High energy blunt trauma and falling from height are usually the main mechanism of injury. It is a retrospective study that was performed in academic level I trauma center. From September 2015 to December 2019, a consecutive series of 26 patients (7 females), with average age of 32 years with unstable pelvic fracture were included in this study. All patients underwent reduction and percutaneous fixation with one sacroiliac screw posteriorly for each sacroiliac joint and anterior arch fixation of the pelvic ring with either platting or external fixator. The average duration of follow up was 36 months. Clinical results: at the final follow-up, all patients had complete radiological healing of the fractures. The subjective functional assessment yielded excellent for twenty-two patients, good for two patients and fair for two patients. Posterior arch fixation of th...

Former security analyses have suggested that the gateways in LoRaWAN in their current state are susceptible to a wide variety of malicious attacks, which can be notoriously difficult to mitigate since gateways are seen as obedient relays by design. These attacks, if not addressed, can cause malfunctions and loss of efficiency in the network traffic. As a solution to this unique problem, this paper presents a novel certificate authentication technique that enhances the cyber security of gateways in the LoRaWAN network. The proposed technique considers a public key infrastructure (PKI) solution that considers a two-tier certificate authority (CA) setup, such as a root-CA and intermediate-CA. This solution is promising, as the simulation results validate that about 66.67% of the packets th...

The management of water resources in dry areas is necessary to maintain the limited quantities of water available and to achieve an appropriate level of development, food security and stability. The current study aims to estimate the water footprint (WF) of Egyptian crops in the old and new lands inside and outside the Nile Valley and Delta. In addition, comparison of the WF of Egyptian crops with the global WF, and calculating the economics of the green and blue WF of Egyptian crops. Methodology of assessment was adopted from the water footprint assessment manual. Meteorological data during the study period were obtained from the weather station at Agricultural Research Center, and from Egyptian Meteorological Authority. Data of crop productivity were obtained from the Economic Affairs Sector-Ministry of Agriculture and Land Reclamation. The results showed that the contribution of green water to the total water footprint of Egyptian crops was small compared to blue water. Average total green plus blue water footprint (WF green+blue) of Egyptian crops was about 680 m 3 /ton. Egyptian crops recorded low WF values compared to the average global WF. In addition, average net return of the WF green+blue of Egyptian crops amounted to 10.25, 5.23 and 5.00 LE/m 3 for winter field crops and vegetables, summer field crops and vegetables, Nili and perennial crops, respectively.

Vaccine hesitancy remains a complex matter influenced by multiple factors, especially in sub-Saharan Africa. Methods: We conducted a cross-sectional study between November 2021 and January 2022 among the general adult public seeking care at six different healthcare facilities in Kenya. The survey, in English, consisted of questions based on demographics, knowledge, and attitudes, including hesitancy towards the COVID-19 vaccine. Results: Of the 3996 surveys collected, 55.1% were from private, 19.5% from faith-based and 25.3% from government facilities., Approximately 81.0% of all the participants reported it was important to get a vaccine to protect other people from COVID-19, 79.9% reported they would take a vaccine to protect against COVID-19, yet 40.5% reported being hesitant to take the vaccin...

This study aimed to explore whether SCD patients have the same DM prevalence as the general population in a country with a high prevalence of DM. This cross-sectional study included all SCD adult patients admitted to Salmaniya Medical Complex, Bahrain, between 2003 and 2010 (n = 2,204). A random sample (n = 520) was taken to establish the prevalence of DM. Laboratory records were examined to determine the presence of DM. There were 376 SCD patients with complete records; of these, 24 (6.4%) had DM. The age- and sex-standardised prevalence of DM was 8.3%. While the prevalence of DM in SCD patients in Bahrain was high, it was lower than expected in this population. SCD may have a protective effect towards DM development. However, the impact of these two conditions on vascular diseases suggest a need for screening and aggressive treatment in this population.

Research paper thumbnail of Optimising Multimodality Treatment of Resectable Oesophago-Gastric Adenocarcinoma

Although there has been an enormous progress in the multimodality management of resectable oesophago–gastric adenocarcinoma, most patients still develop a recurrent disease that eventually becomes resistant to systemic therapy. Currently, there is no global consensus on the optimal multimodality approach and there are variations in accepted standards of care, ranging from preoperative chemoradiation to perioperative chemotherapy and, more recently, adjuvant immune checkpoint inhibitors. Ongoing clinical trials are aimed to directly compare multimodal treatment options as well as the additional benefit of targeted therapies and immunotherapies. Furthermore, our understanding of the molecular and genetic features of oesophago–gastric cancer has improved significantly over the last decade and these data may help inform the best approach for the individual patient, utilising bio...

Atmosphere, 2021

Monitoring the impact of changes in land use/land cover (LULC) and land surface temperature (LST)... more Monitoring the impact of changes in land use/land cover (LULC) and land surface temperature (LST) is of great importance in environmental and urban studies. In this context, this study aimed to analyze the dynamics of LULC and its impact on the spatiotemporal variation of the LST in the two largest urban cities in Syria, Damascus, and Aleppo. To achieve this, LULC changes, normalized difference vegetation index (NDVI), and LST were calculated from multi-temporal Landsat data for the period 2010 to 2018. The study revealed significant changes in LULC, which were represented by a decrease in agricultural land and green areas and an increase in bare areas in both cities. In addition, built-up areas decreased in Aleppo and increased in Damascus during the study period. The temporal and spatial variation of the LST and its distribution pattern was closely related to the effect of changes in LULC as well as to land use conditions in each city. This effect was greater in Aleppo than in Dam...

Methods: A total of 401 children from four primary schools in kassala were random simple stratified selected during a period of two months and prevalence of anaemia was estimated, clinically and by measuring haemoglobin concentration. Results: Clinical examination revealed anaemia in 373 of the students and haemoglobin estimations proved anaemia in 93% of the students enrolled in the study (Hb. less than 13.5 g/dl). Conclusion : The study showed high prevalence of anaemia among schoolchildren in the study area, so we recommend that more studies must be done in this area and the heath authorities may interfere by heath education and other interventional methods (e.g. provision of iron and folate).

The operative management of these types of fractures is often difficult and technically challenging. Almost all of these types of fractures are treated through extensile approaches, and a variety of them have been described. Some of these approaches are associated with complications such as infection, heterotopic ossification, and increased blood loss in addition to increased operative time. This retrospective study was performed at an academic Level I trauma center. From January 2013 to March 2019, 21 patients with complex acetabular fractures were treated with open reduction and internal fixation through a single anterior modified Stoppa approach. The average operative time was 98.80 minutes (range, 60-180 minutes), the average follow-up time was 40 months (range, 24-72 months), and the average Harris Hip Score (HHS) was 87.85 (range, 75-100). At the latest follow-up, 12 (57.1%) patients had an excellent HHS, 8 (38.1%) patients had a good HHS, and 1 (4.8%) patient had a fair HHS. Complex acetabular fractures can be effectively managed by a modified Stoppa approach without the need for extensile or combined approaches. [Orthopedics. 2021;44(3):e353-e358.].

This study also analyzed the relationship between changes in forest cover and selected physical and socio-demographic variables associated with the drivers of change. The results revealed that the study area witnessed a significant decrease in the total forest area (31,116.0 ha, 24.3%) accompanied by a considerable decrease in density, as the area of dense forests decreased by 11,778.0 ha (9.2%) between 2010 and 2020.
