Background: This study aims to investigate the relationship between preoperative erythrocyte sedi... more Background: This study aims to investigate the relationship between preoperative erythrocyte sedimentation rate and survival in patients undergoing pulmonary resection due to lung cancer. Methods: Between January 2011 and July 2017, a total of 575 patients (433 males, 142 females; mean age: 61.2±9.9 years; range, 29 to 82 years) who were operated due to primary lung cancer in our clinic were retrospectively analyzed. The patients were grouped according to erythrocyte sedimentation rate to analyze the relationship between erythrocyte sedimentation rate and survival. Results: The mean overall survival time was 61.8±1.7 months in 393 patients with an erythrocyte sedimentation rate of ≤24 mm/h and 48.9±2.9 months in 182 patients with an erythrocyte sedimentation rate of ≥25 mm/h (p<0.001). Introduction: Malignancy is rarely seen in pregnancy, and admission with superior vena cava syndr... more Introduction: Malignancy is rarely seen in pregnancy, and admission with superior vena cava syndrome (SVCS), which is an emergency case, is an unusual condition. We present a pregnant patient with VCSS. Case: A 32-year-old 27-week pregnant female patient, whose history revealed no known disease, presented with SVCS due to a mediastinal mass. the case managed by a multidisciplinary approach, was applied urgently biopsy and diagnosed as diffuse large B-cell lymphoma. Treatment was planned for the patient, and she was followed with no problems. A Caesarean section (C-section) was performed at the 35th week of the pregnancy. Conclusion: SVCS in pregnancy is an oncologic emergency disorder that is extremely rare encountered. the diagnosis of the patient as soon as possible is the key point of treatment. UHD gerek spontan pnömotoraks sebebi ile tüp torakostomi uygulanan hastalarda gerekse akciğer rez... more UHD gerek spontan pnömotoraks sebebi ile tüp torakostomi uygulanan hastalarda gerekse akciğer rezeksiyonu uygulanan hastalarda sıkça karşı karşıya kalınan bir morbiditedir. Primer spontan pnömotorakslı olgularda 5-7 günden daha uzun süren hava drenajı UHD olarak kabul edilmekte ve cerrahi tedavi açısından da bir endikasyon oluşturmaktadır. UHD postoperatif hastalarda ise plörodezden tekrar operasyona kadar değişebilen ek girişimler gerektirmektedir. Bu önemli özellikleri nedeni ile UHD'nin erken tanısı hastalara bir çok avantaj sağlayabilir. Prospektif olarak yapılandırdığımız çalışmamızda; hastaların hava kaçaklarını hassas bir anemometre ve android işlemcili bir cep telefonu yardımı ile ölçtük. Tanısal amaçlı operasyona alınan hastaya, sol hemitorakstan videotorakoskopik biyopsi uygulandı. Histopatolojik inceleme sonucunda kesin bir tanıya varılamaması üzerine aynı işlem ikinci bir operasyon ile sağ hemitoraksa yönelik uygulandı. Yine tanı elde edilememesi üzerine hasta tekrar operasyona alınarak mediastinotomi uygulandı. Tüm histopatolojik inceleme sonuçları ve klinik bulguları ile birlikte değerlendirilen olguda idiyopatik mediastinal fibrozis tanısına ulaşıldı. Amaç: Ortak etiyolojik faktörler nedeni ile larinks ve akciğer maligniteleri, aynı hastada eş zam... more Amaç: Ortak etiyolojik faktörler nedeni ile larinks ve akciğer maligniteleri, aynı hastada eş zamanlı veya metakron olarak gelişebilmektedir. Çalışmamızda bu hastalara yönelik klinik yaklaşımlar ile tedavi protokollerimizi sunmayı amaçladık. Gereç ve Yöntem: Kliniğimizde Ocak 2010 Nisan 2016 yılları arasında, öncesinde larinks malignitesi tanısı alan ve akciğerde lezyon sebebi ile cerrahi uygulanan hastalar retrospektrif olarak incelendi. Bulgular: Çalışmaya dahil edilen, tamamı erkek 20 hastanın yaş ortalaması 62.18 2.65 (47-84) idi. Öz Objective: To present cases of thoracic complications that developed after urologic interventi... more Öz Objective: To present cases of thoracic complications that developed after urologic interventions and were treated in collaboration with thoracic surgery. Materials and Methods: Patients who were operated in the urology clinic at our hospital between January 2014 and December 2017 and required thoracic surgery consultation were retrospectively reviewed. Forty-two patients with pneumothorax, pleural effusion, hydropneumothorax and diaphragm injury were included in the study. Six patients, who had preoperative diaphragm invasion and underwent preoperative diaphragm incision, were excluded. Results: Tube thoracostomy (TT) was applied in only 5 patients who developed pneumothorax. Three patients with isolated pleural effusion were treated with TT and 3 with thoracentesis. All patients who developed hydropneumothorax were found to have undergone nephrectomy (3 left, 1 right). Background Endoscopic thoracic sympathicotomy is an effective approach to the treatment of idiopa... more Background Endoscopic thoracic sympathicotomy is an effective approach to the treatment of idiopathic localized hyperhidrosis, and compensatory sweating is the main reason for patient dissatisfaction. Our study discusses both the long-term outcomes of sympathicotomy and the course of compensatory sweating. Methods Patients with palmar and/or axillary hyperhidrosis who were operated by the same surgical team between January 2008 and December 2014 were included in the study. After at least 5 years (60 months) from operation, patients were questioned about their treatment outcomes by using an original survey form. Results Of the 137 patients included in the study, 88 (64.2%) were female and 49 (35.8%) were male. The mean time from the operation to the survey interview was 80.9 ± 14.1 (64–136) months. After operation, complaints disappeared in 95.1% of the patients, and decreased in 4.9% with palmar hyperhidrosis. Complaints completely disappeared in 12.9% and decreased in 81.7% of the ...

