. The decreases in total cholesterol and low-density lipoprotein were not statistically significant. Likewise, high-density lipoprotein increased after adjuvant endocrine therapy, although it did not reach statistical significance. The changes in body composition, glucose, lipid profile and FGF-21 were similar in tamoxifen and aromatase inhibitor groups. A positive correlation was found between basal weight, fat mass, fat-free mass and serum FGF-21 levels; however, the correlation was maintained only for the fat-free mass at the 12 th month. CONCLUSION: As part of the present study, we suggest that both tamoxifen and aromatase inhibitors can reduce FGF-21 levels independently of body compositions, and these drugs can provide antihyperlipidemic, antidiabetic and cardio-protective effects. We also recommend that serum FGF-21 level can be utilized as a tumor biomarker in early-stage breast cancer and for monitoring purposes. FGF-21 levels may help physicians estimate prognosis, too. Further studies with larger populations may shed light on the role of FGF-21 in breast cancer.

Half of the cases had a familiar history of idiopathic psoriasis: these were the most severe and invalidating forms and required definitive interruption of the therapy. Conclusions: This study shows that long-term treatment with IFX and ADA was safe and well tolerated in most cases. Considering the remarkable benefits of this new form of medical therapy for IBD in pediatric age, the decision to use an anti-TNFalpha agent should be individualized and based upon specific clinical features and unique risk profile of a given patient.

Background: Minor salivary gland biopsy from the lip region is commonly used for primary Sjögren'... more Background: Minor salivary gland biopsy from the lip region is commonly used for primary Sjögren's syndrome (SS) diagnosis. It is accepted that changes of histopathology is the most important component of diagnostic criteria. In this study, we analyzed the relationship between histopathological changes and laboratory findings/clinical features in SS patients and control group. Objectives: The aim of this study was to determine the histopathological changes in minor salivary tissue for SS diagnosis Methods: A total of 69 minor salivary gland biopsies (29 Sjögren's syndrome and 40 controls) were included in the study. Biopsies were evaluated by a blind pathologist unaware of the diagnosis. Histopathological findings were noted as ductal dilatation, fat tissue percentage (<10% or >10%), acinar atrophy, ductal/ acinar ratio and lobules/foci ratio. It was evaluated whether there was a relationship between histological changes and age, gender, ANA, anti-SSA, anti-SSB, RF and Chisholm scores. Results: The lobules/foci ratio of patients with Sjögren's syndrome was significantly higher than the control group (p<0.001). The cutoff value of the lobules/foci ratio for SS diagnosis was 0.8. Furthermore, there was a significant correlations between the lobules/foci ratio greater than 0.8 and antibodies (ANA, RF, anti-Ro) positivity and Chisholm score (p<0.01). Other histopathological findings such as ductal dilatation, fat tissue percentage, acinar atrophy, and ductal/atherine ratio do not distinguish SS from non-SS. Conclusions: The results of this study revealed that only the lobules/foci ratio from histopathological changes has a strong relation with SS diagnosis. It can be important for pathologists to distinguish SS.

The definitive diagnosis of COVID-19 is made using the real-time polymerase chain reaction (RT-PCR) method. Although the RT-PCR method is the gold standard for confirming infection, it requires a specialized laboratory, expensive equipment, and trained personnel. Thus, simple and alternative laboratory tests are needed to predict PCR positivity and disease. Material and Methods: We analyzed laboratory parameters of 147 patients who were hospitalized with a pre-diagnosis of COVID-19 and sent RT-PCR samples. Of these, 76 tests were positive and 71 were negative. In addition to routine laboratory parameters, we also examined ratios derived from them, such as the De Ritis ratio (AST to ALT ratio) and Neutrophil-to-Lymphocyte Ratio (NLR). We investigated whether these parameters can predict the positivity or negativity of the PCR test. Results: CRP, D-dimer, Ferritin, AST values, NLR, and De Ritis ratio were found to be significantly higher in the PCR-positive group. It was observed that the lymphocyte count was lower in the positive group. Discussion: Our results suggest that routine laboratory tests that can be performed easily may also have the potential to predict COVID-19 positivity and negativity. These parameters can especially be used as an alternative in areas with a shortage of laboratories, equipment and personnel for PCR testing. Combining some hematological parameters and specific ratios derived from hematological parameters can help in identifying false positive/negative RT-PCR tests.

