A periodontite e a hipertensão arterial são doenças crônicas, inflamatórias, e significativamente... more A periodontite e a hipertensão arterial são doenças crônicas, inflamatórias, e significativamente prevalentes na população. Evidências clínicas indicam que o tratamento da doença periodontal pode impactar de forma positiva na pressão arterial em pacientes hipertensos e préhipertensos. Objetivo: O objetivo deste presente estudo foi realizar um levantamento bibliográfico de trabalho clínicos que avaliaram o tratamento da doença periodontal na pressão arterial e marcadores sistêmicos envolvidos com a propensão ao desenvolvimento de doenças cardiovasculares. Material e Métodos: Foram consultadas as bases de dados Pubmed, Cochrane, e Google Scholar, utilizando as palavras-chave: "periodontitis"; "periodontal disease"; "periodontitis treatment"; "blood pressure"; e "hypertension". As estratégias de busca objetivaram encontrar todos os artigos que relacionaram o tratamento periodontal, não segregando qual tipo, com a pressão arterial antes e após o tratamento. Para tanto, dez artigos foram selecionados. Resultados: Dos dez artigos selecionados, sete indicaram que o tratamento periodontal reduz a pressão arterial, por meio da melhora das condições clínicas da doença. Apenas três artigos não observaram diferença na pressão arterial dos pacientes submetidos ao tratamento periodontal. Conclusão: Assim, a maioria dos estudos sugerem que o tratamento periodontal contribui para a redução da pressão arterial. Dessa forma, a redução das condições inflamatórias na cavidade oral, podem impactar de forma positiva na hipertensão.

Introdução: A disfunção temporomandibular (DTM) compreende alterações nos músculos mastigatórios,... more Introdução: A disfunção temporomandibular (DTM) compreende alterações nos músculos mastigatórios, na articulação temporomandibular (ATM) e estruturas associadas. Os principais sinais e sintomas incluem dores nos músculos da mastigação ou na ATM, limitação de abertura bucal, luxação do disco articular, dor articular, sons de cliques ou crepitação na ATM e degeneração da ATM. Devido sua etiologia multifatorial, alguns tratamentos não invasivos de suporte são recomendados, mais recentemente, a injeção com o plasma rico em plaquetas (PRP) foi proposta para diversas condições clínicas do sistema estomatognático. Objetivo: O objetivo deste presente estudo foi realizar um levantamento bibliográfico dos principais artigos que correlacionam o tratamento da DTM com a injeção de PRP. Material e Métodos: Foram consultadas as seguintes bases de dados: PubMed, PMC e Cochrane. Os termos de busca utilizados foram "temporomandibular dysfunction" e "platelet-rich plasma". Devido a inovação do tema, as estratégias de busca não limitaram o ano de publicação e não segregaram por diagnóstico de DTM. Inicialmente, 174 artigos foram encontrados e após a aplicação dos critérios de exclusão, somente 11 deles foram selecionados para a revisão. Resultados: Todos os artigos selecionados indicaram uma redução no quadro doloroso na ATM avaliado por meio da Escala Visual Analógica. Além disso, alguns estudos indicaram melhora no som articular e abertura bucal. Conclusão: Apesar dos resultados satisfatórios do uso do PRP no tratamento da DTM, ainda há pouca evidência científica para uma tomada de decisão clínica precisa no uso dessa terapia nos diversos diagnósticos de disfunção. Portanto, mais estudos são necessários para reduzir a heterogeneidade de protocolos clínicos.

Este estudo teve como objetivo comparar a mudança de cor, após a ação de soluções desinfetantes (... more Este estudo teve como objetivo comparar a mudança de cor, após a ação de soluções desinfetantes (Listerine®, Cepacol®, Periogard®, Riozyme®, hipoclorito de sódio a 1%, e água da torneira como controle), e a ação da cuba ultrassônica. Métodos: Foram obtidos 60 corpos de prova, incluídos em bases cilíndricas de resina termopolimerizável, originando 12 grupos (n = 5). Destes, seis grupos foram submetidos aleatoriamente à ação da cuba ultrassônica com soluções desinfetantes por 180 minutos cada, e seis grupos foram imersos nas soluções por igual período. Os valores de mudança de cor na resina da base das próteses foram avaliados por meio do Espectrofotômetro PCB 6807, utilizando a escala CIE L * a * b * para determinação da cor, comparando as amostras antes e após o teste. Para a análise estatística, foi utilizado o teste ANOVA twoway. Resultados: Não houve alteração estatisticamente significativa na cor das bases das próteses de acrílico, indicando que o método é seguro para a desinfecção das próteses, em qualquer uma das soluções e situações testadas. Conclusões: A soluções testadas associadas ou não à cuba ultrassônica não promoveram alteração de cor nas bases acrílicas, podendo ser usadas pelo tempo determinado.

