Depuis l'introduction de la vaccination anti-haemophilus, le pneumocoque est le germe le plus... more Depuis l'introduction de la vaccination anti-haemophilus, le pneumocoque est le germe le plus frequent des meningites purulentes du nourrisson et du jeune enfant. La frequence croissante des souches de pneumocoques de sensibilite anormale a la penicilline a necessite la mise en place de protocoles associant une cephalosporine de 3 e generation a la vancomycine ou a la rifadine. La mortalite (12%) et la morbidite (15 a 30%) sont independantes de la CMI du germe. Le delai diagnostique et therapeutique reste l'element pronostic essentiel. Chez les plus grands enfants, le meningocoque de type B reste majoritaire. La mortalite de ces meningococcemies est liee aux troubles hemodynamiques des 48 premieres heures. La prevention des infections meningees repose sur le respect du calendrier vaccinal (vaccination pentavalente a partir du 2 e mois) et une antibiotherapie adaptee des infections de l'oreille moyenne.

Les caracteristiques cliniques sont insuffisantes pour determiner les enfants a risque. Des examens complementaires sont donc necessaires afin que ne soient hospitalises que les enfants chez lesquels la probabilite d'une infection severe a ete etablie. Ce mode de fonctionnement impose des contraintes de structure et de fonctionnement des services d'urgences pediatriques mais permet d'eviter de nombreuses journees d'hospitalisation inutiles.

Les intoxications accidentelles chez l'enfant sont nombreuses et restent un motif frequent de... more Les intoxications accidentelles chez l'enfant sont nombreuses et restent un motif frequent de consultation en urgence. Les auteurs ont etudie retrospectivement 239 intoxications aigues vues en un an au Centre d'Urgence et de Diagnostic Rapide. L'analyse clinique et epidemiologique montre que les accidents surviennent aux heures des repas, chez le garcon (55,6 %), de moins de 5 ans (80,3 %). Il s'agissait d'une intoxication medicamenteuse une fois sur deux, d'un toxique solide ou volatil non medicamenteux une fois sur trois, d'un corps etranger digestif une fois sur six. Parmi les medicaments les anti-inflammatoires (19,7 %), les cardiotropes (19,7 %), les sedatifs non barbituriques non benzodiazepines (15,6 %), les benzodiazepines (11,5 %) sont les 4 familles les plus souvent en cause. UNLABELLED Meningitis due to Escherichia coli is rare, and generally observed in very particular ... more UNLABELLED Meningitis due to Escherichia coli is rare, and generally observed in very particular circumstances, such as neonatal period, anatomical anomalies or in immune-deficient patients. CASE REPORT A 16-month-old male infant was admitted for acute meningitis. E coli was detected in the cerebro-spinal fluid (CSF). As appropriate antibiotic treatment proved inefficient, a cerebral computerised tomography (CT) scan was performed, revealing an epidermoid cyst of the posterior fossa. The cyst was resected after CSF sterilisation. Postoperative recovery was satisfactory. CONCLUSION When an unusual bacterial species such as E coli is detected in CSF, the authors suggest consideration of a cutaneous or ETN focus, or a congenital malformation with communication between cutaneous and meningeal structures.

BACKGROUND Routine detection of maternal sensitization during pregnancy sometimes reveals alloimm... more BACKGROUND Routine detection of maternal sensitization during pregnancy sometimes reveals alloimmunization by exceptional antigens. CASE REPORT A first pregnancy was complicated by a severe post-partum anemia in the mother, that required a blood transfusion. Irregular agglutinins were detected during the first trimester of a second pregnancy, for which the father was different from the first. The specific antibody was not identified at that time. The newborn, born at a gestational age of 39 weeks, developed severe jaundice at 3 hours of life, with hemolytic disease, anemia and hepatomegaly. Therapy included two transfusions of packed, washed red cells obtained from the mother on days 7 and 25. Immunologic tests showed that the hemolytic disease of this newborn was due to an anti-Vel alloimmunization. CONCLUSION Antibodies detected during the pregnancy must be identified in order to manage properly any perinatal problems due to rare antibodies.

Vasodilators, inotropic agents and diuretics are the main therapeutics used in the symptomatic treatment. Guidelines for the emergency treatment according to the main etiologies are presented.
L'augmentation du nombre de consultations pour asthme des enfants âges de plus de 2 ans suit ... more L'augmentation du nombre de consultations pour asthme des enfants âges de plus de 2 ans suit l'evolution des consultations hospitalieres. Les crises sont plus frequentes en automne et en hiver chez les plus jeunes. Au-dela de l'âge de 10 ans, cette variation saisonniere disparait. L'implication d'une equipe de pneumologie dans la formation des medecins des urgences et la prise en charge des enfants permet une reduction sensible du nombre des hospitalisations et la mise en place de nouveaux protocoles therapeutiques.

Injury to the spinal cord is still observed in the neonate. Its prognosis is poor. The first neon... more Injury to the spinal cord is still observed in the neonate. Its prognosis is poor. The first neonate was delivered by cesarean section for breech presentation with hyperextension of the neck. She rapidly developed acute respiratory distress and paraplegia. MRI showed spinal cord hemorrhage involving the cervical and upper thoracic cord with rupture of the cord. The patient died a few weeks later. The second neonate was delivered vaginally in breech presentation without any difficulty. She progressively developed tetraplegia evolving into spasticity. MRI showed stretching of cervical spinal cord. The patient later developed sphincter disturbances, repeated urinary and pulmonary infection and severe scoliosis. Early prenatal damage to the spinal cord was possible in the first patient. Ultrasonography could help to evaluate the extent of damage. The condition leads to difficult ethical and therapeutic problems.
