Резюме Предстяваме клиничен случаи на 39-годишна пациентка (В.Т.Т.) с хордом на околoносните кухи... more Резюме Предстяваме клиничен случаи на 39-годишна пациентка (В.Т.Т.) с хордом на околoносните кухини. Извършено e радикално оперативно лечение и следоперативна лъчетерапия. В момента без данни за рецидив на заболяването. Случаят представлява клиничен интерес поради ниската честота на заболяването, трудното лечение и възможните чести рецидиви. Ключови думи: хордома, околоносни кухини, клиничен случай, рецидив на тумора. Увод Хордомите са редки тумори, изхождащи от ембрионални нотохордални остатъци. През 1857 г. Virchow пръв описва класическите отличителни патоморфолични белези на хордома и ги e нарекъл ecchondrosis physalophora (1). Muller 1858 г. е отбелязал сходство между тези тумори и хорда дорзалис и ги е нарекъл хордоидни тумори (2). Биологичното поведение на хордомите ги характеризира като бавно растящи малигнени тумори с изразен експанзивен и деструктивен локален растеж и склонност към късно метастазиране-до 8-10 год. след терапията, като в повечето случаи метастазирането е след локален рецидив (3). Summary We present a rare case report of a 39 aged female patient (V.T.T) of chordoma of the paransal cavities. She was treated operatively with postoperative radiation therapy up to 60 Gy. The case was interesting with very low frequency of occurrence, difficult therapy and high recurrences rate.

The incidence of gastroesophageal reflux disease (GERD) in patients with laryngopharyngeal sympto... more The incidence of gastroesophageal reflux disease (GERD) in patients with laryngopharyngeal symptoms is greater than expect. A prospective study was performed to investigate the incidence of gastroesophageal reflux disease (GERD) and laryngopharyngeal reflux (LPR) in patients with laryngopharyngeal symptoms. Laryngologic evaluation with nasopharyngeal and laryngeal endoscopy and gastroenterologic evaluation with esophagogastroduodenoscopy including measurement of pH value were performed in 46 patients. Therapeutic intervention included general antireflux precautions and antireflux medication, and eradication of Helicobacter pylori in selected cases. Posterior laryngitis was present in 33 patients. GERD was present in 25 patients, hiatus hernia in 4 patients, H pylori-positive gastritis in 11 patients, and Barrett's metaplasia in 2 patients. Increasing evidence shows that GERD may cause laryngopharyngeal symptoms. Otherwise laryngopharyngeal symptoms can be predictors of GERD. Medical antireflux treatment is also effective for laryngopharyngeal symptoms.

Aim: Esthesionevroblastoma is a rare malignant neuroectodermal tumor that begins development of n... more Aim: Esthesionevroblastoma is a rare malignant neuroectodermal tumor that begins development of neuroepithelial cells of the olfactory membrane. Therefore the biology of the tumor, surgery is a method of choice. Endoscopic transnasal method with craniobasal resection is favorable alternative to more invasive surgical methods in a rigorous assessment of patients. Our aim is to describe the first case in the country operated endonasal with typical clinical and histopathological findings of olfactory neuroblastoma, and to discuss opportunities for radical surgery by endoscopic endonasal sinus surgery. Material and methods: We present a patient with nasal obstruction, epistaxis, smell disturbance, headache and eye symptoms. Results: Single step transnasal endoscopic resection with craniektomia, then undergoes postoperative radiotherapy is made on the patient. One year after surgery, no clinical, endoscopic and radiologic evidence of disease recurrence. Conclusion: Esthesionevroblastoma is a rare malignant tumor that untypical because its initial symptoms are often diagnosed at later stages. Early detection and timely performed treatment is important moment for the prognosis.