The protocols were evaluated: brushing (B), brushing and immersion in water (W), 0.25% sodium hypochlorite (SH), and 0.15% triclosan (T), simulating 0, 1, 3, and 5 years. The data were analyzed by ANOVA with repeated measurements, ANOVA (Three-way) and Tukey’s post-test, generalized linear model with Bonferroni adjustment, and ANOVA (Two-way) and Tukey’s post-test (α = 0.05). This study measured the self-efficacy of health professionals in hand hygiene (HH) and glove usage (GU) during the COVID-19 pandemic. It was an observational Brazilian multicenter study with a cross-sectional design with an online application of an instrument measuring the self-efficacy of health professionals in HH and GU. Health professionals (n = 193) participated in this study: 96 (49.7%) were nursing professionals, 38 (20.2%) were dental professionals, 21 (10.9%) were physicians, 10 (5.2%) were pharmacists, and 27 (14.0%) were other health professionals. A mandibular implant-retained overdenture is considered a first-choice treatment for edentulism. ... more A mandibular implant-retained overdenture is considered a first-choice treatment for edentulism. However, some aspects limit the use of standard implants-for example, the width of edentulous ridges, chronic diseases, fear, or costs. This randomized trial compared mandibular overdentures retained by 2 or 4 mini-implants with standard implants, considering oral health-related quality of life (OHRQoL), patient satisfaction, and complications such as lost implant. In sum, 120 edentulous men and women (mean age, 59.5 ± 8.5 y) randomly received 4 mini-implants, 2 mini-implants, or 2 standard implants. Participants provided data regarding OHRQoL and satisfaction until 12 mo. Clinical parameters, including implant survival rate, were also recorded. Both 2 and 4 mini-implants led to better OHRQoL, compared with 2 standard implants. Treatment with 4 mini-implants was more satisfying than 2 standard implants, with 2 mini-implants presenting intermediate results. Implant survival rate was 89%, 82%, and 99% for 4 mini-implants, 2 mini-implants, or 2 standard implants, respectively. Overdentures retained by 4 or 2 mini-implants can achieve OHRQoL and satisfaction at least comparable with that of 2 standard implants. However, the survival rate of mini implants is not as high as that of standard implants (ClinicalTrials.gov NCT01411683).

Complete denture fabrication involves a series of complex technical procedures. Nevertheless, sim... more Complete denture fabrication involves a series of complex technical procedures. Nevertheless, simplified methods may be as effective as conventional ones albeit the lesser use of time and resources, without disadvantage for the patient. This study compared a simplified method for complete denture fabrication to a conventional protocol in terms of oral health-related quality of life (OHRQoL), patient satisfaction and denture quality. Forty-two edentulous patients requesting treatment with complete dentures were randomly allocated into two study groups. Group S received dentures fabricated by a simplified method and Group C received conventionally fabricated dentures. Before interventions and after three and 6 months following insertion, OHRQoL and patient satisfaction were analysed by specific instruments. A prosthodontist assessed denture quality 3 months after delivery. Groups presented no difference for OHRQoL, denture quality and general satisfaction. Differences regarding patient satisfaction with some aspects of the dentures were found after 3 months (S &amp;amp;amp;amp;amp;amp;amp;amp;amp;gt; C), but were insignificant at 6 months. It was concluded that the simplified method is able to produce dentures of a quality comparable to those produced by the conventional method, influencing OHRQoL and patient satisfaction similarly.