P2/MS<45, an effective and noninvasive marker reflecting the degree of hepatic fibrosis, demonstrates serious hepatic fibrosis. In a retrospective observational study, we studied the relationship between P2/MS and eRVR among HCV genotype 1 (G1) infected patients who were treated with triple therapy patients. P2/MS was evaluated whether it is a predictor for treatment response by foreseeing eRVR. METHOD: Thirty one patients infected with HCV-G1and were treated with triple therapy involving telaprevir, pegylated interferon-α and ribavirin, were enrolled in the study. P2/MS values were calculated before the treatment. HCV-RNA levels were measured at the initiation period, the 4 th and 12 th weeks. RESULTS: Nine patients of 31 had detectable HCV-RNA in the 4 th week. Among 6 of these 9 patients had P2/MS<45. HCV-RNA was undetectable in 22 patients at the 4 th week of the treatment. Sixteen of them had P2/MS>45. Among 30 of 31 patients, HCV-RNA was undetectable at the 12 th week of the treatment. We found that eRVR success was 85% in patients with P2/MS score>45 and 45.4% in patients with P2/MS<45. CONCLUSION: P2/MS is a noninvasive marker for detecting hepatic fibrosis in HCV patients. A positive correlation between P2/ MS score and eRVR is demonstrated. We suggest that P2/MS should be used as a pretreatment predictive marker for HCV-G1 infected patients, estimating SVR and the patients suitable for shortening therapy.

The relationship between gastro-esophageal reflux disease is still controversial. Our aim in this study is to evaluate whether there are differences in terms of demographic, endoscopic, or motility findings between IEM patients with pathological esophageal acid reflux and physiological reflux. Patients diagnosed with IEM according to the Chicago classification v3 with high-resolution manometry (HRM) before acid monitoring constituted the study group of our investigation. The patients were divided into 2 groups as patients with pathological esophageal reflux and patients with physiological reflux according to 24-hour acid monitoring. Demographic data, endoscopic findings, and HRM findings were compared between 2 groups. A total of 62 patients who were diagnosed with IEM according to the Chicago classification v3 were included in the study. Patients in the physiological reflux group were 7 years younger on average than the pathological reflux group. Esophagitis rates were significantly higher in the pathological reflux group (P = .033). Lower esophageal sphincter resting pressure, integrated relaxation pressure, and the presence of hernia were found to be similar in the 2 groups (P = 392, P = 182, P = 657, respectively). The rate of severe IEM was also similar between the 2 groups (P = .143). The fact that the physiological reflux patient group is younger may suggest that the IEM develops in the early period and then reflux accompanies the picture with advancing age.

One third of all extranodal lymphomas are located at the gastrointestinal tract, and 10-20% of ga... more One third of all extranodal lymphomas are located at the gastrointestinal tract, and 10-20% of gastrointestinal lymphomas are located in the colon. Primary colorectal lymphomas constitute 0.1-0.5% of all large bowel malignancies. The most common type of lymphoma is diffuse large B cell lymphoma. It is twice as common in males than females and generally is encountered in 6 th and 7 th decade of life. Abdominal pain and weight loss are the most common symptoms, and because these symptoms are non-specific, time of diagnosis is usually delayed. Isolated surgical therapy or chemotherapy can be employed, and combination treatment might be preferred. In this case report, we present a colonic non-Hodgkin lymphoma patient who was admitted with weight loss and abdominal pain. This case reports on a patient who was younger than what has generally been reported in the literature.
