Hope is essential in the experience of living with advanced chronic disease. Maintaining hope in ... more Hope is essential in the experience of living with advanced chronic disease. Maintaining hope in situations of physical, psychological, spiritual and social suffering requires expertise in managing hope enabling and inhibiting factors'. Its' balance can be very difficult to achieve without the help of professionals, nurses in particular. The focus on hope promotion is imperative in caring for patients at end of life. Fostering hope in patients can result in comfort and quality of life. However interventions designed to promote hope are often nonspecific in its nature, and often don't address the real needs of patients. On the other hand, as they are nonspecific, their effectiveness is hard to measure. Aiming to develop and test a program to promote hope for people with advanced disease, we conducted four studies The first was a cross cultural validation into Portuguese of HHI to measure hope; HCQ to measure comfort, and MQOL to measure quality of life in a sample of 205 palliative patients in different contexts. The results confirm that the all Portuguese versions are valid good psychometric properties. They fit the culture and are suitable for measuring the effectiveness of nursing interventions. The second study aimed to characterize hope, comfort and quality of life (QOL) in the same sample, to identify the relationship between these variables and their mediators. The results allowed us to identify the needs of patients regarding hope promotion and identify socio demographic and clinical variables that impact in their hope. These factors were taken into consideration and incorporated in the design of the intervention program. The objective of the third study was to construct a program of nursing intervention to promote hope from the literature review and the results of the previous study. The Hope Promotion Program (EPP) satisfies the assumptions of nursing interventions effective. In the fourth study we tested the effectiveness of PPE variables in hope, comfort and QOL, in a longitudinal experimental design (pretest-post test) over a month. A sample of 26 palliative patients was randomized into two equivalent groups. The intervention took place at patients home. The results indicate that PPE is effective in promoting hope, improving comfort and QoL. The results contribute to a more effective practice, nursing education using autonomous intervention and to a better research.

Health Care professionals and organizations play a major role in promoting adherence among people with mental disorders. However, defining therapeutic adherence remains complex. We used Rodgers’ evolutionary concept analysis to explore the concept of therapeutic adherence in the context of mental health. We conducted a systematic literature search on Medline/PubMed and CINAHL for works published between January 2012 and December 2022. The concept analysis showed that major attributes of therapeutic adherence include patient, microsystem and meso/exosystem-level factors. Antecedents are those related to patients, such as their background, beliefs and attitudes, and acceptance of mental illness–and those related to patient-HCP therapeutic engagement. The lack of available and accessible in-person counselling resources is often reported by ICs. Online resources can promote early access to help and support for patient-IC dyads in palliative care. Over a two-week period, five 2-hour online journal club sessions were conducted in April 2020. During each session, five journal articles were presented synchronously to a live online audience via the Zoom Classroom technology. To better inform practice and theory on hope, palliative care research should include both patients and their family carers, given their strong interdependence. The aim of this study was to explore how hope is experienced in dyads formed by end-of-life patients and their family carers. In this qualitative study, data were collected by in-depth interviews with seven Portuguese family dyads. Analysis followed a thematic analysis approach. The analysis of the interviews shed light on the importance of hope for all participants, and the challenges involved. Family dyads noted several barriers and facilitators to perceptions of hope. Barriers to hope included limitations imposed by illness, feelings of anguish and helplessness, and poor communication with clinicians. They often work long hours, face emotional stress, and are at a high risk of psychosocial and somatic illnesses. Nurses sometimes fall sick but work regardless, leading to presenteeism and subsequent risks to quality of care and patient safety due to the increased likelihood of patients falling, medication errors, and staff-to-patient disease transmission. This paper describes the development and testing of the General Public's Attitudes Toward Advance Care Directives (GPATACD) Scale. Design and methods Methodological study. The development of the instrument was based on a literature review, updated in 2018. Face and content validity were verified by an expert panel and piloted among six participants. Data were collected in an online survey of 1024 Portuguese adults. The obtained data were analyzed using Varimax rotation, while the reliability was evaluated by calculating Cronbach's alpha. Results The scale achieves good Item-Content Validity Index (I-CVI) values, between 0.89 and 1.00, and scale-CVI values of 0.91. A principal component analysis generated four dimensions with 26 items as a final scale, with overall Cronbach's alpha of 0.848. Conclusions The findings demonstrate that the scale is valid...

This study evaluates the knowledge of palliative care among a sample of Portuguese adults and correlates their level of knowledge with age, gender, profession, and experience of family member’s palliative care. Method: A cross-sectional online survey was carried out on a sample of 152 HCPs and 440 NHCPs who completed an anonymous questionnaire of sociodemographic, family, and professional data, and an instrument of 26 dichotomous (true or false) questions focusing on palliative care goals and purposes. Results: The 592 participants had a mean age of 31.3 ± 11.1 years, and most were female. Statistically significant differences between statements considered as correct by HCPs and NHCPs were found in 24 statements; HCPs had the highest percentage of correct answers. It also aims to assess the relationship between the degree of dependency of each self‐care domain, the length of admission, the length of dependency and the age of the dependent person at the time of discharge.MethodsA cross‐sectional study was conducted. The sample comprised hospitalised adults and elderly in the medical services of a Portuguese hospital during the months of March, April and May of 2018.
