Sarcomas are a rare type of solid cancers, encompassing more than one hundred subtypes, and occur at any age. 1 Sarcomas of the meninges may originate primarily from mesenchymal cells or may be metastases of extracranial soft tissue sarcomas. 2 Symptoms such as nausea, vomiting, headache, seizure, and spinal cord compression may occur depending on the size and location of the tumor. Cranial magnetic resonance imaging (MRI) is often used for diagnosis. The primary treatment for sarcomas is surgery; other treatment options include radiotherapy (RT), chemotherapy (CT), targeted therapies, and immunotherapy. 3 Though RT is one of the risk factors for sarcoma development, RT-related dural sarcomas are extremely rare. 2 There is no consensus on the management of these extremely rare, malignant neoplasms. Here, the authors report a case of RT-related dural sarcoma and its management. CASE REPORT A 42-year-old woman presented with a headache and swelling on the scalp, growing gradually over the last three months. The physical examination did not reveal any abnormal finding except for the 4x2 cm swelling of the left parietal region. The patient presented a seven-year-old history of surgery for the left frontoparietal, intracranial mass originating from the calvary. After the complete resection of the tumor, the pathological report was found to be consistent with atypical meningioma (WHO grade 2), and the patient received adjuvant RT with a total dose of 6000 centigray (cGy) external RT in 30 fractions.

Dermoid cysts have two types as intradural and extradural. Intradural dermoid cysts are originated from the intracranial cerebrospinal fluid space. However, calvarium-originated extradural lesions increase in size with a slow growth in cyst volume, but the cause of active growth is unknown. Dermoid cysts act like slow-growing cerebral tumors. Differential diagnosis includes other cysts and cystic tumors. Their radiologic appearance looks like polycystic lesions which is associated with a wide expansion in the cerebrospinal fluid areas and cause brain translocation. In the cranial tomography and magnetic resonance imaging assessment, high-cholesterol-containing dermoids demonstrate cerebrospinal fluid characteristics. The present study based on the review of central nervous system patients with highly rare dermoid localizations such as calvarial bone, temporal lobe and lateral ventricle.

Anevrizmal subaraknoid kanama (aSAK) sonrası şant bağımlı kronik hidrosefali insidansı %6 ile %67... more Anevrizmal subaraknoid kanama (aSAK) sonrası şant bağımlı kronik hidrosefali insidansı %6 ile %67 oranları arasında değişiklik göstermektedir (1). Hidrosefali, aSAK'li hastaların klinik düzelmelerini olumsuz yönde etkileyen, acil müdahale gerektiren cerrahi bir durumdur (2). Hastalarda, akut dönemde SAK'yi takiben saatler içinde gelişebildiği gibi kronik dönemde de hidrosefali tablosu ile karşılaşılabilir (3). Amaç: Lamina terminalis fenestrasyonu, serebral anevrizma cerrahisinde ameliyat sonrası hidrosefali gelişme olasılığını azaltmak için kullanılan bir yöntemdir. Bu yazının amacı, yüksek dereceli subaraknoid kanamalı hastalarda lamina terminalis fenestrasyonunun hidrosefali üzerine kısa dönem sonuçlarını araştırmak ve bu konudaki klinik tecrübemizi aktarmaktır. Gereç ve Yöntem: Kliniğimizde 2014-2015 yıllarında serebral anevrizmasına yönelik cerrahi klipaj uyguladığımız 10 yüksek dereceli subaraknoid kanamalı hastayı değerlendirdik. Bulgular: Çalışmamızda değerlendirmeye aldığımız 10 hastadan üçünde hidrosefali gelişti. Bu üç hastadan birinde ameliyat öncesi dönemde de hidrosefali bulunmaktaydı. Ayrıca, preoperatif dönemde hidrosefalisi olan üç hastanın ise postoperatif dönemde hidrosefali tablolarının kaybolduğu gözlemlendi. Sonuç: Lamina terminalis fenestrasyonu, gerek cerrahi klipaj öncesi serebral relaksasyonu sağlamak gerekse klipaj sonrası hidrosefali gelişme riskini azaltmak amacıyla uygulanabilen kolay ve etkili bir işlemdir.