The aim of this study was to reveal the incidence and risk factors of PAL in videoassisted thoracic surgery (VATS) lung cancer resection, and to evaluate the effect of PAL on postoperative complications, postoperative length of stay (PLOS), and medical costs. Methods: Continuous patients who underwent VATS major pulmonary resection for lung cancer between January 2014 and December 2015 were studied. Clinical data of these patients were obtained from the Western China Lung Cancer Database. PAL was defined as air leak more than 5 days after surgery. The risk factors for PAL were analyzed, as well as the effect of PAL on postoperative clinical recovery.

Background: Although the etiology of primer spontaneous pneumothorax (PSP) is not exactly underst... more Background: Although the etiology of primer spontaneous pneumothorax (PSP) is not exactly understood, it has been suggested to be associated with rupture of bulla/bleb formations at the lung apices in thin, tall, young adults. In this study, we aim to evaluate whether delayed puberty is a new etiological factor for PSP. Materials and Methods: A total of 91 patients who were treated for PSP in our clinic between January 2013 and September 2016 and who were scheduled for a follow-up visit by phone call and underwent pubertal-stage assessment were retrospectively analyzed. Results: Of all patients, 10 (12.1%) were females and 81 (87.9%) were males. The mean age was 25.90 ± 6.83 (range, 17 to 38) years and the mean height was 178.90 ± 7.6 (range, 161 to 197) cm. The mean age at menarche of female patients was 14.20 ± 1.31 (range, 12 to 17) years. The women had a late age at menarche compared to the overall Turkish female population (p < 0.0001). The mean age for the development of facial hairs in the men was 16.63 ± 1.69 (range, 13 to 22) years, showing a later age, compared to the overall Turkish male population (p < 0.0001). Conclusion: Our study results showed that the patients with pneumothorax had an advanced age at puberty, compared to the normal overall population, and that the pubertal growth spurt occurred within a short period of time. This condition is also suggested to have an effect on the lung maturity and the development of pneumothorax.

Methods Patients, who were being followed-up at our clinic between December 2016 and July 2017, were separated into three groups and were investigated prospectively. Materials and Methods A total of 887 patients, who underwent lung resection due to primary lung m... more Materials and Methods A total of 887 patients, who underwent lung resection due to primary lung malignancy at our clinic between January 2000 and December 2016, were retrospectively analysed. Group 1 (n = 44) consisted of the patients who developed PAF and Group 2 (n = 843) consisted of the patients without PAF. Age and sex of the patients, comorbidities, previous diagnosis of malignancy, and surgery-related variables were evaluated using statistical methods for their effects on the development of AF. A score was assigned to each identified risk factor and scores of the patients were calculated. The risk of developing PAF was evaluated based on this scoring system. Results We found that !60 years of age and the diagnosis of chronic obstructive pulmonary disease (COPD) were significant risk factors for the development of PAF (p < 0.05). The risk of developing PAF was not associated with male sex, previous history of malignancy, presence of comorbidities, and the type of surgery applied. There was an increased risk of AF with increasing scores in the risk calculation system. Conclusion Advanced age and the presence of COPD were found to be associated with an increased risk of developing PAF. In addition we found a significant increase in the risk of developing PAF in the presence of multiple factors, although they did not reach statistical significance alone.