Objectives: To compare a simplified method to a conventional protocol for complete denture fabric... more Objectives: To compare a simplified method to a conventional protocol for complete denture fabrication regarding masticatory performance and ability. Methods: A sample was formed by edentulous patients requesting treatment with maxillary and mandibular complete dentures. Participants were randomly divided into two groups: Group S, which received dentures fabricated by a simplified method, and Group C (n = 21 each), which received conventionally fabricated dentures. After three months following insertion, masticatory performance was evaluated by a colorimetric assay based on chewing two capsules as test food during twenty and forty cycles. Masticatory ability was assessed by a questionnaire with binary answers and a single question answered by means of a 0-10 scale. A third group (DN) formed by seventeen dentate volunteers served as an external comparator. Groups were compared by statistical tests suitable for data distribution (a = 0.05). Results: Thirty-nine participants were assessed for three months (twenty from Group C and nineteen from Group S). Groups C and S presented similar masticatory performance which corresponded to approximately 30% of Group DN. Results for masticatory ability showed similarity between S and C, regardless of the assessment method, although an isolate questionnaire item showed more favourable results for the first group. Conclusions: The simplified method for complete denture fabrication is able to restore masticatory function to a level comparable to a conventional protocol, both physiologically and according to patient's perceptions. Clinical significance: Although masticatory function is impaired by the loss of natural teeth and dentures can restore only a fraction of such function, patients can benefit from a simplified protocol for complete denture fabrication to the same extent they would by conventional techniques.

The aim of this study was to systematically examine the outcomes of interest of trials in removab... more The aim of this study was to systematically examine the outcomes of interest of trials in removable prosthodontics and to assess their overall quality. Material and methods: Electronic databases were searched up to August 2011 and complemented by hand searching for published randomized controlled trials (RCTs) in the field of removable prosthodontics in the six major prosthodontic and implant journals. The primary outcome of RCTs was considered as the outcome of interest of this review, and only manuscripts in which the study's primary outcome was reported in the abstract were included. Outcomes were classified according to the Strength of Recommendation Taxonomy (SORT) classification. The quality of individual reports of RCTs was assessed following the Cochrane Handbook for Systematic Reviews of Interventions (Higgins & Green, www.cochrane-handbook.org). Results: The search retrieved 86 reports of RCTs. Among these publications, 43% reported a patient-oriented primary outcome. Most of the publications did not present a clear description of sequence generation (54%) or allocation concealment (65%). Blinding of participants, clinicians, and outcomes assessors was not applicable or reported in most trials. Handling of withdrawals and losses was adequate in 52% of trials. To verify the effect of occlusion on joint sounds, temporomandibular joints (TMJ) were analyzed in 78 asymptomatic individuals with various dental conditions. Electrosonography was used to determine the intensity of the vibration in the temporomandibular joint (TMJ) on opening and closing the mouth. Transducers (piezoelectric accelerometer) were placed on the right and left joints. Results were tabled and analyzed using the Kruskal-Wallis test (a=0.05). It was concluded that TMJ vibration in partly edentulous individuals from Kennedy classes I, II and III is statistically higher than in dentate and fully edentulous subjects.

Purpose: This study aimed to quantify the costs of complete denture fabrication by a simplified m... more Purpose: This study aimed to quantify the costs of complete denture fabrication by a simplified method compared with a conventional protocol. Materials and Methods: A sample of edentulous patients needing conventional maxillary and mandibular complete dentures was randomly divided into group S, which received dentures fabricated by a simplified method, and group C, which received conventionally fabricated dentures. We calculated direct and indirect costs for each participant including unscheduled procedures. This study assessed 19 and 20 participants allocated into groups S and C, respectively, and comparisons between groups were conducted by the Mann-Whitney and Student's t-test (α = 0.05). Results: Complete denture fabrication demanded median time periods of 173.2 and 284.5 minutes from the operator for groups S and C respectively, and 46.6 and 61.7 minutes from the dental assistant (significant differences, p < 0.05). There was no difference between groups regarding postinsertion adjustments. Group S showed lower values for costs with materials and time spent by patients than group C during the fabrication stage, but not during adjustments. Conclusions: The median direct cost of complete denture treatment was 34.9% lower for the simplified method. It can be concluded that the simplified method is less costly for patients and the health system when compared with a conventional protocol for the rehabilitation of edentulous patients.

Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a benign lesion of the oral m... more Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a benign lesion of the oral mucosa of an unclear pathogenesis. We analyzed the profile of the inflammatory infiltrate in 12 cases of TUGSE by using immunohistochemical analysis and polymerase chain reaction-based repertoire analysis to detect T-and B-cell receptor gene rearrangements. The inflammatory infiltrate consisted in most cases of B and T lymphocytes, macrophages, abundant eosinophils, and large atypical cells. In 5 cases, CD30+ cells were found. Spectratyping analysis displayed a polyclonal rearrangement of the T-cell receptor γ gene in 6 cases and oligoclonality in 5 cases. Monoclonality was observed in 1 case that also fulfilled histologic criteria for lymphoma. Healing was uneventful in all cases, including the one suspected of being lymphoma, with no recurrences in more than 2 years' follow-up. TUGSE can be regarded as reactive. Some cases, however, may harbor a dominant clonal T-cell population; in these cases, long-term follow-up is mandatory.

However, some aspects limit the use of standard implants-for example, the width of edentulous ridges, chronic diseases, fear, or costs. This randomized trial compared mandibular overdentures retained by 2 or 4 mini-implants with standard implants, considering oral health-related quality of life (OHRQoL), patient satisfaction, and complications such as lost implant. In sum, 120 edentulous men and women (mean age, 59.5 ± 8.5 y) randomly received 4 mini-implants, 2 mini-implants, or 2 standard implants. Participants provided data regarding OHRQoL and satisfaction until 12 mo. Clinical parameters, including implant survival rate, were also recorded. Both 2 and 4 mini-implants led to better OHRQoL, compared with 2 standard implants. Treatment with 4 mini-implants was more satisfying than 2 standard implants, with 2 mini-implants presenting intermediate results. Implant survival rate was 89%, 82%, and 99% for 4 mini-implants, 2 mini-implants, or 2 standard implants, respectively. Overdentures retained by 4 or 2 mini-implants can achieve OHRQoL and satisfaction at least comparable with that of 2 standard implants. However, the survival rate of mini implants is not as high as that of standard implants (ClinicalTrials.gov NCT01411683).

Nevertheless, simplified methods may be as effective as conventional ones albeit the lesser use of time and resources, without disadvantage for the patient. This study compared a simplified method for complete denture fabrication to a conventional protocol in terms of oral health-related quality of life (OHRQoL), patient satisfaction and denture quality. Forty-two edentulous patients requesting treatment with complete dentures were randomly allocated into two study groups. Group S received dentures fabricated by a simplified method and Group C received conventionally fabricated dentures. Before interventions and after three and 6 months following insertion, OHRQoL and patient satisfaction were analysed by specific instruments. A prosthodontist assessed denture quality 3 months after delivery. Groups presented no difference for OHRQoL, denture quality and general satisfaction. Differences regarding patient satisfaction with some aspects of the dentures were found after 3 months (S &amp;amp;amp;amp;amp;amp;amp;amp;amp;gt; C), but were insignificant at 6 months. It was concluded that the simplified method is able to produce dentures of a quality comparable to those produced by the conventional method, influencing OHRQoL and patient satisfaction similarly.

Objectives: To compare a simplified method to a conventional protocol for complete denture fabric... more Objectives: To compare a simplified method to a conventional protocol for complete denture fabrication regarding masticatory performance and ability. Methods: A sample was formed by edentulous patients requesting treatment with maxillary and mandibular complete dentures. Participants were randomly divided into two groups: Group S, which received dentures fabricated by a simplified method, and Group C (n = 21 each), which received conventionally fabricated dentures. After three months following insertion, masticatory performance was evaluated by a colorimetric assay based on chewing two capsules as test food during twenty and forty cycles. Masticatory ability was assessed by a questionnaire with binary answers and a single question answered by means of a 0-10 scale. A third group (DN) formed by seventeen dentate volunteers served as an external comparator. Groups were compared by statistical tests suitable for data distribution (a = 0.05). Results: Thirty-nine participants were assessed for three months (twenty from Group C and nineteen from Group S). Groups C and S presented similar masticatory performance which corresponded to approximately 30% of Group DN. Results for masticatory ability showed similarity between S and C, regardless of the assessment method, although an isolate questionnaire item showed more favourable results for the first group. Conclusions: The simplified method for complete denture fabrication is able to restore masticatory function to a level comparable to a conventional protocol, both physiologically and according to patient's perceptions. Clinical significance: Although masticatory function is impaired by the loss of natural teeth and dentures can restore only a fraction of such function, patients can benefit from a simplified protocol for complete denture fabrication to the same extent they would by conventional techniques.

