To improve the treatment result for locally advanced head and neck carcinoma, the authors used a ... more To improve the treatment result for locally advanced head and neck carcinoma, the authors used a combination of radiotherapy with superselective continuous intraarterial therapy using carboplatin. The dose limiting toxicity (DLT), maximum tolerated dose (MTD), and treatment effectiveness were tested in Phase I and II protocols. PATIENTS AND METHODS. Thirty-five patients were entered into the study from August 1992 to May 1997. The target arteries were the lingual artery in 18 cases, facial artery in 5 cases, maxillary artery in 11 cases, and external carotid artery initially changing to lingual artery in 1 case. Escalating daily carboplatin doses were tested, starting from 10 mg/m(2) (total dose, 360 or 400 mg/m(2)) to 15 mg/m(2) (total dose, 405 or 450 mg/m(2)) and then 20 mg/m(2) (total dose, 460 or 500 mg/m(2)). Radiotherapy was administered using a 6-megavolt linear accelerator to a total dose of 50-60 grays. Interstitial radiotherapy boost also was used for carcinoma of the tongue. Excluding 3 patients who discontinued treatment, the treatment results of 32 patients were complete response in 21 cases, partial response in 10 cases, and no change in 1 case. Neutropenia was the DLT, and the MTD was 500 mg/m(2). The local control rate was 64%. Superselective continuous intraarterial carboplatin and concurrent radiation therapy can be delivered safely with good efficacy for locally advanced carcinomas of the tongue and base of the tongue. Surgical treatment of these diseases usually incurs severe functional loss. This current approach may be a breakthrough in these cancers.

This paper reports on the measured wheel-rail creep force characteristics and the mechanism of co... more This paper reports on the measured wheel-rail creep force characteristics and the mechanism of corrugation formation by using a full-scale bogie on a test stand. The measured creep characteristics agree well with the calculated values based on Kalker's linear theory. The relationship between creep characteristics and contact position is also discussed. The results show that a small change in wheelset lateral displacement may lead to a great change in creep characteristics. Mathematical models are developed in order to explain the mechanism of corrugation development. Simulation results agree well with practical phenomena. From the simulation results we find that the creep force characteristics and the vertical oscillation system have great influence on the formation mechanism of rail head corrugation.

When nedaplatin (NDP) was used as a single agent in the phase I study, the dose-limiting toxicity... more When nedaplatin (NDP) was used as a single agent in the phase I study, the dose-limiting toxicity (DLT) was thrombocytopenia and the recommended dose (RD) was 100 mg/m2. However, the DLT, maximum tolerated dose (MTD) and RD of NDP used in combination with 5-fluorouracil remained unknown. Therefore, we performed this study to assess the DLT and RD of NDP administered after 5-fluorouracil (5-FU). In this study, 5-FU was administered to 38 patients at a fixed dose (700 mg/m2/d on days 1-5) and NDP administered on day 6 at an initial dose of 80 mg/m2, which was subsequently increased to 100, 120, 130, 140, 150, and 160 mg/m2. The DLT of NDP was leukopenia and its MTD and RD were 160 and 150 mg/m2, respectively. Concerning impairment of renal function, only two patients had a grade I increase in serum creatinine. There were 19 responders (50%, 19/38) achieving partial response or complete response in the evaluation of antitumor effect. The result of this study is notable in that administration of 5-FU before NDP allows the dose of NDP to be substantially increased.

On the basis of this theory, we analyzed the growing process of corrugation by numerical simulation.

In Japan, detached breakwaters made with wave dissipating concrete blocks such as Tetrapods have ... more In Japan, detached breakwaters made with wave dissipating concrete blocks such as Tetrapods have been widely applied, but the effectiveness of such kinds of detached breakwaters on tsunami disaster prevention and stability of the structure have not been discussed in detail. Only few studies on wave run-up reduction were done after Nihonkai Chubu Earthquake Tsunami in 1983. In order to mitigate tsunami disasters in sea coast areas, the effectiveness of detached breakwaters and the stability of wave-dissipating concrete blocks should be properly evaluated. Authors has just studied on the effectiveness of detached breakwater on wave run-up reduction and wave pressure reduction on the seawall taking actually possible damage to detached breakwaters into account . In this particular study, the hydraulic model tests have been systematically and carefully carried out to discuss the stability of wave-dissipating blocks of detached breakwater against solitary tsunami waves. Tetrapods of rear side of the breakwater tends to be damaged like land slide. The damage by tsunami is bigger than that by ordinary wind waves. A countermeasure for improving the stability is also proposed based on the damage process of detached breakwater.

Measurement of wheel-rail contact forces is very important, but it is not executed so often becau... more Measurement of wheel-rail contact forces is very important, but it is not executed so often because of its difficulties. A new measuring method introduced in this paper can measure such contact forces without special wheelsets equipped with strain gauges and slip rings or telemeters. In this method the lateral contact force is measured from the lateral distortion of wheel measured directly by several non-contact gap sensors. Normal and longitudinal contact forces are also measured from the deflection of spring or the strain of parts of the bogies except wheelsets. According to the results of full-size bogie stand tests and train running tests, it is verified that the new method has sufficient practicability of monitoring the contact forces on commercial lines.
