Amyloidosis is a rare, benign, slowly progressive disease characterized by extracellular accumula... more Amyloidosis is a rare, benign, slowly progressive disease characterized by extracellular accumulation of amyloid in different tissues of the body. It accounts for 0.2-1.2% of benign laryngeal tumors and usually presents as an isolated localized laryngeal amyloidosis, but can also be part of systemic amyloidosis. A 26 years old female with history of gradually developing, persistent hoarseness, and progressive dyspnea since 1 year, worsened over the past three days. Outpatient Department based endoscopy showed bilateral mobile, thickened vocal cords with subglottic edematous thickness. Computed tomography scan showed symmetrical thickening of bilateral vocal cord, causing glottic narrowing about 70% on the AP view and about 50% on the lateral view and no cartilage invasion or lymphadenopathy. Microlaryngoscopy and biopsy of the specimen were performed and histopathology confirmed the diagnosis of amyloidosis with Congo red stain. Patient was managed by surgical excision of the mass and long term follow-up. To rule out systemic amyloidosis patient is referred to rheumatology clinic and hematology clinic for further evaluation and management. Histopathological examination of the involved tissue confirms the diagnosis, and long term follow up is mandatory in the management of amyloidosis.

Replantation is the reattachment of a severed body part, with attempts to restore neurovascular a... more Replantation is the reattachment of a severed body part, with attempts to restore neurovascular and musculoskeletal integrity, function, and aesthetics. On September 7, 1964, the first extremity replantation-a completely amputated hand-by vascular anastomosis technique was successfully performed. Soon after, the first replantation of a complete thumb amputation using microvascular anastomosis in a human was successfully conducted by Komatsu et al. in 1968. The overall success rate of limb replantation surgery is around 83.2%. The mechanism of injury plays a role in the outcome; guillotine amputations-for example-carry a better prognosis than crush amputations. Background: Osteoporosis is considered as a major worldwide health problem. Little is known about... more Background: Osteoporosis is considered as a major worldwide health problem. Little is known about its definition, risk factors and prevalence in KSA. Objectives: Evaluating the general knowledge of Saudi females in Riyadh about osteoporosis. Methods: This was an analytical and descriptive study. A reliable osteoporosis knowledge assessment tool (OKAT) questionnaire was published online for considering the levels of osteoporosis awareness from February 2017 to July 2017. Results: The majority of females were educated, married, had college degree and living in medium economic state. Only ten patients reported having osteoporosis. The majority of participants (%) had high OKAT scoreindicating high prevalence of good knowledge among included subjects. The young age, being highly educated and good economic status were significantly associated with good awareness. Conclusion: A good level of knowledge about osteoporosis was found among included subjects. Although having good awareness about osteoporosis in Riyadh, the majority of subjects obtained their knowledge from relatives and physicians. Thus, authorities should create educational programs at all health care facilities to assess a preventive programs for osteoporosis.

Cochlear implant (CI) is considered as safe, and effective surgical procedure for patients with S... more Cochlear implant (CI) is considered as safe, and effective surgical procedure for patients with Sensory Neural Hearing Loss (SNHL). The cochlear implant is an electronic device implanted behind the ear, with electrode arrays which are inserted into the Scala Tympani of the cochlea to stimulate the auditory pathway. The complications rate of cochlear implant surgery is very low, and more common in pediatrics. Furthermore, electrode migration is one of the complications which are defined as gradual slipping out of electrode from cochlea or extrusion from tympanic membrane. Data were being collected through search engines included PUBMED, Google Scholar, and BMJ. The process of sorting out of articles as per inclusion criteria which has an outcome related to electrode migration in post cochlear implant. After removal of duplicate studies, total of 12 articles were found eligible for review analysis. Electrode migration is more common as previously reported, it has shown that fixation techniques should be more reliable to avoid migration of electrode. The purpose of this study is to get a detailed review of electrode migration causes and prevalence in post cochlear implant patients and what factors can lead to migration intraoperative and post-operative cases.

Therefore, primary health care physicians play a major role in controlling these risk factors, and prevent further kidney damage. Sometimes, Nephrology consultation is needed to provide them with an ideal medical care as soon as the diagnosis is made. Purpose: The aim of the study was to assess the level of satisfaction among nephrologists towards primary health care referral system. Methods: a cross-sectional study was carried out in Riyadh city, Saudi Arabia. Participants were nephrologists who work in major hospitals. A designed questionnaire was used for data collection consisted of demographic data of the participant, causes of referral, quality of referral forms, and patients' care prior referral. Results: a total of 89 nephrologists were recruited in the study, most of them were males and non-Saudi physicians. Confirming the diagnosis and providing recommended treatment for the patients are the most common causes for referral. Most of the participants were satisfied about quality of referral and prior referral care. There was no difference in the satisfaction level among consultants compared to specialists. Whereas, private sector nephrologists were more satisfied. Conclusion: Governmental hospitals have to work harder to improve their health system, as they are the main source for most of Saudi citizens' health problems. Providing a nephrologist with good referral letter, and good communication between primary care physicians and other medical specialties provides a better health care to the patients.

Patient was treated successfully with endoscopic curettage of the mass with no inadvertent ill effects.

It accounts for 0.2-1.2% of benign laryngeal tumors and usually presents as an isolated localized laryngeal amyloidosis, but can also be part of systemic amyloidosis. A 26 years old female with history of gradually developing, persistent hoarseness, and progressive dyspnea since 1 year, worsened over the past three days. Outpatient Department based endoscopy showed bilateral mobile, thickened vocal cords with subglottic edematous thickness.
