Primary Pulmonary Angiomatoid Fibrous Histiocytoma is a recently described soft tissue tumor with... more Primary Pulmonary Angiomatoid Fibrous Histiocytoma is a recently described soft tissue tumor with challenging differential diagnosis both clinically and pathologically due to its rarity in this location. It may also occur as a secondary malignancy and its occurrence either as a somatic malignancy arising in the germ cell tumor or as a secondary malignancy after chemotherapy is questionable. In this report, we present a 29-year-old male patient with a mass in the lower lobe of the left lung, who underwent orchiectomy and received adjuvant chemotherapy due to a mixed germ cell tumor 8 years ago. Morphology, immunophenotype, and molecular findings were consistent with the diagnosis of primary pulmonary angiomatoid fibrous histiocytoma. Fluorescent in situ hybridization was unable to demonstrate the presence of 12p amplification or isochromosome 12p, which is known as the key event in the development of testicular germ cell neoplasia even present in somatic malignancies arising in germ cell tumors. Our results support that angiomatoid fibrous histiocytoma arising as a secondary malignancy does not represent the somatic transformation of germ cell tumors.

Bu çalışmada amaçlanan, TB tanısı konan olguların mikrobiyolojik verilerini ortaya koyarak, tüber... more Bu çalışmada amaçlanan, TB tanısı konan olguların mikrobiyolojik verilerini ortaya koyarak, tüberküloz tanısında kullanılacak örnek sayısını belirlemek, radyolojik değişiklikler, yaş ve ek hastalıkların mikrobiyolojik sonuçlarla olan ilişkisinin araştırılmasıdır. Gereç ve Yöntemler Çalışmaya toplam 140 erkek hasta alınmıştır. Hastaların tamamından antitüberküloz tedavi başlanmadan önce en az üç adet mikroskopi ve kültür öreneği ile birlikte posterioanterior ve lateral göğüs grafileri çekildi. Mikroskopik değerlendirme için Ehrlich-Zeihl-Neelsen (EZN) yöntemi ve kültür ortamı olarak sıvı (BACTEC) ve/veya Löwenstein-Jensen katı besiyeri kullanıldı. Bulgular Balgam örneklerinin tüberküloz tanısına sağladığı katkıya bakılacak olursa, ilk mikroskopi pozitifliğinin %71.2, ikinci mikroskopi pozitifliğinin %17.6 ve üçüncü mikroskopi pozitifliğinin katkısının %7.2 düzeyinde olduğu (ilk üç mikroskopinin katkısını toplamda %96) anlaşılmaktadır. Kültürde üreme saptama oranının ise %67.14'tür. İlk üç kültürde saptanan kültür pozitifliği ise %93.61 olarak hesaplanmıştır. Olguların %21.5'de (30/140) ek hastalığı olduğu anlaşılmıştır. Sonuç Akciğer tüberkülozu tanısı almış hastaların mikrobiyolojik verilerine bakıldığında, hastalığın tanısı için üç balgam örneği alınması yeterli gözükmektedir. Göğüs grafisinde kavite varlığı mikroskopi pozitifliği görülme oranını arttırmaktadır. Ayrıca ek hastalığın varlığı ile kavite oluşumunu ve mikroskopi pozitifliği arasında da pozitif bir ilişki vardır. Uzamış semptom süresi ile kavite oluşumu ve ilk mikroskopi pozitifliği oranları belirgin korelasyon göstermektedir.

The aim of the present study was to evaluate the application of tuberculosis preventive treatment... more The aim of the present study was to evaluate the application of tuberculosis preventive treatment (TB-PT). Demographic data, indications and results for cases that received TB-PT at the Ankara Tuberculosis Control Dispensary No. 7 between 2008 and 2011 were retrospectively evaluated. The 'Prevention with Drugs' registry at the dispensary was used. A total of 463 cases received TB-PT, with the indications including close contact with an active TB case (44%), positive tuberculin skin test (TST) in a child <15 years-old (25%) and immunosuppressive therapy (31%). The immunosuppressed group (n=144) were administered steroids (10%) or tumor necrosis factor (TNF)-α inhibitors (90%). Indications of TST conversion and sequela lesions were not observed among the cases. The male/female ratio was 106/98 for cases with TB close contact, 61/54 for TST-positive cases and 85/59 for immunosuppressed cases. The mean ages of these groups were 9±5.7, 9.5±3.8 and 38±14.9 years, respectively. TB-PT was completed in 364 cases (78.6%), and the rate of discontinuation due to adverse effects was 1% for TB close contact and 2% for TST-positive cases, but 5% for immunosuppressed cases. While the percentage of TB close contact cases receiving TB-PT decreased during the four-year study period, the percentage of cases with immunosuppression (in particular patients using TNF-α inhibitors) increased. Among the studied cases, only two subjects developed active TB. The first case involved a 1.5-year-old female that had close contact exposure to TB from a parent, while the other case involved a 14-year-old TST-positive male (induration size,16 mm). In conclusion, patients receiving TB-PT should be monitored and/or followed-up carefully to control any side-effects from the treatment and development of active TB.

