The purpose of this review is to summarize the pathophysiology of ejaculation and look into preva... more The purpose of this review is to summarize the pathophysiology of ejaculation and look into prevalence, aetiology, diagnosis, and treatment of painful ejaculation. We carried out a comprehensive search of PubMed in order to look for literature on male painful ejaculation using keywords post-orgasmic pain, painful ejaculation, dysejaculation, odynorgasmia, post-orgasmic pain, or dysorgasmia. Painful ejaculation has an alarming prevalence throughout the world, between 1 to 25%. It has a detrimental effect on patients' quality of life as it reduces individual self-esteem and is associated with sexual dysfunction. Its aetiology includes simple infection or inflammation of the urinary tract, benign prostate hyperplasia, ejaculatory duct obstruction, post-radical prostatectomy and side effects of certain medications. Once reported, it should be investigations and treatments should be tailored according to the etiology. Both medical and surgical treatment is available depending on the cause of painful ejaculation. Due to the sensitive nature of its presentation, it is a symptom that can be identified best when specifically asked. Our understanding regarding painful ejaculation is very limited and only a few articles have revealed insight into this topic. Further research is required in order to set proper guidelines for diagnosis and treatment of painful ejaculation.

Objectives: There are many grey zones in the field of penile rehabilitation after radical prostat... more Objectives: There are many grey zones in the field of penile rehabilitation after radical prostatectomy. One of the most important aspect for improve the patient's compliance to the treatment is the preservation of the full dimensions of the penis after surgery. We show the first cases series of patients treated by laparoscopic extra-peritoneal radical prostatectomy (RP) and simultaneous penile prostheses implantation (PPI), in order to preserve the full length of the penis and improve patients' satisfaction. Materials & Methods: From June 2013 to June 2014, 10 patients underwent simultaneous PPI (AMS "inibizone"), how shown in Figure 1 and RP by the same surgeon at the same Centre. Patients were evaluated with urological visits, dedicated questionnaires and instrumentation before surgery, at the hospital discharge and on day 21-28, each 3 months for the first year and each 6 months after. The main outcome measures were oncological (biochemical recurrence free rate), functional (penile length) and QoL. Results: 10 patients [median age of 61.1 (56-65)] completed the study follow-up period (median 41 months). The median penile length at the time of the surgery was 9 cm and 8.9 cm after 2 years follow-up. The pre-surgery SF-36 median value was 97.7 (96.8-98.6). Pts were satisfied with their penile implants, and the couple level of sexual satisfaction was rated 8.5 (7-10). The median SF-36 score was 98.9 (97.8-99.4). It is far from over and yet, it has affected every aspect of human life. The Department of Urology at King's College Hospital London provides all types of urology care ranging from benign to cancer treatments to the community. However, this service was badly affected by COVID-19. Policies were made by the experts in the field to reduce patient traffic in the hospital and at the same time, attempting to ensure appropriate and timely treatment was provided to patients suffering from urological conditions requiring urgent attention. In this article, we discuss the triage guidelines set up at our centre. Treatments for benign conditions such as kidney stones were delayed for 3-6 months.