Introduction: There is no specific marker for lung cancer, but, in some lung cancer types, carcin... more Introduction: There is no specific marker for lung cancer, but, in some lung cancer types, carcinoembryonic antigen (CEA) can reach high levels in the blood and pleural fluid. Aim: This study investigated the relationship of CEA levels in blood (CEA B) and intraoperative pleural lavage fluid (CEA P) in non-small-cell lung cancer (NSCLC) with the type, stage, and extent of lung cancer. Material and methods: A total of 50 patients, who underwent surgery at our clinic due to NSCLC (group I) or benign lung pathology (group II), were assessed. For this prospectively designed study, 25 consecutive patients were included in each group, and their CEA B and CEA P levels were investigated. Results: When the levels of CEA P were compared, the average value of group I (1.35 ng/ml) was significantly higher than the average value of group II (0.04 ng/ml) (p = 0.027). When CEA levels were examined separately, and average values were taken according to surgical pathology results, both CEA B and CEA P levels of adenocarcinoma patients were found to be higher than those of the other groups. This difference was only significant for the level of CEA P (p = 0.026). Conclusions: Although the average CEA B levels of patients with adenocarcinoma were higher than those of patients with other histopathological types, this difference was not statistically significant. However, we found that CEA P levels were significantly higher in patients with adenocarcinoma. These results have led us to consider that CEA P elevation is a more sensitive marker than the elevation of CEA B .

Pneumothorax is a rarely seen condition during pregnancy, when changes in the respiratory physiol... more Pneumothorax is a rarely seen condition during pregnancy, when changes in the respiratory physiology can sensitize the mother and fetus to the signs of hypoxia. Symptoms of pneumothorax, such as dyspnea, tachypnea, and chest pain, can also be attributed to pregnancy and complications with pregnancy and this can lead to misdiagnoses. The limitations in the use of diagnostic tests, such as chest X-ray and computed tomography, make treatment more difficult. Here, we report of two cases admitted to our hospital due to spontaneous pneumothorax during pregnancy. The diagnosis of pneumothorax was made based on a thorax ultrasonography in both patients, whereas a chest X-ray was used in the follow-up period, without the need for a thorax tomography. While one patient was treated via a tube thoracostomy, the other was treated via video-assisted thoracoscopic surgery.

Objective: Seropositive myasthenia gravis (MG) depends on the presence of acetylcholine receptor ... more Objective: Seropositive myasthenia gravis (MG) depends on the presence of acetylcholine receptor antibodies (AchR-Ab) against nicotinic acetylcholine receptors at the postsynaptic neuromuscular junction. In this study, we investigated the effect of AchR-Ab levels to symptoms and treatment in the MG patients underwent surgery for thymic pathology. Materilas and Methods: The records including level of preoperative AchR-Ab, type of thymic pathology, the changes of symptoms and treatment after surgery for thymic pathology of 37 MG patients between January 2007 and December 2015 have been viewed retrospectively. Results: The mean age of 37 patients (21 females, 16 males) was 40.2 ± 14.9 years (range, 18 to 75 years). The mean of the level of AchR-Ab was 144.7 ± 427.6 nmol/L (range, 0.1 to 1806 nmol/L). In the patient group including the reduced use of anticholinesterase after surgery the mean of level of AchR-Ab was 241.3 nmol/L while it was 10.8 nmol/L in the patient group including the non-reduced use of anticholinesterase (p = 0.082) after a mean follow-up period of 23 months postoperatively. The mean levels were 246.7 nmol/L and 8.5 nmol/L for the reduced and non-reduced use of corticosteroid patient groups, respectively (p = 0.001). In the 25 patients with fewer symptom after surgery the mean of the AchR-Ab level was 205.3 nmol/L while it was 18.3 nmol/L in the patients without any changes (p = 0.071). Conclusion: We concluded that the preoperative level of AchR-Ab was associated with postoperative dosage of anticholinesterase and corticosteroids and severity of symptoms postoperatively. We think that the levels of AchR-Ab titers can be a marker for the efficacy of thymic surgery.

