Objective: To evaluate the prognostic value of baseline neutrophil-to-lymphocyte ratio (NLR) in t... more Objective: To evaluate the prognostic value of baseline neutrophil-to-lymphocyte ratio (NLR) in the prediction of long-term mortality in patients with pulmonary arterial hypertension (PAH). Methods: This prospective study recorded NLR during initial diagnostic right-sided cardiac catheterization in adult patients with PAH. Demographic, clinical, laboratory and haemodynamic variables were compared by NLR tertile. Univariate and multivariate Cox regression analyses were used to determine whether NLR was independently associated with mortality. Results: Adults with PAH (n ¼ 101) were followed-up for mean AE SD 36.8 AE 23.6 months. The number of deaths, New York Heart Association functional capacity (NYHA FC), levels of brain natriuretic peptide (BNP) or C-reactive protein (CRP) and presence of pericardial effusion increased as the NLR tertile increased, but haemoglobin and tricuspid plane annular systolic excursion (TAPSE) decreased. On univariate analysis, high NLR values were associated with mortality, but on multivariate analysis, NLR did not remain an independent predictor of mortality. Baseline NYHA FC, TAPSE, BNP level and pericardial effusion were independent predictors of mortality. Conclusions: NLR was correlated with important prognostic markers in PAH such as NYHA FC, BNP and TAPSE. This simple marker may be useful in the assessment of disease severity in patients with PAH.

Rings were mounted in an organ bath chamber and prewntracted with potassium chloride. Responses of prawntraded IMA rings to increasing dose of sertraline (lo'-1OsMol.L~') were compared with vehicle. Relaxations were expressed as a percentage of precontraction. Secondly, we assessed dose-response curves of phenylephrine (10.'-1O~sMol.L") in the absence or presence of serlraline (105Mol.L~>. Results: Sertraline induced concentration-dependent vasodllation (p<O.Ol; repeated measures ANOVA combined with post-hoc t-statistics), which was profound in nature (maximal dilatory effects exceeding 100% in the concentration-range used). There were no significant differences between rubbed and unrubbed rings,, thus vasodilation was independent of the endothelium. Presence of sertraline markedly attenuated contractile responses to phenylephrine. resulting in a significant rightward shift of the concentrationresponse curve (~~0.05). Conclusion: Our results suggest that sertraline may be protective against arterial vasospasm in depressive post-CABG patients. Vasodilatory properties of sertraline may have important clinical implications in the use of SSRls.

The purpose of this study was to identify LVDD analyzing by new echocardiograpic criteria as well as to evaluate determinants of the LVDD in acromegaly. This cross-sectional study examined 42 patients with acromegaly; 16 in active disease (AA) and 26 cured/ well controlled (CA), and compared them with 30 healthy controls (CG). Ventricular systolic and diastolic functions were studied by conventional and tissue Doppler imaging based on the E/Em ratio and myocardial performance index (MPI). Other clinical parameters possibly contributing to LVDD in acromegaly were also investigated. The prevalence of LV hypertrophy (33%) and LVDD (35.7%) were increased in acromegaly, however, there were no differences between the AA and CA groups. Acromegalic patients had higher LV volumes and LV mass, and septal E/Em ratio compared to CG, whereas LV ejection fraction and MPI were not different. The presence of acromegaly (r = 0.29, P = 0.013), diabetes mellitus (DM) (r = 0.41, P < 0.001), hypertension (r = 0.35, P = 0.002), and sleep apnea (r = 0.56, P = 0.003) were found to be correlated with LVDD, whereas duration and activity of acromegaly were not. In regression analysis, advanced age (OR: 8.53, P = 0.006) and DM (OR: 25.9, P = 0.007) were found to be independent risk factors for LVDD. The risk of LVDD according to new criteria increases in acromegaly. However, it seems to be related to the presence of DM and advanced age and is independent of disease duration and activity.

We conducted a subgroup analysis in ASPECT-NOAC study to determine anticoagulant discontinuation pattern in AF patients with recently initiated NOAC therapy. Methods ASPECT-NOAC was a national, multicenter, 12-month observational study conducted in 34 outpatient cardiology clinics of state, university, private, and research hospitals covering all geographic regions of Turkey. Adult AF patients who were under NOAC therapy for less than four months were enrolled. Patients who discontinued using their NOACs were recorded at the end of 12 months. A comparative analysis of patients with discontinuation of medication was conducted. Results This study included 991 non-valvular AF patients. NOAC continuation data were available for 854 patients. Cardiovascular complications are the leading cause of mortality in patients with DM. DM is a strong independent predictor for both death and myocardial infarction (MI) (1). Unfortunately, the prevalence of DM in the general population is expected to increase in the coming decades (2). As is well known, diabetic patients have more triple-vessel and less single-vessel CAD than nondiabetics (3). Autopsy studies have reported that diabetic patients had a larger extent of raised atherosclerotic lesions, greater incidence of left main coronary artery stenosis, and more extensive and diffuse disease compared with nondiabetic patients. Distal coronary involvement is also more frequent in diabetics than nondiabetics (4,5). To our knowledge, there are currently no data concerning the prevalence of complex coronary lesions (such as bifurcation and ostial lesions) in patients with DM. The aim of the present study was to evaluate the prevalence of bifurcation and ostial coronary lesions in diabetics and to compare it with that in nondiabetics.