HIV (Human Immunodeficiency Virus) is the virus that causes AIDS (Acquired Immune Deficiency Synd... more HIV (Human Immunodeficiency Virus) is the virus that causes AIDS (Acquired Immune Deficiency Syndrome), while PCP (Pneumocystis jiroveci pneumonia), tuberculosis, CMV (Cytomegalovirus) and candidiasis are the OIs (opportunistic infections) occurring due to immune deficiency. OIs, and bacterial pneumonias in particular, are the most common causes of mortality, which makes the screening and prophylactic therapy for OIs necessary. The synergy between tuberculosis and HIV has long been known, and worsens the prognosis. PCP is an OI that is caused by a fungus named P. jiroveci. COVID-19 has emerged as a new cause of death among AIDS patients. Screening and prophylactic therapy for Ols is vital for patients with AIDS, however, mortality may be high due to delays in screening and prophylaxis in those whose HIV positivity is detected coincidentally. We report here on a patient who applied to our hospital with suspected COVID-19 pneumonia who was found during followup to be HIV positive with PCP and pulmonary tuberculosis.

Fresh non-small cell lung carcinoma surgical specimens were taken from 17 patients and 3 controls... more Fresh non-small cell lung carcinoma surgical specimens were taken from 17 patients and 3 controls and screened for genetic abnormalities of the c-myc oncogene. Southern blot hybridization analysis demonstrated twoto fivefold amplification of the c-myc gene in 10 cases, i.e., 7 of 13 epidermoid lung carci-nomas, 2 of 3 adenocarcinomas and 1 of 1 osteogenic sarcoma metastatic to the lung. Twoto fivefold amplification was observed in tissues from stage Ill and IV epidermoid carcinomas and adenocarcinomas of the lung. A correlation between cancer stage and c-myc gene amplification was found.

The International Classification of High-resolution Computed Tomography for Occupational and Envi... more The International Classification of High-resolution Computed Tomography for Occupational and Environmental Respiratory Diseases (ICOERD) has been developed for screening and diagnosis of occupational lung diseases. We evaluated the association of icoerd with the International Labor Organization (ILO) classification and respiratory functions in pneumoconiosis. Chest x-rays of patients with pneumoconiosis were classified with ilo and icoerd using hrct, irregular opacity, pleural pathology, and emphysema was detected in 78, 19, and 53 patients, and using chest x-rays in 47, 4, and 14 patients, respectively. There was a significant correlation between ILO categories and ICOERD grades. There was a negative correlation between ILO categories and FEV1% and FVC%, whereas, ICOERD grades were not correlated with FEV1% and FVC%. HRCT was superior to chest x-rays to detect pneumoconiosis in early stage, but not in evaluating pulmonary functions.

Mannose-binding lectin (MBL) is able to bind pathogens as an opsonin and plays an important role ... more Mannose-binding lectin (MBL) is able to bind pathogens as an opsonin and plays an important role in the innate immunity. The aim of the present study was to determine the frequencies of the MBL gene variants in the Turkish population and to examine the presence of any association between MBL variants and development of tuberculosis (TB) in adults and recurrent respiratory tract infections in children. Two structural gene mutations in exon 1 of MBL gene (codon 54 and codon 57) were studied. The overall distribution of genotypes did not significantly differ between controls and TB patients/children with recurrent respiratory system infections. The frequency of allele B was calculated as 0.14, 0.09 and 0.06 for control, TB patients and children with recurrent respiratory system infections, respectively. It was found to be significantly lower in children with recurrent respiratory system infections than in controls (chi2: 4.68, d.f: 1, p: 0.030).

In 18 patients, ultrasonography demonstrated regular pleural thickening which was less than 1 cm except in 1 case. In 4 cases there were a few pleural nodules, whereas in 2 cases the pleural surface showed small nodularity. The latter finding may be diagnostic for a tuberculous etiology. Eighteen patients had multiple, delicate, mobile septations in the effusions, and a lattice-like appearance had formed in 6 cases. Computed tomography was obtained in 7 cases, and pleural thickening was demonstrated in 6 of them. Ultrasonography is a useful imaging modality in the diagnosis of tuberculous pleurisy. Immunosuppression caused by antineoplastic treatments for primary cancer may facilitate the reactivation of latent tuberculosis (TB) infection, which can mimic cancer metastasis. The thought of lung metastasis is an important entity that changes the follow-up and treatment approach. Evaluating TB in initial differential diagnosis can prevent unjustifiable chemoradiotherapy and surgery. In this case report, we submit a 2-years-old boy under follow up of alveolar type rhabdomyosarcoma in masseter muscle also received chemoradiotherapy with a pre-diagnosis of lung metastasis patient who underwent surgical resection when the treatment didn’t produce any benefit and all of the 34 excised nodules from the right lung were reported as “granulomatous inflammation suggestive of TB.”

Immunosuppression caused by antineoplastic treatments for primary cancer may facilitate the reactivation of latent tuberculosis (TB) infection, which can mimic cancer metastasis. The thought of lung metastasis is an important entity that changes the follow-up and treatment approach. Evaluating TB in initial differential diagnosis can prevent unjustifiable chemoradiotherapy and surgery. In this case report, we submit a 2-years-old boy under follow up of alveolar type rhabdomyosarcoma in masseter muscle also received chemoradiotherapy with a pre-diagnosis of lung metastasis patient who underwent surgical resection when the treatment didn’t produce any benefit and all of the 34 excised nodules from the right lung were reported as “granulomatous inflammation suggestive of TB.”