Materials and Methods: Patients who were operated in the urology clinic at our hospital between January 2014 and December 2017 and required thoracic surgery consultation were retrospectively reviewed. Forty-two patients with pneumothorax, pleural effusion, hydropneumothorax and diaphragm injury were included in the study. Six patients, who had preoperative diaphragm invasion and underwent preoperative diaphragm incision, were excluded. Results: Tube thoracostomy (TT) was applied in only 5 patients who developed pneumothorax. Three patients with isolated pleural effusion were treated with TT and 3 with thoracentesis. All patients who developed hydropneumothorax were found to have undergone nephrectomy (3 left, 1 right). Our study discusses both the long-term outcomes of sympathicotomy and the course of compensatory sweating. Methods Patients with palmar and/or axillary hyperhidrosis who were operated by the same surgical team between January 2008 and December 2014 were included in the study. After at least 5 years (60 months) from operation, patients were questioned about their treatment outcomes by using an original survey form. Results Of the 137 patients included in the study, 88 (64.2%) were female and 49 (35.8%) were male. The mean time from the operation to the survey interview was 80.9 ± 14.1 (64–136) months. The aim of this study was to reveal the incidence and risk factors of PAL in videoassisted thoracic surgery (VATS) lung cancer resection, and to evaluate the effect of PAL on postoperative complications, postoperative length of stay (PLOS), and medical costs. Methods: Continuous patients who underwent VATS major pulmonary resection for lung cancer between January 2014 and December 2015 were studied. Clinical data of these patients were obtained from the Western China Lung Cancer Database. PAL was defined as air leak more than 5 days after surgery. The risk factors for PAL were analyzed, as well as the effect of PAL on postoperative clinical recovery.

Background: Although the etiology of primer spontaneous pneumothorax (PSP) is not exactly underst... more Background: Although the etiology of primer spontaneous pneumothorax (PSP) is not exactly understood, it has been suggested to be associated with rupture of bulla/bleb formations at the lung apices in thin, tall, young adults. In this study, we aim to evaluate whether delayed puberty is a new etiological factor for PSP. Materials and Methods: A total of 91 patients who were treated for PSP in our clinic between January 2013 and September 2016 and who were scheduled for a follow-up visit by phone call and underwent pubertal-stage assessment were retrospectively analyzed. Results: Of all patients, 10 (12.1%) were females and 81 (87.9%) were males. The mean age was 25.90 ± 6.83 (range, 17 to 38) years and the mean height was 178.90 ± 7.6 (range, 161 to 197) cm. The mean age at menarche of female patients was 14.20 ± 1.31 (range, 12 to 17) years. The women had a late age at menarche compared to the overall Turkish female population (p < 0.0001). The mean age for the development of facial hairs in the men was 16.63 ± 1.69 (range, 13 to 22) years, showing a later age, compared to the overall Turkish male population (p < 0.0001). Conclusion: Our study results showed that the patients with pneumothorax had an advanced age at puberty, compared to the normal overall population, and that the pubertal growth spurt occurred within a short period of time. This condition is also suggested to have an effect on the lung maturity and the development of pneumothorax.

Materials and Methods A total of 887 patients, who underwent lung resection due to primary lung m... more Materials and Methods A total of 887 patients, who underwent lung resection due to primary lung malignancy at our clinic between January 2000 and December 2016, were retrospectively analysed. Group 1 (n = 44) consisted of the patients who developed PAF and Group 2 (n = 843) consisted of the patients without PAF. Age and sex of the patients, comorbidities, previous diagnosis of malignancy, and surgery-related variables were evaluated using statistical methods for their effects on the development of AF. A score was assigned to each identified risk factor and scores of the patients were calculated. The risk of developing PAF was evaluated based on this scoring system. Results We found that !60 years of age and the diagnosis of chronic obstructive pulmonary disease (COPD) were significant risk factors for the development of PAF (p < 0.05). The risk of developing PAF was not associated with male sex, previous history of malignancy, presence of comorbidities, and the type of surgery applied. There was an increased risk of AF with increasing scores in the risk calculation system. Conclusion Advanced age and the presence of COPD were found to be associated with an increased risk of developing PAF. In addition we found a significant increase in the risk of developing PAF in the presence of multiple factors, although they did not reach statistical significance alone.