Although increasing evidence indicates that cancers are associated with inflammation, the prognos... more Although increasing evidence indicates that cancers are associated with inflammation, the prognostic role of the neutrophil-to-lymphocyte ratio (NLR) and the platelet-tolymphocyte ratio (PLR) in patients with esophageal squamous cell carcinoma remains controversial. We determined the prognostic roles of NLR and PLR in patients with esophageal squamous cell carcinoma who underwent surgical treatment. We retrospectively reviewed 119 patients with esophageal squamous cell carcinoma who underwent surgical resection and complete lymph node dissection from 2004 to 2012. The preoperative NLR and PLR were measured. The patients included 112 (94.1 %) males (mean age, 63.64±8.42 years) of whom 37 (31.1 %) were pathological stage I, 33 (27.7 %) were stage II, and 49 (41.2 %) were stage III. The median follow-up period was 28.68 months. Recurrence was reported in 48 (40.3 %) patients. Mean NLR and PLR were 2.35±1.39 and 140.77±70.47, respectively. A multivariate analysis revealed that NLR was a risk factor for disease-free survival (DFS) (hazard ratio [HR], 1.194; p=0.031) and overall survival (OS) (HR, 1.230; p=0.011), whereas PLR was not a risk factor for DFS or OS. The 3-year OS rates were 51.0 % in low-NLR (<2.97) patients and 17.4 % in high-NLR (≥2.97) patients (p=0.007). Akaike's information criterion decreased when the NLR was included in the multivariate model compared to the multivariate model without NLR. A high NLR was a significant prognostic factor for OS and DFS in patients with surgically treated esophageal squamous cell carcinoma, whereas PLR showed no prognostic significance.

The patient was treated with streptokinase during the first myocardial infarction and primary stenting during reinfarction. To our knowledge, this is the first case in which the occluded right coronary artery was treated with primary stent implantation.

Cardiovascular complications are the leading cause of mortality in patients with DM. DM is a strong independent predictor for both death and myocardial infarction (MI) (1). Unfortunately, the prevalence of DM in the general population is expected to increase in the coming decades (2). As is well known, diabetic patients have more triple-vessel and less single-vessel CAD than nondiabetics (3). Autopsy studies have reported that diabetic patients had a larger extent of raised atherosclerotic lesions, greater incidence of left main coronary artery stenosis, and more extensive and diffuse disease compared with nondiabetic patients. Distal coronary involvement is also more frequent in diabetics than nondiabetics (4,5). To our knowledge, there are currently no data concerning the prevalence of complex coronary lesions (such as bifurcation and ostial lesions) in patients with DM. The aim of the present study was to evaluate the prevalence of bifurcation and ostial coronary lesions in diabetics and to compare it with that in nondiabetics.

Although increasing evidence indicates that cancers are associated with inflammation, the prognos... more Although increasing evidence indicates that cancers are associated with inflammation, the prognostic role of the neutrophil-to-lymphocyte ratio (NLR) and the platelet-tolymphocyte ratio (PLR) in patients with esophageal squamous cell carcinoma remains controversial. We determined the prognostic roles of NLR and PLR in patients with esophageal squamous cell carcinoma who underwent surgical treatment. We retrospectively reviewed 119 patients with esophageal squamous cell carcinoma who underwent surgical resection and complete lymph node dissection from 2004 to 2012. The preoperative NLR and PLR were measured. The patients included 112 (94.1 %) males (mean age, 63.64±8.42 years) of whom 37 (31.1 %) were pathological stage I, 33 (27.7 %) were stage II, and 49 (41.2 %) were stage III. The median follow-up period was 28.68 months. Recurrence was reported in 48 (40.3 %) patients. Mean NLR and PLR were 2.35±1.39 and 140.77±70.47, respectively. A multivariate analysis revealed that NLR was a risk factor for disease-free survival (DFS) (hazard ratio [HR], 1.194; p=0.031) and overall survival (OS) (HR, 1.230; p=0.011), whereas PLR was not a risk factor for DFS or OS. The 3-year OS rates were 51.0 % in low-NLR (<2.97) patients and 17.4 % in high-NLR (≥2.97) patients (p=0.007). Akaike's information criterion decreased when the NLR was included in the multivariate model compared to the multivariate model without NLR. A high NLR was a significant prognostic factor for OS and DFS in patients with surgically treated esophageal squamous cell carcinoma, whereas PLR showed no prognostic significance.