Objectives: The shape and localization of the scalp incision are the factors that affect surgical... more Objectives: The shape and localization of the scalp incision are the factors that affect surgical outcome. With the help of developing technology, it is aimed to decrease the size of incisions and decrease the morbidity. The ideal shape and length of the incision should not disrupt the scalp feeding and provide the best possible wound healing, but also have to provide adequate control of the surgical field. In our study, we aimed to observe the effects of linear scalp incision on patients' surgical comfort and wound healing. Materials and Methods: In our retrospective study, we evaluated linear scalp incision in 26 patients with supratentorial cortical-subcortical tumors between 2017 and 2018. Tumor location was determined by neuronavigation and linear incision lines were planned pre-operatively in all patients. Results: The size of superficially located tumors was equal or less than 5 cm. The average length of incision was 9.2 cm and was parallel to the coronal suture. No difficulties were seen in reaching the lesion in terms of incision length or location. There was no significant morbidity and mortality in the postoperative period, except of patients which were temporary paresis. No complications were seen in wound healing. Silk sutures were taken on average for 7.4 days. Thirty four of the thirty two patients were discharged on the 3 postoperative day. rd Conclusion: A linear incision can be applied as an alternative to other incisions in superficially localized and to adequate size (≤5 cm) supratentorial region tumors which were determined with neuronavigation.

The most common extradural tumors are metastasis, lymphomas, benign tumors and malignant bony tumors. Herein, we present a-29-year-old Turkish woman with pure extradural mass that histopathologically diagnosed as AVM. When an extradural spinal compressive mass is detected, neurosurgeons should consider on AVM in differential diagnosis and requirement of selective spinal angiography.

The location of burr holes in posterolateral cranial base approaches should be appropriate to pro... more The location of burr holes in posterolateral cranial base approaches should be appropriate to provide an adequate operative field, and surgical freedom is crucial for bone window opening. The aim of this study was to search for more convenient and easily detectable landmarks in comparison with current landmarks in posterolateral cranial base surgery. Twenty 3-dimensional reconstructed head and neck computed tomography angiography images (group 1) and 20 cadaver heads (group 2) were evaluated. An imaginary line connecting the angle of the mandible and the mastoid tip was extended upward. A second line passing through the lateral edge of the zygomatic arch was also extended posteriorly. The authors examined if the first line met with the sigmoid sinus throughout its course and determined the location of the intersection point of these 2 lines relative to the sigmoid-transverse sinus junction. The intersection point did not correspond to the sinus region in 3 images from group 1 and 4 ...

█ INTRODUCTION A dvances in communication technology have transformed mobile phones into smartpho... more █ INTRODUCTION A dvances in communication technology have transformed mobile phones into smartphones, which have become nearly indispensable in daily life and can function as portable pocket computers (14). The 'apps', which are applications specific to hand-held computers, including smartphones and tablets, have expanded the functions of mobile technology in social and business activities. In the medical field, many apps developed for use in health care, health education and health science are available for online purchase and downloading (10,13,18). The benefits of these AIM: Accurate localizations of cerebral lesions ensure good surgical outcomes and minimize intraoperative complications. Currently, microscope-integrated neuronavigation systems compensate these requirements. Since these units are expensive and large, the need to create more practical and portable systems in this "smart" century has become apparent. MATERIAL and METHODS: The See-Through Camera smartphone application (app) is a photography app that penetrates the existing background picture and merges this with a camera image on the smartphone display. This app was applied to neurosurgery for visualization of the projection of the tumors over head in 20 patients. App accuracy was tested according to neuronavigation system. RESULTS: Precision of the app was evaluated according to the superposition ratio of the tumor image drawn by the navigation system with the tumor picture on the smartphone display. This ratio was 40%-100% in 16 (80%) patients and below 40% as an unfavorable result in the rest of the patients. CONCLUSION: Basic rationality of this app can provide the basis of future pocket navigation systems in terms of its design and principles.

She had previously presented to the Department of Psychiatry with related symptoms. Cranial magnetic resonance imaging (MRI) revealed a 4.8×4.5×4.5 cm, poorly marginated, diffusely infiltrating necrotic lesion with peripheral contrast enhancement in the right frontotemporoparietal region (Figure 1). The mass lesion and related edema involved the capsula interna, thalamus, right cerebral peduncle and mesencephalon, suggesting a high-grade glioma. Diffusionweighted MRI indicated a diffusion restriction in the lesion. A midline shift of 8 mm and compression of the right lateral and third ventricles warranted emergency surgery.

Foot drop generally occurs in spinal and peripheric pathologies. But central lesions associated w... more Foot drop generally occurs in spinal and peripheric pathologies. But central lesions associated with paracentral lobule (tumor, ischemic or hemorrhagic stroke) may cause foot drop. As we know paracentral lobule is associated with movements of lower extremities and urinary function. If neurological examination does not explain to radiology, clinician should be considered with central lesions. Although in central lesions there are signs of upper motor neuron findings like babinski sign, hyperreflexia, klonus that we could not find any sign of upper motor neuron. In this case, we report a parasagital meningioma result in foot drop and improved dramatically after operation.