Material and methods: Electronic databases were searched up to August 2011 and complemented by hand searching for published randomized controlled trials (RCTs) in the field of removable prosthodontics in the six major prosthodontic and implant journals. The primary outcome of RCTs was considered as the outcome of interest of this review, and only manuscripts in which the study's primary outcome was reported in the abstract were included. Outcomes were classified according to the Strength of Recommendation Taxonomy (SORT) classification. The quality of individual reports of RCTs was assessed following the Cochrane Handbook for Systematic Reviews of Interventions (Higgins & Green, www.cochrane-handbook.org). Results: The search retrieved 86 reports of RCTs. Among these publications, 43% reported a patient-oriented primary outcome. Most of the publications did not present a clear description of sequence generation (54%) or allocation concealment (65%). Blinding of participants, clinicians, and outcomes assessors was not applicable or reported in most trials. Handling of withdrawals and losses was adequate in 52% of trials. To verify the effect of occlusion on joint sounds, temporomandibular joints (TMJ) were analyzed in 78 asymptomatic individuals with various dental conditions. Electrosonography was used to determine the intensity of the vibration in the temporomandibular joint (TMJ) on opening and closing the mouth. Transducers (piezoelectric accelerometer) were placed on the right and left joints. Results were tabled and analyzed using the Kruskal-Wallis test (a=0.05). It was concluded that TMJ vibration in partly edentulous individuals from Kennedy classes I, II and III is statistically higher than in dentate and fully edentulous subjects.

Purpose: This study aimed to quantify the costs of complete denture fabrication by a simplified m... more Purpose: This study aimed to quantify the costs of complete denture fabrication by a simplified method compared with a conventional protocol. Materials and Methods: A sample of edentulous patients needing conventional maxillary and mandibular complete dentures was randomly divided into group S, which received dentures fabricated by a simplified method, and group C, which received conventionally fabricated dentures. We calculated direct and indirect costs for each participant including unscheduled procedures. This study assessed 19 and 20 participants allocated into groups S and C, respectively, and comparisons between groups were conducted by the Mann-Whitney and Student's t-test (α = 0.05). Results: Complete denture fabrication demanded median time periods of 173.2 and 284.5 minutes from the operator for groups S and C respectively, and 46.6 and 61.7 minutes from the dental assistant (significant differences, p < 0.05). There was no difference between groups regarding postinsertion adjustments. Group S showed lower values for costs with materials and time spent by patients than group C during the fabrication stage, but not during adjustments. Conclusions: The median direct cost of complete denture treatment was 34.9% lower for the simplified method. It can be concluded that the simplified method is less costly for patients and the health system when compared with a conventional protocol for the rehabilitation of edentulous patients.

Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a benign lesion of the oral m... more Traumatic ulcerative granuloma with stromal eosinophilia (TUGSE) is a benign lesion of the oral mucosa of an unclear pathogenesis. We analyzed the profile of the inflammatory infiltrate in 12 cases of TUGSE by using immunohistochemical analysis and polymerase chain reaction-based repertoire analysis to detect T-and B-cell receptor gene rearrangements. The inflammatory infiltrate consisted in most cases of B and T lymphocytes, macrophages, abundant eosinophils, and large atypical cells. In 5 cases, CD30+ cells were found. Spectratyping analysis displayed a polyclonal rearrangement of the T-cell receptor γ gene in 6 cases and oligoclonality in 5 cases. Monoclonality was observed in 1 case that also fulfilled histologic criteria for lymphoma. Healing was uneventful in all cases, including the one suspected of being lymphoma, with no recurrences in more than 2 years' follow-up. TUGSE can be regarded as reactive. Some cases, however, may harbor a dominant clonal T-cell population; in these cases, long-term follow-up is mandatory.