Introduction: There is no specific marker for lung cancer, but, in some lung cancer types, carcin... more Introduction: There is no specific marker for lung cancer, but, in some lung cancer types, carcinoembryonic antigen (CEA) can reach high levels in the blood and pleural fluid. Aim: This study investigated the relationship of CEA levels in blood (CEA B) and intraoperative pleural lavage fluid (CEA P) in non-small-cell lung cancer (NSCLC) with the type, stage, and extent of lung cancer. Material and methods: A total of 50 patients, who underwent surgery at our clinic due to NSCLC (group I) or benign lung pathology (group II), were assessed. For this prospectively designed study, 25 consecutive patients were included in each group, and their CEA B and CEA P levels were investigated. Results: When the levels of CEA P were compared, the average value of group I (1.35 ng/ml) was significantly higher than the average value of group II (0.04 ng/ml) (p = 0.027). When CEA levels were examined separately, and average values were taken according to surgical pathology results, both CEA B and CEA P levels of adenocarcinoma patients were found to be higher than those of the other groups. This difference was only significant for the level of CEA P (p = 0.026). Conclusions: Although the average CEA B levels of patients with adenocarcinoma were higher than those of patients with other histopathological types, this difference was not statistically significant. However, we found that CEA P levels were significantly higher in patients with adenocarcinoma. These results have led us to consider that CEA P elevation is a more sensitive marker than the elevation of CEA B .

Pneumothorax is a rarely seen condition during pregnancy, when changes in the respiratory physiol... more Pneumothorax is a rarely seen condition during pregnancy, when changes in the respiratory physiology can sensitize the mother and fetus to the signs of hypoxia. Symptoms of pneumothorax, such as dyspnea, tachypnea, and chest pain, can also be attributed to pregnancy and complications with pregnancy and this can lead to misdiagnoses. The limitations in the use of diagnostic tests, such as chest X-ray and computed tomography, make treatment more difficult. Here, we report of two cases admitted to our hospital due to spontaneous pneumothorax during pregnancy. The diagnosis of pneumothorax was made based on a thorax ultrasonography in both patients, whereas a chest X-ray was used in the follow-up period, without the need for a thorax tomography. While one patient was treated via a tube thoracostomy, the other was treated via video-assisted thoracoscopic surgery.

Objective: Seropositive myasthenia gravis (MG) depends on the presence of acetylcholine receptor ... more Objective: Seropositive myasthenia gravis (MG) depends on the presence of acetylcholine receptor antibodies (AchR-Ab) against nicotinic acetylcholine receptors at the postsynaptic neuromuscular junction. In this study, we investigated the effect of AchR-Ab levels to symptoms and treatment in the MG patients underwent surgery for thymic pathology. Materilas and Methods: The records including level of preoperative AchR-Ab, type of thymic pathology, the changes of symptoms and treatment after surgery for thymic pathology of 37 MG patients between January 2007 and December 2015 have been viewed retrospectively. Results: The mean age of 37 patients (21 females, 16 males) was 40.2 ± 14.9 years (range, 18 to 75 years). The mean of the level of AchR-Ab was 144.7 ± 427.6 nmol/L (range, 0.1 to 1806 nmol/L). In the patient group including the reduced use of anticholinesterase after surgery the mean of level of AchR-Ab was 241.3 nmol/L while it was 10.8 nmol/L in the patient group including the non-reduced use of anticholinesterase (p = 0.082) after a mean follow-up period of 23 months postoperatively. The mean levels were 246.7 nmol/L and 8.5 nmol/L for the reduced and non-reduced use of corticosteroid patient groups, respectively (p = 0.001). In the 25 patients with fewer symptom after surgery the mean of the AchR-Ab level was 205.3 nmol/L while it was 18.3 nmol/L in the patients without any changes (p = 0.071). Conclusion: We concluded that the preoperative level of AchR-Ab was associated with postoperative dosage of anticholinesterase and corticosteroids and severity of symptoms postoperatively. We think that the levels of AchR-Ab titers can be a marker for the efficacy of thymic surgery.
